This inspection was carried out on 6th December 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Sycamore Court provides a good standard of nursing from staff who are qualified and experienced in the care of older people. There is a varied of menu and catering appears to be of a good quality with adequate choice and with attention being made to ensure that the meals appear attractive. Activities are provided which include trips out to Brighton and musicians and other entertainers visiting the home. Residents state that "the staff are kind" and some stated they enjoy living at the home, " there is always someone to talk to". No negative comments or complaints were received on this day. The home invests a lot of time on staff training, and although only two members of present staff have NVQ2, a further 16 are studying for this. Registered nurses are also encouraged to undertake further training and update their skills in order that they can deliver care, which is up to date and well researched, to residents.

What has improved since the last inspection?

All requirements made at the last inspection were complied with; this was particularly noticeable regarding trained nurses signing the MAR charts following drug administration. There was only one signature missing on this occasion and the manager is following this through. A further requirement has not been made but will be made in the future if deemed to be necessary. The home is generally very good and has maintained its high standard rather than making improvements.

What the care home could do better:

Care staff need to complete their NVQ 2 qualification as the home is expected to have over 50% of its staff with this, however sixteen staff are at present studying for this. A method of ensuring that residents receive interest on individual money in the bank needs to be found and money given in for safe keeping or personal allowance must not be pooled together, although separate balances are kept and money always available. An activities programme must be displayed so that residents can see what is planned for the day or the week. All previous requirements had been complied with.

