What the care home does well

Sutton Court Residential Home offers an exceptionally pleasant and homely environment for service users. Management and staff within the home are committed to providing an environment that is conducive to achieving optimum independence, comfort and enjoyment for the service user. Staff employed at the home are provided with the opportunity to attend training courses to promote the safety and well being of the service users. All documentation appertaining to the provision of training courses is clear, concise and comprehensive. It is evident that service users are very satisfied with the health care provision afforded to them at the home together with the homes comprehensivefacilities. All service users spoken with stated that staff within the home respected their privacy and dignity at all times. Service users are provided with a varied and stimulating environment and are actively encouraged to develop and maintain contact within the broader community. Transport is available to the service users to access activities beyond the homes immediate environment.

What has improved since the last inspection?

It was evident from the last inspection report that on some care planning documentation the staff used a form of "shorthand" which they understand but is not immediately clear to others, instead of recording a negative the initials n/a were used. It was evident that the manager of the home has addressed this practice effectively and abbreviations were not evident within the case tracked notes. Previously the home had so many different plans for each resident it was difficult to gather concise information and the daily notes did not bear any relationship to care. It was evident that the care plans are now maintained very effectively and the evaluations of the care plans are appropriate and effective.

What the care home could do better:

Sutton Court Residential Home continues to provide an excellent service to its residents and the inspection process did not identify area areas of concerns on this occasion. Service users spoken with were very complimentary in relation to all aspects of care afforded to them at the home and had no concerns whatsoever.

