Conditions of registration: 1 Of the accommodated in the home, up to 5 may fall within the category (PD) Physical Disabilities (from the age of 40 years) or PD(E) Physical Disabilities over 65 years. 2 The age range for service users falling within the category OP (Older People) is 60 YEARS & OVER Date of last inspection 28 October 2004 Brief Description of the Service: Sutton Lodge is a privpate care home that provides nursing care, owned and managed by BUPA. Service provision is predominantly for older people from the age of sixty years, with five places for physically disabled adults from the age of forty years. This Edwardian house has been tasefully converted and extended over the years to provide a comfortable, welcoming home with modern equipment and facilities. There are car parking facilities to the front of the premises and an enclosed, mature furnished garden to the rear. The home is conveniently located near to shops and all community amenities in the small town of Weybridgie. Bedroom accommodation is arranged on the ground, first and second floors, accessible by a passenger lift. Most bedrooms have en-suite facilities and are predominantly single rooms. The home has comfortable spacious lounges and a separate dining room. A conservatory affords an adittional sitting area overlooking the garden. A full time nurse manager and deputy are employed. This is an overview of what the inspector found during the inspection. This is the first inspection to be undertaken in the Commission for Social Care Inspection year 2005/2006. This unannounced inspection took place on 28th June and lasted for five and one half hours. Neither management, service users nor staff had prior knowledge of this inspection. The Inspector spent the first part of the visit in discussion with the person in charge checking the requirements of the last inspection, followed by a tour of the home, speaking with relatives and service users, observing service users and staff interaction. Lunchtime proceedings were observed to be dignified and orderly, the meals were attractively presented, with a choice. The inspector sampled the food, which was of a very good standard. Service users spoken to say they enjoyed their lunch and that the food is always good. The latter part of the inspection was spent reviewing service users records followed by feedback on the findings of the inspection. What the service does well: What has improved since the last inspection?
The home has commenced fitting automatic magnetic door closures, which are linked to the fire alarm system. Service users now have locks on their bedroom doors. The home is now using a new and very efficient transport company, which service users say it very good. There are new sofas in sitting room 1 and new curtains in the dining room. A revised statement of purpose containing all the requirements of Schedule 1 of the amended care homes Regulation is in place. The service users guide has also been revised. Risk assessments for locks to bedroom doors have been competed and signes as appropriate.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 2 3 4 The homes statement of purpose and service user guide are updated, providing service users and prospective service users with details of the services the home provides, enabling an informed decision prior to admission into the home. EVIDENCE: The statement of purpose has been updated to include the requirements contained in schedule 1 of the amended Care Homes Regulations. The service users guide has also been updated to reflect the contents of Regulation 5. Copies of both these documents have been forwarded to the commission and it was observed that service users were issued with copies for their personal use. Service users files revealed full assessment of health and social needs are carried out and reviewed at regular intervals. Relatives spoken to on the day of inspection said they were happy with the care their relatives received at this home, and that the staffs were all approachable and helpful. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) .7 8 9 The care plans demonstrates that Service users are involved in their care. EVIDENCE: Speaking with service users and review of their files and care plans revealed that service users health, personal and social care needs are being identified and met. Service users are risked assessed and where capable they self medicate. The inspector spoke with two service users who are self medicating, and they were able to fully discuss their medication needs and times of medication. They assured the inspector that their medication is securely locked in their cupboards in their bedrooms and that they have the keys. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 14 15 Dietary needs of service users are well catered for with a balanced and varied selection of food available that meets service users tastes and choices. Service users maintain contact with their relatives and the home operates open visiting. EVIDENCE: On the day of inspection the inspector spoke to a number of relatives who were visiting service users. They all expressed their satisfaction with the care their relatives were receiving and the general running of the home. There is a church of England service held at the home on alternative Tuesdays and the church service from the local church of England is taped on a Sunday and played back to service users on the following Sunday. Those service users who are able attend church services as they wish. Service users are involved in the planning of their care as reflected in the extensive care plans. Service users are risk assessed with solutions to minimise identified risks which are documented and signed and dated by both carer and service user or relative if necessary. Sutton Lodge Nursing Home H09 H58 S17647 Sutton Lodge Nursing Home V226270 280605 Stage 4.doc Version 1.40 Page 11 The homes menu reflect a wholesome diet for service users, and on the day of inspection the inspector observed that the food being served at lunch time was appealing, attractively presented and service users spoken to said the fodd was very good. There was a choice of hot meals, which was chosen by some service users. The inspector sampled the food and it was indeed tasty. The home employs a full time activities coordinator who works Monday to Friday. The inspector noticed that there was a full programme of activities for service users. One service user informed the inspector she particularly enjoys the flower arranging sessions. During the inspector’s tour of the home service users were engaged in playing a game of scrabble. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 18 The home has a satisfactory complaints system with some evidence that service users feel that their views are listened to. Staffs have the knowledge and understanding of Adult Protection issues, which protects service users from abuse. EVIDENCE: The home has a complaint procedure, which note that service users can make a complaint in person or by phone or letter any time they wish to do so. Service users spoken to on the day of inspection supported this statement and named members of staff they would complain to if they had a reason to complain. The person in charge informed the inspector that to the best of her knowledge, no complaints have been received in the home since the last inspection. The person in charge informed the inspector that all staffs have attended courses on the recognition and reporting of abuse. The inspector was unable to verify this in staff files as the files are locked away and the manager was away on a course. However, staffs spoken to on the day informed the inspector they have attended the course on abuse and have regular updated information on abuse. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 20 21 22 Recent investment has significantly improved the appearance of this home, creating a comfortable and safe environment for those living there and visiting. EVIDENCE: The home’s environment is clean and is maintained by adequate amount of domestic staff. Service users bedrooms reflect their personal choices and lifestyles. All bedrooms except one have en-suite facilities and there are a number of assisted baths on each floor. Toileting facilities are strategically placed around the home to enable easy access for service users. The lift. Hoists and wheelchairs are serviced regularly and records are kept of these services. The home has adequate indoors-communal facilities, which are enhanced, by the large conservatory. The spacious grounds of the home allow for relaxation on the various seats provided or safe paths for wondering around the gardens. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 28 Evidence gathered during this inspection confirmed that the home meets each of the assessed standards. Staffs appeared enthusiastic and committed to supporting the service users. Training and development are given high priority. EVIDENCE: On the day of inspection the duty rota for the twenty-four hour period was shown to include a good staff mix suitable to meet the assessed needs of the service users. From discussions with service users, relatives the person in charge and care workers it was evident that service users are in safe hands at this home. The observed interaction between service users and staff appears to be warm and relaxed. Service users are encouraged and risk assessed to be as independent as far as possible and to manage their care as much as they can. According to the care workers training and development are given high priority with over 50 of staff completing their NVQ L2. The inspector was also informed that BUPA is now offering “Personal Best Training” for all employees across the board. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 37 38 The home regularly reviews aspects of its performance through a good programme of self-review and consultations, which includes seeking the views of service users. EVIDENCE: Individual records and home records reviewed on the day of inspection were secure, in good order and securely stored. A yearly care home resident satisfaction survey is carried out each year and 2005 survey has been completed in March 2005 and highlighted areas such as the food, which needs improving. This has been acted upon and on the day of inspection service users expressed their satisfaction with the meals. Service records, fire drill records fire testing records and records of water temperature were observed and appear satisfactory. The COSHH room is kept locked.
