Conditions of registration: 1. No service user to be admitted under the age of 60 years of age. Date of last inspection 21st June 2005 Brief Description of the Service: Tanfield Care Centre is a Reading Borough Council care home. It is purpose built and situated in Whitley, a suburb of Reading. The home is registered to provide personal care and accommodation for thirty one service users, (twelve in the Intermediate Care unit, eight in the E.M.I. unit, and eleven in the Assessment and Intervention unit). The home currently accommodates three long stay residential service users, twelve in the Intermediate Care unit, 8 in the EMI unit and seven in the Assessment and Intervention unit. The home is equipped with a passenger lift and aids and equipment to assist the more dependent service users. Thirteen of the bedrooms have en-suite facilities. The aim of the home is to provide a homely and comfortable atmosphere; a lifestyle that satisfies their social, cultural, religious and recreational interests and needs; promote and maintain service users’ health; and to ensure the independence and individuality of the service users is respected. An occupational therapist, physiotherapist and a rehabilitation manager have been employed by the Health Authority to work in the Intermediate Care unit alongside local authority care workers. This is an overview of what the inspector found during the inspection. This was a routine unannounced inspection, which was carried out between 11.15am and 4.05pm on Tuesday 21st June 2005. The Inspector spoke with three service users, the manager, deputy manager and a member of staff. The Inspector also looked at records and observed the interaction between staff and service users. Eight standards were assessed of which five were met, one nearly met and two exceeded. No requirements or recommendations were made. The manager wants ‘tired’ areas of the home to be redecorated and new carpets where needed. The manager will ensure all staff complete the medication records correctly. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Numbers 8, 9 and 10. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. EVIDENCE: Service users looked personally well cared for. They said staff respond well to any health issues. An agency member of staff considers the personal and health care of the service users is excellent. Service users can keep their own GP. Records showed service users have access to specialist medical, nursing, dental, chiropody and therapeutic services. Service users are encouraged to keep mobile. The service users in the dementia unit have regular ‘exercise to music’ sessions and a physiotherapist provides individual therapy for the service users in the rehabilitation unit. Service users are able to self medicate after a risk assessment is undertaken. A pharmacist visits the home at regular intervals to assess the standard of recording, storage, handling, administration and disposal of medicines. She
Tanfield Care Centre DS0000039942.V264109.R01.S.doc Version 5.0 Page 10 visited on the day of the inspection and noted there were a several gaps in the recording of medication and the downstairs trolley required a means of fixing it to the wall when not in use. The manager said she would address these issues. Service users said staff were considerate, approachable, respectful and kind. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Numbers 12, 13, 14 and 15. Service users find the lifestyle experienced in the home matches their expectations and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/friends and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet. EVIDENCE: When service users are admitted to the home they are asked what their interests are and what is their routine of daily living, i.e. what time they like to get up and go to bed, whether they prefer breakfast in their own rooms, etc. The home employs two activity coordinators. One coordinator works ten hours a week on the dementia unit and the other coordinator works twenty-eight hours a week with the other service users. A wide range of activities is organised such as bingo, sing songs, raffles and seasonal socials. As well as the activities provided on their unit, service users who suffer from dementia are invited to all suitable activities and socials taking place in the home. Local clergymen visit the home to give communion. Service users can have visitors at any reasonable time. Relatives and friends are invited to social events. Service users are taken out to community

Several service users belong to day centres and are taken to local shops and to lunch at local pubs. Community groups are encouraged to visit the home. Service users are entitled to bring personal possessions. Service users have access to their records through the Reading B.C. ‘access to records’ procedure. Service users can smoke and consume alcohol in their rooms unless it has been risked assessed as being too great a risk to themselves or other service users. Service users can handle their own financial affairs. Service users said they enjoy the meals very much. There is choice, plenty to eat and if they are vegetarian or do not like the main meal an alternative meal is provided. The inspector was informed fresh vegetables are used as much as possible and processed food is used as little as possible. Two part time cooks and a part time assistant cook prepare the meals.
