What the care home does well

It was evident that Sutton Lodge Care Home continues to provide a pleasant, clean and homely environment for its residents. An effective Management structure within the home ensures that the home continues to provides a safe environment for the residents enjoyment.Staff employed at Sutton Lodge Care Home benefit from ongoing training opportunities so as to promote the resident`s safety and well-being. It is established that residents are very satisfied with the care provision afforded to them at the home and all residents spoken with stated that staff respected their privacy and dignity at all times.

What has improved since the last inspection?

It is a requirement that residents only establish occupancy within a care home after a suitably qualified individual has performed a full pre-admittance needs assessment. At the previous inspection performed on 4th January 2006, minor shortfalls in relation to the pre-admittance assessment procedure at Sutton Lodge Care Home were identified. The case tracking process identified that the manager at the home has addressed the minor shortfall and it was evident that resident`s holistic needs are being fully assessed and met effectively at the home.

Service users shall be within categories of OP(43), DE(11) or PD(4) within a total registration of 43 4th January 2006 Date of last inspection Brief Description of the Service: Sutton Lodge is situated on a complex with two sister homes on Priestic Rd Sutton in Ashfield. It is registered to provide personal care for 43 service users. The home has 11 beds registered for dementia care and 4 for physically disabled service users. It is situated on a busy road, opposite a supermarket and close to local shops and public transport. There are very attractive gardens and patio areas to the side and rear of the home. Handrails are sited throughout the home. Hoists are provided and a stand -aid. Pictures/symbols are provided on rooms in the dementia unit to assist service users. A lift provides access to the first floor and call alarms are in place. Assisted bathing and toilets are provided. The fees charged at the home are currently £307-£349 for social service funded residents with privately funded residents fees ranging from £360 - £410 Sutton Lodge Care Home DS0000024664.V299576.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The unannounced inspection took place over a 4.5 hour period and involved one inspector. The main method of inspection was case note tracking, this is a method of selecting residents within the home and discussing with them their expectations and experiences of living within the home environment. The case tracking method also analyses the records of the residents to ascertain if the residents identified needs are being addressed appropriately within the care home setting and that their safety and well being is being maintained. On this occasion three residents notes were case tracked. Also as part of the case tracking process, a staff member within the home was informally interviewed to further evidence the quality of care afforded to the residents. It was evident that the registered manager and the care staff on duty at the time of the inspection are very committed to providing a high standard of care for the residents. The manager and staff within the unit were very helpful and cooperative thus ensuring that the inspection process progressed in a professional and efficient manner. A relative was interviewed at this inspection to glean further information as to the quality of care afforded to the service users. The relative stated that he felt confident that his relative was receiving a high standard of care from the dedicated team of staff. The manager of the home performs effective pre-admittance assessments to ensure identified needs can be met at the home. The home does not provide intermediate care services. EVIDENCE: The pre-admittance assessments within the three case tracked notes were detailed in identifying the specific needs of the service users to maintain optimum independence and health within the home. The assessment process identified the holistic needs of the residents and utilised the Activities of Daily Living (ADL) assessment tool. Residents assessments are stored securely to ensure that the residents confidentiality is maintained within the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. This judgement has been made from evidence gathered both during and before the visit to this service. Residents “holistic” needs are addressed effectively through an individual care planning process. Residents are protected by the homes policies and procedures in relation to dealing with medication. Residents at the home are treated with respect and dignity and their privacy is maintained effectively. EVIDENCE: On the day of the inspection three residents care plans were examined. The care plans were found to be clear, concise and well organised. It was evident that care plans are evaluated on a monthly basis so as to address any changes in the residents needs. It was evident that this requirement is being addressed effectively at the home. Residents and relatives spoken with stated that they have access to their care plans and have been involved in the care planning process. At the time of the inspection no service users were responsible for the selfadministration of medicines. It was established that should a service user wish to be independent in the administration of medicines the manager would perform a comprehensive risk assessment, if the service users was deemed as being safe, self-medication would be facilitated if at all possible. It was evident that the home had undergone a pharmacy inspection and training programme on 20th March 2006. The pharmacist did not express any concerns in relation to the medicine management at the home. All residents spoken with were positive about the way staff spoke to them The inspector witnessed interactions between residents and staff at the home and it was evident that staff promoted the principles of respect and dignity at all times. Residents stated that staff always knocked on the resident’s bedroom door before entering and also stated that the staff respected their privacy and dignity when bathing or performing personal care. Staff interviewed demonstrated a thorough knowledge of the principles of privacy, dignity and respect and were also able to express how these principles are maintained within the home environment. The manager confirmed that issues relating to the promotion of respect and dignity are addressed within the induction process and also discussed within supervision sessions which all staff receive on a bi-monthly basis. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. Quality in this outcome group is good. This judgement has been made from evidence gathered both during and before the visit to this service. Residents have the opportunity and choice to participate in varied and stimulating social activities. Residents are able to maintain contact with family, friends, and representatives from the local community as they wish. Residents can exercise choice and control over their lives. Residents are provided with a wholesome, appealing and balanced diet. EVIDENCE: Residents can benefit from varied social activities within the home such as bingo, cards and domino evenings, crafts, quizzes, guest performers and keep fit sessions. Sutton Lodge Care Home DS0000024664.V299576.R01.S.doc Version 5.2 Page 14 To promote interactions beyond the homes immediate environment a mini bus is available to the residents to access trips to the coast, local parks, areas of interest and religious events. On the day of the inspection several social activities were being advertised within the foyer of the home which included a summer fayre, a street party to celebrate the Queens birthday and a Football evening at which time residents will have beer and snacks it they wish. To further promote social interactions with members from the local community the home provides lunches, which are charged at £2.00 per person. To encourage attendance the home can provide transport to and from the home at an additional charge of 50 pence per person. At the time of the inspection a relative was visiting his mother. The inspector had a brief conversation with the relative and it was confirmed that the home operates an open policy in relation to visiting times. It was established that he had every confidence in the management at the home and he praised the staff at the home in providing an appropriate and safe environment for his mother. To ensure that residents can exercise choice and control over their lives the manager ensures that an effective consultation process is performed. Residents are encouraged to attend bi-monthly residents meetings to discuss social activities within the home and preferences in relation to the menu provision at the home. The residents meetings also allow for the identification of any concerns in relation to the care provision at the home and both residents and their relatives are encouraged to fill out questionnaires appertaining to the overall quality of care at the home. Residents spoken with were very well presented, dressed in appropriate clothing for the season. Residents stated that the laundry service is efficient, and it was evident that service users cloths are returned clean and well ironed. At the time of the inspection the lunchtime meal was being served. It was evident that the food provided at the home is wholesome, nutritionally appropriate and varied. The inspector sampled a meal on the day of the inspection. The meal consisted of meat pie and fresh vegetables, the meal was well presented and of a high standard. Residents spoken with stated that they always have a choice of meals and that daily menus are displayed appropriately for resident’s perusal. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18. Quality in this outcome group is good. This judgement has been made from evidence gathered both during and before the visit to this service. Residents and their relatives are confident that their complaints are listened to, taken seriously and acted upon. Residents are protected from abuse at the home. EVIDENCE: It was established that the manager at the home was not investigating any complaints at the time of the inspection. There have been no allegations of abuse since the last inspection and the Commission for Social Care Inspection has not received any complaints relating to the service provision at the home Complaint information is displayed in the foyer of the home and residents or their representatives are given the complaints procedure within the homes information pack and Statement of Purpose. An examination of the complaints records evidenced that complaints are documented effectively together with an appropriate record of the resulting outcomes and actions. Sutton Lodge Care Home DS0000024664.V299576.R01.S.doc Version 5.2 Page 17 The case tracked residents stated they felt safe within the home, and should they have any concerns they would feel confidant in discussing theirs concerns with any members of staff at the home. A relative of a resident stated that he had confidence in the managers ability to deal with any concerns he or his relative might have in relation to the care provision at the home. A staff member was spoken with at the time of the inspection and it was evident that she had and appropriate knowledge of the complaints procedure utilised at the home. The member of staff confirmed that she had received information appertaining to the principles of adult protection within her induction and it was evident that she was aware of the appropriate actions to be taken if she suspected abuse was happening at the home. This judgement has been made from evidence gathered both during and before the visit to this service. Residents at Sutton Lodge care home benefit from a safe, well-maintained environment Sutton Lodge care home provides a very pleasant, comfortable and clean environment for its residents. EVIDENCE: A partial tour of the premises was completed as part of this inspection. The accommodation was well decorated, very comfortably furnished and maintained to a good standard. It was evident that resident’s bedrooms are homely, safe and personalised. The case tracked residents had many personal possessions in their bedrooms that included family pictures, small items of furniture, television, radio and Sutton Lodge Care Home DS0000024664.V299576.R01.S.doc Version 5.2 Page 19 ornaments. To promote safety within the resident’s rooms window restrictors are in place and radiator guards, together with appropriately placed nurse call buttons to ensure staff can attend to residents needs effectively over the 24hour period. An examination of maintenance documentation evidenced that mobility equipment; gas servicing, electrical equipments, legionella prevention, fire alarm tests, hot water outlet monitoring and Environmental Health Visits are performed appropriately to ensure the home is safe for both residents and staff alike. All documentation relating to the routine maintenance within the home was well organised and easily accessible thus aiding the inspection process. