Conditions of registration: One named service user under age 65 (PD) Date of last inspection 31st January 2005 Brief Description of the Service: The Gables is an old largely Georgian house in the centre of the small Cambridgeshire town of Chatteris, which has been updated and extended to provide accommodation for up to 40 older people. The home curently has one service user under the age of 65 which has been agreed with the CSCI. The accommodation is split over two floors. On the ground floor, there areBedrooms and communal areas, which provide secure accommodation for 17 older people with dementia, and on the first floor facilities for up to 23 service users, but without dementia. There are bathroom and toilet facilities on both floors, and a large enclosed rear garden Gables Residential Home (The) Version 1.10 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was the first inspection of the current year, and an unannounced visit, and was followed up by the inspector attending both staff and service user meetings to try and get as much information from them on what living or working in the home is really like. In addition, service user and relatives Comment cards were left in the home, and a large number returned to the CSCI before this report was written. These comment cards were all favourable, clearly illustrating the way in which the home is run. The inspection lasted about 5 hours in total, and the manager of the home was available at all times to provide the information required. At the time of the inspection, the home had one vacancy upstairs and one downstairs, but these were being filled soon after the inspection. Since the last inspection new owners have taken over the home, and a programme of refurbishment is underway. Care plans have all been reviewed, and are now up to date, and a regular system of review in place. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3, 4, and 5 The Gables has a detailed admission procedure, including visits from service users, ensuring that the needs of service users moving into the home can be met. EVIDENCE: During the inspection, the registered manager was available to provide information on the homes Statement of Purpose and Service User guide. Following the recent changes in ownership, revised documents have been sent to the CSCI, and these meet the requirements under the regulations. Included in these documents are details of the services available, the management and staffing arrangements and information on making a complaint. Prior to any new admission, the registered manager visits prospective service users, and completes a detailed assessment. This enables the home to ensure that the home is suitable for the prospective service user, and that they are able to meet the service users needs. At the present time, the assessment is not signed, and this was left as a recommendation, to ensure a clear record of who carried out the assessment is available.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 9, The healthcare needs of service users in the home are identified, and individual care plans detail these needs and how they should be met, to ensure all carers have the information they need when caring for the service user. EVIDENCE: During the inspection, the care plans and related documents of a number of service users were looked at, and all showed that the information gathered by the home is satisfactory. The care plans are reviewed monthly, and a record of this was also seen which ensures that the care plans are up to date at all times, giving care staff the information they require. The home has a good relationship with the local GP practice, with regular visits from the GP to the home. It was agreed with the manager that the inspector for the home would attend both a service user meeting and a staff meeting which offered service users and staff the chance to discuss any issues or areas of concern, and comments and information gained at these meetings has been included in this report. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,and 15 The Gables offers a particularly wide variety of social activity, and a varied and balanced diet, ensuring service users have a choice in all areas of their lives. EVIDENCE: During the inspection and service user meeting a number of service users commented on the activities available within the home, and all agreed that a good variety of activities are available. During the inspection, the homes activity worker was planning for two very different activities later that week, the funeral of the Pope and the marriage of the Prince of Wales. Regular church services are arranged in the home, and some of the service users attend the local church opposite the home. The food provided at the home appeared to be of a high standard, with choices available to service users, and regular discussions take place with the service users and cook to discuss changes. The service user meeting was attended by about 12 service users, the responsible individual and the relatives of one service user, and everyone living in the home receives a copy of the minutes of the meeting. It allowed service users to comment on various aspects of the home, and to be kept up to date with plans for the future. Gables Residential Home (The) Version 1.10 Page 12 Complaints and Protection
The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The homes complaints policy is available to all service users and their families, and service users are confident that any concerns they may have will be dealt with properly. Adult protection policies are in place to ensure the protection of service users from abuse. EVIDENCE: Since the last inspection, all senior staff have had further internal discussions around adult protection issues, and are now familiar with the policy, and aware of action they should take if any form of abuse is suspected. This will be passed on to other staff over the coming weeks. All staff have also received training from the local Social Services trainer in all aspects of adult protection. Although there have been no complaints made to either the home or to the CSCI since the last inspection, the homes policy on complaints was discussed, which meets the requirements, and ensures that any complaint is properly dealt with. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 24, 25 and 26 The programme of refurbishment continues, offering service users a comfortable and safe environment to live in, with their own possessions around them if they choose. EVIDENCE: The inspector was able to look round the building during the inspection. It was noted that the programme of redecoration has continued since the change in ownership. The upstairs corridor was being decorated at the time, and the manager advised that new lighting was also being installed. New lighting has also been installed in the upstairs lounge, and further refurbishment and redecoration is planned. As rooms become empty, they are being completely refurbished, with new freestanding furniture, carpets and washbasins. Examples of rooms that have already been refurbished were seen during the inspection. The home was clean, and free from unpleasant smells, and the rooms of service users rooms all had items of a personal nature, including furniture and other effects.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29 and 30 The procedures for the recruitment of staff are robust and offer protection to people living in the home. The deployment and number of staff available is appropriate to meet the needs of the service users. EVIDENCE: The manager confirmed that the home currently has a good level of staffing, with vacancies for one member of staff on days and 2 on nights. Other new staff have been recruited, and service users were particularly impressed with the standard of the new staff. Staff are well trained, and records of the training being provided were seen during the inspection. Gables Residential Home (The) Version 1.10 Page 16 Management and Administration

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35, 36 and 38 The home is well managed and staff receive leadership, guidance and direction to ensure service users receive consistent quality care. The health, safety and welfare of the people using the service is promoted and safeguarded at all times. EVIDENCE: The home has changed ownership since the last inspection, and a number of changes are likely as a result. As has already been indicated, a programme of refurbishment and redecoration is continuing, and further more extensive changes are planned. Some of these were discussed with the inspector, and further discussions will occur.
Gables Residential Home (The) Version 1.10 Page 17 The home has policies and procedures in place to ensure that service users monies are looked after at all times, and health and safety training is provided to all staff on a regular basis. The registered person must ensure that the monthly report required is sent to the CSCI, and that all the records required are up to date.
