This inspection was carried out on 7th October 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The inspector spoke with four residents and three visitors. Residents said that they were happy at the home, and that staff were nice to them. A new visitor felt that the staff were, `friendly` and had made her feel welcome. The staff administer medication in the correct way. Residents can make choices about every day issues, and can complain if they wish. The home is well maintained and clean. The home employs care staff, kitchen staff and cleaning staff, which means that staff have time to organise activities. Staff have good training in relevant topics, and ten staff have achieved a NVQ qualification in care. The home has a questionnaire in place which seeks the views of residents.

What has improved since the last inspection?

Resident`s care plans are now more detailed, to include information about their mental health needs, for example, if someone is feeling `low`. The staff knew what to do if there was an allegation or suspicion of abuse.

What the care home could do better:

The home should ensure that references for new staff have a date on them, so that it can be evidenced that these were received before staff began work.

The home is owned by Mr and Mrs Jackson, who have an active role in the home, and managed by Karen Edney. The home is located in the quiet residential area of Netley, which is on the outskirts of Southampton. There are local shops and amenities nearby. The accommodation is on two floors, and there are 14 single bedrooms and 5 shared. Communal space is comprised of a lounge, dining room and large conservatory on the ground floor. There is an enclosed garden and patio area at the rear of the property and a car park at the front. This was the second unannounced inspection of the year, and took place over three and three quarter hours. The inspector looked around the home, talked with residents and visitors, the providers, deputy manager and staff. Records such as care plans were also seen. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The home ensures that its policies and procedures protect residents with regard to medication. EVIDENCE: Staff described the procedure used to administer drugs safely. The provider, manager and deputy manager have undertaken a day course on medication. One of the staff said they were participating in in-house training and that they were able to go at their own pace. Staff monitor if residents refuse, or chew, their medication. A pharmacist had visited the home and given a training session about crushing medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 The manager ensures that residents can make choices. EVIDENCE: The inspector saw evidence that residents can maintain their bedrooms as they wish. One resident does not like staff to tidy her room, and so possessions were left as the resident liked them. Residents are welcome to bring possessions into the home. One resident brought their cat into the home, and a visitor commented how nice this was. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home ensures that residents and their visitors feel able to complain. EVIDENCE: The home has a complaints procedure in place. The inspector spoke with two visitors who said that they felt able to complain if necessary. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 The providers and manager ensure that residents live in a well maintained, clean and hygienic environment. EVIDENCE: One bedroom has been re-decorated, and six bedrooms have new window blinds. Communal areas have been re-carpeted and the hallway has been redecorated. The main bathroom is undergoing total re-furbishment, which will include a new specialist bath. The garden is maintained and residents encouraged to sit outside when the weather allows. There is a sluice room which is used for cleaning commodes. Staff told the inspector that they reduced the risk of cross infection in the home by washing their hands before and after tasks, wearing gloves and aprons, using the sluice programme on the washing machine to rinse soiled laundry, etc. The home was clean and free from odour and two cleaners are employed Monday to Friday. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30 The home ensures that residents’ needs are met by trained and skilled staff, in numbers appropriate to residents’ needs. The recruitment procedure ensures that residents are protected. EVIDENCE: The rotas show that there are generally three or four care staff, plus the manager, on duty. Additionally, the home employs a cook and kitchen assistant every day, and two cleaners, Monday to Friday. The providers are frequently at the home in addition to the staff on the rota. Two staff are employed at night, both of whom remain awake. The deputy manager said that as there is now more staff in the evenings, there is time to do more activities with residents. Ten staff have achieved NVQ2 and staff said that they have been supported to complete the qualification, and had been given the time to do so. Other staff who had been approached by the management about doing the course, were considering this, and felt that they would be supported in their decision. The inspector looked at three new staff files. Two references were in place for each worker, although these did not have a date on them. The inspector advised the provider to ensure a record was kept regarding the date the references were returned. The provider undertakes a Protection of Vulnerable Adults check before employing new staff, and a Criminal Bureau Records check The Gables DS0000011862.V256159.R01.S.doc Version 5.0 Page 15 follows this. The provider had not kept a record of the date of two of these checks, and needed to make a telephone call to ascertain the information. Staff told the inspector that they had received training in various topics, including Moving and Handling, Fire Training, Dementia workshop, Adult Protection, Medication, First Aid and Infection Control. The inspector saw records for in-house inductions for new staff. The level of induction was appropriate as the new staff were experienced in the care sector. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38 The manager ensures that the home is run in the interests of the residents, and that their financial interests are safeguarded. The home is maintained in a way which keeps residents safe. EVIDENCE: The manager was unavailable on the day of the inspection, but the inspector was told that she has completed her Registered Manager’s Award qualification. The manager has also updated her training including Challenging Behaviour and Staff Health. The home has undertaken a questionnaire to seek the views of residents. A copy of the results has been sent to the inspector, and any issues highlighted have been discussed further within the home. The inspector checked three accounts and found that the records matched the amount of money. Receipts were also kept. The Environmental Health Officer had conducted a routine visit and made some recommendations which the home have implemented. These included moving the cutlery tray and taking the temperature of cooked meats. Food was appropriately stored. Up to date maintenance certificates were available for equipment such as the lift.