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30. Quality in this outcome group is good. This judgement has been made from evidence gathered both during and before the visit to this service. The staffing levels and skill mix employed at Sutton Lodge is sufficient in meeting the residential needs of the 23 service users. Residents are in safe hands at all times. The recruitment policies and practices within the home maintains the residents safety. Care staff within the home receive appropriate levels of training to ensure the service users are in safe hands at all times. EVIDENCE: Staff employed at Sutton Lodge is sufficient to meet the needs of the 23 residents accommodated at the home and an appropriate skill mix was evidenced. On the morning of the inspection one senior carer and three carers were on duty. Throughout the afternoon period, one senior carer and three carers and were on duty and one senior carer with one carer covered the night period. Sutton Lodge Care Home DS0000024664.V299576.R01.S.doc Version 5.2 Page 21 Two Staff files were examined as part of the inspection process and it was evident that staff members only commence employment once satisfactory Protection of Vulnerable Adult (POVA) checks and Criminal Record Bureau (CRB) checks have been obtained together with two satisfactory references, thus protecting the residents from potential abuse. In ensuring that residents are safe and a high quality service is provided, the manager evidenced that appropriate mandatory training opportunities are available to care staff employed at the home. It was established that all new staff members received an appropriate induction programme which examines the principles of care, the role of the worker within the care setting, understanding the needs of residents within the care setting and maintaining safety at work. The documentation appertaining to training provision is well managed, clear and concise. The manager was able to evidence the statutory training courses that staff at the home have attended together with the planned training courses that the staff will attend within the next year. In ensuring that staff are fully informed of the needs of the residents the manager continues to operate an effective “key worker” system. Residents “key workers” are responsible, under the supervision of the manager, to ensure that the resident’s care needs are reviewed on a monthly basis so as to identify the changing needs of the residents and formulate appropriate care plans. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38. Quality in this outcome group is good. This judgement has been made from evidence gathered both during and before the visit to this service. It was evident that the manager at Sutton Lodge is professional, fit to be in charge, of good character and is able to discharge her responsibilities fully. The manager ensures that the home is run in the best interests of the service users. The manager ensures that the financial interests of residents are safeguarded at Sutton Lodge. The manager at Sutton Lodge ensures that the health and safety of residents and staff alike is promoted and protected within the homes environment. EVIDENCE: Sutton Lodge Care Home DS0000024664.V299576.R01.S.doc Version 5.2 Page 23 The manager of the home attended a Fit Person Interview at the Commission For Social Care Inspection in February 2005, at which time she was assessed as being suitable to perform a management role within the care home setting. It is the intention of the manager to commence the Registered Managers Award in January 2007 so as to build on her current NVQ level 4 qualification in care. Sutton Lodge is one unit within a larger three-unit business on the same site. The manager at Sutton Lodge liaises effectively with the managers of two other units so as to gain support as required from a team of managers who benefit from a wealth of experience within the health care setting. Effective communication processes are encouraged as the three managers perform weekly meetings, thus ensuring quality care provision is afforded to the residents. In the absence of a manager the day to day running of the unit is performed by a newly appointed deputy manager. It was evident the manager of the home maintains effective documentation appertaining to the needs and aspirations of the residents together with the maintenance programme within the home. At the time of the inspection it was evidenced that the service users monies are effectively managed. A clear record of monitory transactions could be evidenced. The case tracked residents, monies are securely stored, in individual plastic folders in a security numbered safe. Receipts are evident for all transactions performed, thus protecting the service users from financial abuse. It was evident that residents and visitors to the home continue to hold the manager of the home in high regard and they feel confident in her abilities to manage the home in a clear supportive and professional manner. In determining that the residents are safe within the homes environment a range of Health and Safety records were seen, relating to Fire Safety, Gas Safety Certificate, Control of Substances Hazardous to Health (COSHH) the maintenance of mobility equipment such as hoists and wheelchairs, Portable Appliance Tests (PAT), and the testing of emergency lighting. All were found to be satisfactory and the documentation relating to the maintenance of the home is well organised and managed effectively.
