Date of last inspection Brief description of the care home Thames Side is a purpose built residential care home to accommodate older people. It was opened January 2003 and the home is sited in its own grounds with good size, well-maintained gardens that are accessible to the residents. Car parking facilities are available at the front of the building. The home is well presented, providing accommodation for up to 60 residents over the age of 65 years. All bedrooms are for single occupancy and have en-suite facilities. The home has five separate units, each Care Homes for Older People
Page 2 of 8 with its own sitting room, dining room and kitchen. Stairs or passenger lift accesses the upstairs accommodation. At the time of this site visit the fees were from #457.52 to 741.60 per week. This does not include hairdressing, some chiropody, papers, toiletries and transport for trips. The cost to hire a vehicle for outings is #25.00 the cost is divided between the numbers of residents using the transport. Care Homes for Older People Page 3 of 8 What we found:
This inspection was a specialist pharmacist inspection to look at the handling of medicines in the home following concerns that were brought to the attention of The Commission. People who use the service have their medicines given to them by designated trained care staff. Written procedures are available to staff to provide guidance and to enable medicines to be handled consistently by all staff. No one was looking after their own medicines when we visited. This can be arranged if people wish to do this. The medication administration records, supplies and care plans were looked at on three of the five units. Clear records were kept to show when people had their medicines, including creams, ointments and nutritional supplements. These showed that people get their medicines correctly. However, one person had not been given one medicine at the correct dose interval on one occasion and this could have adversely affected their welfare. When a variable dose of medicine was prescribed the actual dose given was being recorded by some staff but not others. Clear records are kept when Warfarin is prescribed for people. Personalised care plans, describing how people like to take their medicines, were available for all of the people whose records we looked at. These should be expanded to describe how people are to be given any medicines prescribed to be given only when needed. The use of body maps to show where creams and ointments are to be applied was particularly helpful to the care staff. Peoples allergies to medicines were recorded on their medication administration record (MAR) charts, their medication care plans and their personal details. Sometimes these were not recorded consistently. For example. One persons care plan and personal details said that they were allergic to penicillin but this was not recorded on their MAR chart. Another person had an allergy to penicillin on their MAR chart but this was not included on their care plan or personal details. Allergies must be recorded accurately and consistently so as to prevent someone being given a medicine that they are allergic to. All medicines were stored securely so as to protect the people who use the service. A Controlled Drugs cupboard was available to provide extra security for these medicines. Additional records were kept of the usage of Controlled Drugs so as to readily detect any loss. The entries in these records were being initialed not signed.