The service can provide up to forty (40) nursing places and five (5) personal care places. The maximum number of services users to be accommodated is forty (40). That one named service user can be admitted aged fifty eight (58) years. 1st June 2005 Date of last inspection Brief Description of the Service: Sycamore Court is a purpose built home owned and run by Anchor Trust, it is registered to offer both nursing and residential care to people over 65 years of age. The home is situated in a residential area on the outskirts of Brighton within approximately five minutes walk of local transport services. The home can also offer parking for around 14 cars. Spanning three floors, all of which are served by a shaft lift, the home has a large lounge/ dining area on the first floor and small lounge areas on each of the other floors. The small lounges have tea and coffee making facilities for relatives and friends of residents. There is also a small well-maintained garden, which is accessible to all from the ground floor. There are five assisted bathrooms and all residents rooms have an en-suite facility consisting of shower, washbasin and w.c.The home is staffed by registered nurses twenty four hours a day. Sycamore Court Nursing And Residential Care Home DS0000014065.V268768.R01.S.doc Version 5.0 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place on the 6th December 2005 over a period of five and a half hours and was facilitated by Mrs Suzan Green, home manager. During the course of the inspection a tour of the home was undertaken, records including menus, care plans, personnel files, health and safety documentation and training records were examined and six members of staff, twelve residents and one visitor was spoken with. All residents made positive comments about their life in the home and the staff that care for them, they were enthusiastic about the food and the majority stated that they enjoyed living at the home and were very well cared for. Thanks are extended to Mrs Green, the staff and residents and also to the regional manager, Mrs Angela Scott, who attended at the end of the inspection, for their courtesy, time and hospitality during the day. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5 The home provides sufficient documentation to enable residents can make an informed choice regarding their proposed admission to the home. The assessment conducted by the manager or her deputy ensures that the home can meet the needs of the prospective resident. EVIDENCE: A statement of purpose and service users guide is produced which meets this standard. All service users now have a copy of the service users guide and it is recommended that the manager takes a copy of these documents with her when assessing those residents who are unable to visit the home, to ensure that they have some information relating to the home prior to deciding whether they wish to be admitted to the home. The assessment ensures that the home can meet the needs of the resident and informs both the manager and the prospective resident of whether the home is suitable for them, this is normally undertaken by the manager or her deputy. Residents and their representatives can visit the home prior to their admission, and are admitted for an initial trial period. The manager must ensure, that under the present circumstances whereby the home is running a pilot scheme for interim care through the PCT, that no resident is admitted unless the home is sure that they can meet this residents needs, that there are sufficient staff to do this and that the manager allows sufficient time between admissions for the staff to be aware of the needs of the any resident previously admitted under this scheme. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 Care plans met this standard, showing that residents were involved in their formation and review. Frequent review of the care plans ensured that resident’s current needs were being noted. The date of opening of eye-drops must be recorded to ensure their efficacy for resident’s optical health. EVIDENCE: A percentage of the care plans were examined and these identified that they had been updated on a monthly basis, in most cases had been signed by the resident or their representative to show that they had been consulted about the care to be given, and contained consent for the use of bedrails and other risk assessments relating to the resident. All care plans identified the care to be given in a clear manner and included nutritional, social, physical and psychological assessments. There was evidence that the wound care nurse visits the home and that other health care professionals including dentists are accessed as required. Residents have a choice of GP providing they are within the area, or the home will access Sycamore Court Nursing And Residential Care Home DS0000014065.V268768.R01.S.doc Version 5.0 Page 11 one for them. The home has a physiotherapist who visits the home on a regular basis and treats all residents. The majority of residents appeared well cared for and those residents requiring nursing in bed or in their rooms seemed comfortable with identified care being given. However it was noted that one resident had dirty fingernails and this was discussed with the RGN on duty and the manager. Residents stated that they felt that the staff respected both their wishes and their privacy and that they received medical and nursing attention in their respective rooms. All medications with the exception of one had been signed on administration and there was evidence of stock rotation. It is good practice to ensure that the date of opening is written on eye-drops and that these are discarded after one month. Likewise medication such as antibiotics, which are made into a suspension by the issuing chemist, should be discarded after a month of being received. All controlled drugs were accounted for and had signatures in place. The clinic rooms appeared to need the services of a domestic assistant and more care must be taken in these areas. The manager states that she will address this with the nursing staff. Some of the registered nurses and care assistants have attended training regarding the care of the dying resident and the use of syringe drivers and other analgesia. All residents within the home that were being nursed in bed, were seen to be comfortable with correct nursing interventions taking place and a letter was seen that thanked and complemented the staff on their care of a recently deceased resident. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 The home provides a good, varied standard of catering and activities, thus ensuring a pleasant quality of life for residents. Staff interaction with residents was seen to be good, ensuring that residents know that staff are interested in them as individuals. EVIDENCE: The home provides a variety of activities including board games, sing-along, Music for health, and outings. The home also provides the opportunity for residents to take part in an art group on alternate months. At present the activities are provided by a care assistant with time built in for this on the duty rota. A programme of the activities, which will take place, must be displayed where residents can see this. There is a Christmas party planned and all residents have received information relating to this. Residents can make choices around their activities of daily living and on whether they wish to take part in the leisure activities provided by the home. There is an open visiting policy and a copy of this is displayed. Only one visitor was in the home on this inspection, and he stated that he was made welcome when he came. Residents said that ‘ they always make my daughter a cup of tea when she comes’, and ‘ my visitors stay a long time but the staff are alright with this’. Sycamore Court Nursing And Residential Care Home DS0000014065.V268768.R01.S.doc Version 5.0 Page 13 The manager is aware of how to access solicitors, financial advisors and advocates for residents. The standard of catering is good, with a varied menu being provided and a cooked breakfast provided twice a week to those who wish to have it. There are choices at each meal and the staff advise the residents of what is available, this is also displayed in the dining room each day. Records are kept of those choices made by residents which are different to the main menu. The meal of the day was chicken curry or cheese omelette with vegetables and there is always a sweet trolley with five or six different desserts to choose from. The standard of supper menu was also good and varied. Snacks can be provided to residents at any time. Residents spoken with said, “ The food is lovely”, “ he makes his own cakes “, “ always a good choice of food”, “ if you don’t like what they offer they do something different”. Residents spoke positively about the chef saying “ he is always laughing”, “ he’s a very good cook”,” He will always discuss the menu and change it to what you want”. Some very good interactions were noted between staff and residents at lunchtime, and especially so with one care assistant, this was relayed to the manager and he was to be commended on his attitude towards the resident he was assisting with her food. All carers were seen to take their time and be talking to residents whilst assisting them. The kitchen was clean and all records relating to food and fridge temperatures were in place. All catering staff have their food hygiene course and the cook is a trainer for this. The impermeable flooring is starting to lift in one place and the cook was asked to address this with the manager. