The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Management and staff within the home are committed to providing an environment that is conducive to achieving optimum independence and comfort for the service users. It was evident that Sutton Lodge Care Home continues to provide a pleasant, clean and homely environment for service users. It was evidenced that staff employed at Sutton Lodge Care Home have attended appropriate training courses to promote the safety and well being of the service users and staff alike. It is established that service users are very satisfied with the residential care provision afforded to them at the home. All service users spoken with stated that staff within the home always respected their privacy and dignity.

What has improved since the last inspection?

Requirements formulated from the previous inspection have been addressed effectively at the home. The care plans examined through the case tracking process now contain details to instruct carers at the home on appropriate interventions to maximise the service users health and well being. At the previous inspections it was evident that service users full names were not always evident on care plans, the manager at the home has addressed this minor shortfall effectively. The "one off" medication receipt book now evidenced signatures to ascertain which staff member receives the medication into the home.

What the care home could do better:

It is a requirement that service users only establish residency within a care home only after a full pre-admittance needs assessment has been performed by a suitably qualified individual. The case tracking process identified minor shortfall in the pre-admittance procedure at Sutton Lodge Care Home and as such it could not be fully established if the service users holistic needs are being fully met at the home.

Service users shall be within categories of OP(43), DE(11) or PD(4) within a total registration of 43 10th November 2005 Date of last inspection Brief Description of the Service: Sutton Lodge is situated on a complex with two sister homes on Priestic Rd Sutton in Ashfield. It is registered to provide personal care for 43 service users. The home has 11 beds registered for dementia care and 4 for physically disabled service users. It is situated on a busy road, opposite a supermarket and close to local shops and public transport. There are very attractive gardens and patio areas to the side and rear of the home. Handrails are sited throughout the home. Hoists are provided and a stand -aid. Pictures/symbols are provided on rooms in the dementia unit to assist service users. A lift provides access to the first floor and call alarms are in place. Assisted bathing and toilets are provided. The unannounced inspection took place over a 4 hour period and involved one inspector. The main method of inspection was case note tracking, this is a method of selecting service users within the home and discussing with them their expectations and experiences of living within the home environment. The case tracking method also analyses the records of the service users to ascertain if the service users identified needs are being addressed appropriately within the care home setting and that their safety and well being is being maintained. On this occasion two residents notes were case tracked. At the time of the inspection a total of 23 residents were accommodated at the home. It was evident that the management and staff within the home are very committed to providing a high standard of care for the service users. The manager and staff within the unit were very helpful and cooperative thus ensuring that the inspection process progressed in a professional and efficient manner. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The case tracking process identified shortfalls in the pre-admittance procedure at Sutton Lodge Care Home and as such it could not be fully established if the service users holistic needs are being fully met at the home. EVIDENCE: The pre-admittance assessment documentation of two service users was inspected. It was established that the home utilises a relatively comprehensive assessment tool designed to identify the holistic needs of the service users but unfortunately for one case tracked service user the assessment tool had not been filled out effectively. The service users cognitive abilities, dietary needs, history of falls, foot care, personal safety, oral health, preferred social activities or religious needs had not been assessed effectively at the time of admission and as such it could not be fully established if the service users holistic needs are being fully met at the time of admission. The second case tracked service users had a more Sutton Lodge Care Home DS0000024664.V273028.R01.S.doc Version 5.0 Page 9 comprehensive pre- admittance assessment but once again the service users cognitive/psychological abilities had not been assessed effectively at the time of admission. At the time of the inspection the Commission for Social Care Inspection issued a requirement to address these identified shortfalls and the manager of the home agreed that she would address the shortfalls within the next three weeks thus ensuring that all assessments are effective and holistic in nature. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7. 9. 10. The case tracking procedure established that the care plans examined addressed the identified health needs of the service users, but given the shortfalls in the pre-admittance assessment process, as mentioned earlier, it could not be established that health needs were being fully met at the home. No service users within the home are responsible for the administration of their own medication at the time of the inspection, although facilities for selfadministration of medicines are available should it be required. Service users spoken with stated that staff at the home maintains respect and dignity at all times. EVIDENCE: It was evident that the manager at the home continues to maintain clear, concise and very well organised care plans, which addresses service users identified needs. At the previous inspection shortfalls were identified in the care plan process appertaining to the management of complications associated with diabetes. Sutton Lodge Care Home DS0000024664.V273028.R01.S.doc Version 5.0 Page 11 The manager of the home has rectified the shortfall and it was evident that care plans formulated at the home now give