With easy access for service users, the home is only a few minutes walk from local amenities including a supermarket and newsagent. The accommodation comprises of two floors with a specialist 12-bedded area for people with dementia. All bedrooms had en - suite facilities and there were 7 lounge / quiet areas and a large and small dining room. In addition the home had some shared bathing facilities and extensive garden areas. The home also boasts a swimming pool, Jacuzzi and coffee bar area for the service users enjoyment. The unannounced inspection took place over a 4 hour period and involved one inspector. The main method of inspection was case note tracking, this is a method of selecting service users within the home and discussing with them their expectations and experiences of living within the home environment. The case tracking method also analyses the records of the service users to ascertain if the service users identified needs are being addressed appropriately within the care home setting and that their safety and well being is being maintained. On this occasion two service users notes were case tracked. Also as part of the case tracking process, service users within the home are informally interviewed to further evidence the quality of care afforded to them. At the time of the inspection a total of 47 service users were accommodated at the home. It was evident that the management and staff within the home are very committed to providing an exceptionally high standard of care for the service users within an environment that is maintained to a very high standard. The manager and staff within the home were very helpful and accommodating thus ensuring that the inspection process progressed in a professional and efficient manner. Service users spoken with were very complimentary and happy with the care service they receive at the home and no concerns were voiced whatsoever. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3. 6. It was evident that no service users gain residency at the home before a thorough needs assessment has been performed and have been assured that the identified needs can be met at the home. It was established that Sutton Court does not provide intermediate care services. EVIDENCE: The pre-admittance assessments within the two case tracked notes were detailed in identifying the specific needs of the service users to maintain optimum independence within the home. The pre admittance assessment documentation is clear, concise, signed and dated by the assessor. The pre admittance assessment documentation is stored with the care plans appertaining to individual service users and as such it was easily established that the identified needs were being met effectively at the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8. The service users health, personal and social care needs are set out in well maintained, individual care plans that addresses identified needs gleaned from the pre-admittance assessments, as such it was evident that service users identified needs are fully met at the residential home. EVIDENCE: The manager at Sutton Court ensures that care plans are effectively maintained in a clear, concise and very well organised manner. It could be clearly evidenced that service users needs were assessed holistically and all identified needs were addressed effectively within the care planning process. It was evident that the care plans are evaluated on a monthly basis or as required so that the changing needs of service users are fully met. As far as practically possible all service users or relatives were involved in the care planning process, this was evidenced by service users or their representatives signatures within the relevant documentation. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12. 14. Service users confirmed that they are afforded the choice to participate in varied and stimulating social activities both within the homes environment and within the broader community. EVIDENCE: The pre admittance assessment allows for the identification of service users hobbies and interests which staff members within the home will attempt to facilitate if at all possible. The home employs a community liaison officer on a part timer basis, who is responsible for the provision of varied and stimulating social activities and interactions beyond the home immediate environment. Within the home service users can choose to participate in activities such as knitting, bingo, colouring and film afternoons. A variety of guest entertainers visit the home on a regular basis to provide entertainment. Should the service users wish, they can visit the Sutton in Ashfield town centre to shop or browse. Transport is available to take service users to local areas of interest and the service users spoken with stated that they particularly enjoy the outings. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16. 18. Sutton Manor utilises an effective complaints procedure to ensure the protection of the vulnerable adult. EVIDENCE: In an attempt to facilitate an effective complaints procedure the manager at Sutton Manor ensures that the complaints procedure utilised by Ashmere Care Homes Group is on display at the entrance hall to the home. The complaints procedure is also included within the admission documentation given to service users and their representatives at the time of admission. At the time of the inspection the manager of the home verified that no complaints are being investigated and the Commission for Social Care Inspection had not received any complaints. The service users spoken with were very complimentary in relation to the abilities of the management and care team at the home. Service users felt confident that the manager and the care staff would always address any complaints or concerns in a sensitive and professional manner. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26. Sutton Court provides an exceptionally well-maintained, clean and pleasant environment for service users enjoyment. EVIDENCE: A partial inspections of the home evidenced that Sutton Court provides a very spacious, exceptionally clean and well-decorated environment for service users. The home benefits from a secure, very well maintained garden area in which patio furniture is available for the service users to enjoyment in the summer months as they wish. Service users spoken with stated that the exceptionally high standards of hygiene and cleanliness are the “norm” at the home and have never had any concerns in relation to the internal or external environment of the home. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Staffing levels are appropriate in meeting the holistic needs of the service users at Sutton Court. EVIDENCE: Staff employed at the home is sufficient to meet the needs of the service users and an appropriate skill mix was evidenced. It was evidenced that the manager and her deputy manager have initiated a 24 hour an call rota to ensure that a senior member of staff is available should any emergencies develop of the home. On the morning of the inspection 47 residential service users were accommodated at the home six carers, were on duty in the morning, throughout the afternoon six carers were on duty and 4 carers covered the night period. Several interactions between service users and carers at the time of the inspection, it was evident that service users were afforded appropriate health promoting interventions based on mutual respect and dignity. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33. 35. 38. The home is run in the best interests of the service users through the provision of an effective consultation process. Service users financial interests are safeguarded by the policies and procedures utilised at Sutton Manor. The manager at Sutton Manor Residential Home ensures that the health, safety and welfare of service users and staff are promoted and protected EVIDENCE: The manager, in ensuring that the home is run in the best interests of the service users performs a service user meeting on a bi-monthly basis. The meetings are intended for the identification of issues relating to the care provided at the home and allows for the discussion of plans for future Sutton Court Care Home DS0000008803.V279373.R01.S.doc Version 5.1 Page 18 developments such as refurbishments and social activities. The content of the resident’s forum are minuted effectively for future reference. At the time of the inspection it was evidenced that the service users monies are effectively managed. The home maintains separate written records of all monitory transactions and the service users monies are not “pooled” thus protecting the service users from financial abuse. It was also evident that secure facilities are available in service users rooms to ensure the safe-keeping of money and valuables. A partial inspection of the premises evidenced that the home is maintained to a very high standard throughout. The handyman performs comprehensive external and internal maintenance procedures to ensure all areas are safe and aesthetically pleasing for the service users. It was determined that appropriate precautions are taken in relation to the control of Legionalla contamination, the testing of emergency lighting, the testing of fire equipment and fire alarms. All equipment, designed to aid the mobility of the service users is serviced appropriately by external contractors thus ensuring the health and safety of the service users and staff alike are safeguarded. Electrical equipment utilised within the home had undergone the Portable Appliance Test (P.A.T) to ensure that it is in good working order and safe. Hot water outlet checks are performed on a monthly basis and window restrictor checks are also performed on a monthly basis together with the staff call system which is also checked monthly. In ensuring the health, safety and welfare of the service users are protected it was evident that care staff receive comprehensive training opportunities that includes mandatory training supported by wide-ranging additional training opportunities provided by an external training agency. .