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18 The manner in which complaints are addressed and the training of the staff in adult protection ensure that residents can feel safe and listened to within the home. EVIDENCE: The home has had one complaint since the last inspection, which is still being addressed. Complaints are seen to be addressed in a fair and robust manner with a detailed complaints procedure being available in the entrance hall and in the service users guide. All residents who wish to do so are registered for postal votes, therefore ensuring they can take part in the electoral process. All staff have undertaken some training in the protection of the vulnerable adult. At present this is mainly done in house but it is recommended that senior staff attend updates with social services training department. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26 Sycamore Court provides a well maintained safe home for residents. Although quite large, division into separate units ensures that it remains a homely environment. EVIDENCE: The home is well maintained and pleasantly decorated. Attention is paid to minor maintenance issues. Recently the home has had a new extractor unit and fly screens installed in the kitchen and staff stated that this had improved working conditions. There is a large dining room and lounge area available on the ground floor and small lounges on each floor with tea and coffee making facilities. The ground floor lounge accesses the garden area. The home has five assisted bathrooms and resident’s rooms have ensuite facilities with assisted showers. There is a range of equipment, which includes variable height beds, full body hoists, and the home has been assessed by a qualified occupational therapist Sycamore Court Nursing And Residential Care Home DS0000014065.V268768.R01.S.doc Version 5.0 Page 16 to ensure that it has sufficient equipment, grab rails and other aids for those who are disabled. Residents rooms have radiator guards and window restrictors in place, and water temperatures are tested regularly with the majority falling within recommended parameters, some however need adjusting to ensure that all water is delivered around 43C. All residents’ rooms are fully furnished but residents can bring in furniture following discussion with the manager, and are encouraged to bring in their own possessions to personalise their rooms. Not all rooms were inspected on this occasion and it was not noted whether the four rooms without locked facilities seen at the last inspection had now got these, the manager is asked to ensure that these are in place. Infection control measures are in place; this includes staff wearing aprons and gloves for care and protective clothing when entering the kitchen. All bathrooms and toilets were in a clean condition, as were the sluices, and all soiled washing is put in red alginate bags. All communal bathrooms and toilets have soap and disposable towel dispensers. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 The recruitment process, staff training and duty rotas ensure that the care needs and the safety of residents are maintained. EVIDENCE: The duty rota identified that there were sufficient staff on duty during a twenty-four hour period to meet the needs of the residents. Staff spoken with stated that they felt that there were enough staff on duty to enable them to work without being under too much pressure or being rushed. However with the new interim care scheme being piloted, the manager must ensure that staff numbers are increased if more residents with high nursing needs are admitted. The manager stated that there has been a recent staff turnover with some registered nurses having left, but she is interviewing other registered nurses at present to make up this deficit. Staff hours are being covered by bank staff as required. The home does not use agency staff. Only two members of staff have completed their NVQ 2, but a further sixteen are undertaking this course at present. Staff training undertaken includes: Protection of the Vulnerable Adult, Wound care Training, First Aid (12 members of staff are attending this course), catheter care and some study in Palliative care. All staff have undertaken mandatory training including moving and handling and fire training. Personnel files relating to recently employed members of staff were examined and these were seen to contain all documentation required by the regulations. The manager is asked to continue to check the validity and expiry dates on work permits on an ongoing basis. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,34,35,36,37 Residents positive comments show that they benefit from the good ethos and atmosphere within the home. The present banking arrangements for residents monies can put some EVIDENCE: The manager Mrs Suzan Green has been in post for thirteen months. She is a registered nurse and has many years experience in the care of older people, previously having been the manager at Sycamore Court. She has in the process of completing her Registered Managers Award and has been registered as manager by the CSCI. Staff and residents spoken with stated that they were happy at the home. Staff stated that it was “a good place to work” and that they “ felt able to make concerns known”, whilst residents stated that “ The staff are happy and smile a lot”, “ Staff are quite nice”, “ Most staff are lovely”. The atmosphere within the Sycamore Court Nursing And Residential Care Home DS0000014065.V268768.R01.S.doc Version 5.0 Page 20 home is lively without appearing rushed or too busy and residents and staff interaction was good. The home has an on-going quality assurance programme, although the SAMS method used by Anchor is not in progress at present. Frequent staff meetings are held and the manager stated that some residents meetings take place. Questionnaires are forwarded to residents and relatives and the manager said that she is in the process of collating these. Stakeholders including GPs and other health care professionals must be included in these questionnaires. The accounts and business plan for Anchor Trust relating to Sycamore Court has been seen in the past eighteen months and the manager has stated that she participates in the annual business plan for the home. There is still an ongoing concern around the placing of resident’s monies in accounts that do not pay individual interest to residents. The CSCI is addressing this on a national level with Anchor and the manager stated that she believes that Anchor is looking to do this. Likewise the pooling of money brought in for residents and their personal allowance is at variance with this standard, the manager states that Anchor is addressing this. Supervision of staff is taking place at intervals as stated in this standard, with the process of supervision being delegated to all senior members of staff. All records are kept securely in a locked office. Residents care plans are kept in their rooms. All certificates relating to the servicing of utilities and equipment were seen to be in date and all staff have received mandatory training. No issues that may affect the health and safety of residents were found at this inspection.