sufficient details to ensure care staff can access information to provide appropriate health promoting interventions at the home. It was evidenced that care plans are re-evaluated on a monthly basis so as to address any changes in the service users needs and all care plans examined were signed and dated by the assessor. At a previous inspection the case tracking process highlighted the need to ensure that the service users full name is evident on all care plans, it was established that the manager of the home has addressed this shortfall effectively. Through discussions with the manager of the home and the homes proprietor it was established that the care planning format would be reviewed in the near future, at present they are somewhat bulky and tend to be rather repetitive. It is the intention that the new care planning formats will streamline the process to increase efficiency and aid the communication process within the home. At the time of the inspection no service users were responsible for the selfadministration of medicines. It was established that should a service user wish to be independent in the administration of medicines the manager would perform a comprehensive risk assessment, if the service users was deemed as being safe, self-medication would be facilitated if at all possible. At the time of the inspection, service users were clean, presentable and dressed in appropriate clothing for the season. Service users spoken with stated that staff spoke to them in a considerate and friendly manner as they did with all the service users within the home. The service users also stated that the staff always respected their privacy and dignity and that all the staff within the home are helpful and pleasant. It was established that staff always knocked on the bedroom doors of service users before entering thus promoting the principles of privacy and dignity. All service users were very presentable at the time of the inspection, a service user stated that the laundry facilities within the home are very good and that their clothes were always returned promptly and in a clean well ironed condition. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 Sutton Lodge Care home provides a very pleasant, comfortable and clean environment for service users. EVIDENCE: A partial tour of the premises was completed as part of this inspection. The accommodation is well decorated, comfortably furnished and maintained to a good standard. Service users spoken with stated that they are very satisfied with their bedrooms and are encouraged to bring small items of furniture and ornaments etc to personalise their individual bedrooms to promote a homely environment. The overall appearance of the communal living areas, which included the dining room and lounge areas are very pleasing, all dining tables were clean and dressed appropriately. Sutton lodge also benefits from garden areas, which are attractive, accessible to service users and well maintained by the handyman at the home. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27. 28. 30. The staffing levels and skill mix employed at Sutton Lodge is sufficient in meeting the residential needs of the 23 service users. Care staff within the home receive appropriate levels of training to ensure the service users are in safe hands at all times. EVIDENCE: Staff employed at Sutton Lodge is sufficient to meet the needs of the 23 service users accommodated at the home and an appropriate skill mix was evidenced. On the morning of the inspection one senior carer and four carers were on duty. Throughout the afternoon period, one senior carer and three carers and were on duty and one senior carer with two carers covered the night period. The home does not employ any qualified nurses, as the home does not provide nursing services. The manager performs supervision sessions for staff members on a regular basis to ensure all staff members provide quality care for the service users at Sutton Manor. In ensuring that staff are fully informed of the needs of the service users the manager operates a “key worker” system whose responsibility is, under the supervision of the manager, is to ensure that all aspects of the service users care is reviewed on a monthly basis and that the changing needs are addressed appropriately. Sutton Lodge Care Home DS0000024664.V273028.R01.S.doc Version 5.0 Page 17 It was evidenced that appropriate training opportunities are available to care staff employed at the home to ensure quality care is provided for the service users. It was established that all new staff members received an appropriate induction process and that clear and concise records are maintained by the manager of the home to evidence statutory training courses, together with additional training courses attended by care staff at the home. Training courses planned for the forthcoming year could also be evidenced. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38. Sutton Lodge ensures that the health and safety of service users are promoted and protected within the homes environment. Mobility equipment utilised within the home is maintained effectively by the homes “handyman” together with outside contractors. EVIDENCE: It was evidenced that the home is maintained to a very high standard throughout and it was determined that appropriate precautions are taken in relation to the control of Legionalla contamination, the testing of emergency lighting, the testing of fire equipment and fire alarms. It was also evidenced that the manager of the home ensures that appropriate maintenance schedules are performed by external maintenance contractors on all equipment designed of aid mobility such as wheelchairs, hoists and specialist baths. Sutton Lodge Care Home DS0000024664.V273028.R01.S.doc Version 5.0 Page 19 It was evidenced that electrical equipment utilised within the home had undergone the Portable Appliance Test (P.A.T) to ensure that it is in good working order and safe. The manager of the unit could also demonstrate that, hot water outlet checks are performed on a monthly basis and window restrictor checks are also performed on a monthly basis together with the staff call system which is also checked monthly. The records appertaining to the all the aforementioned issues were clear, concise and well maintained. The handyman, employed for 37.5 hours per week performs comprehensive external and internal maintenance procedures to ensure all areas are safe and aesthetically pleasing.
