Date of last inspection Brief description of the care home The Knoll Nursing Home is a large detached building, situated in a residential area of Walmer. It is near to local shops and amenities, Walmer Castle, and the sea, and is within easy reach of the town of Deal. The home is owned by Raj and Knoll Limited, and managed by Mrs. A. Patel, one of the providers. Accommodation is mostly provided in ground and first floor bedrooms, although there is also a shared room on the second floor. All floors can be reached via a small passenger lift. Eight rooms have Care Homes for Older People
Page 2 of 8 en-suite facilities. Outdoor space is currently limited to a small patio area which is accessible for service users to sit out in good weather. However, there is a large public park almost opposite The Knoll which is often used to take Service Users out for walks. The providers have purchased the adjoining premises, demolished the building, and are in the process of building to provide an extension to the home. Some structural alterations and refurbishment of the existing building will also take place. Care Homes for Older People Page 3 of 8 What we found:
A random inspection was carried out on 28th October 2008 as a result of a recent safeguarding alert, which raised issues that the home may be in breach of the regulations. This inspection focused on contracts/terms of residence for the people who live in the home, the complaints and safeguarding procedures. The outcome of the visit is as follows: The home carries out a care needs assessment for prospective residents before admission to the home, however, it is not being confirmed in writing that the home can meet his or her needs. The Registered Manager says this will now be implemented in time for the next admission to the home. Each person who comes to live in the home is provided with a contract/terms of residence. Two examples were looked at and were signed and dated by the Registered provider and person living in the home. Currently there are two formats of contract in place. One contract for self funding people and one for people who are funded by the local authority. All of the people who live in the home have been given a contract. There are shortfalls in the information provided in the contract and the following information needs to be included: The agreed room number at the time of admission, overall care services including food, covered by the fee, additional services (including food and equipment) to be paid for over and above those included in the fees, rights and responsibilities of the service user and the registered provider and who is responsible if there is a breach in the contract, and what constitutes a breach in the contract. The Registered Provider/Manager stated that she would review and update the contracts to have one standard contract for all service users. This will be reissued to each individual to ensure that the above information is included. The Registered Provider/Manager acknowledges the shortfalls in the contract, and is taking action to address these issues therefore a requirement will not be made in this report. A recommendation will be made to ensure good practice. The Commission received a complaint, which was processed as a safeguarding alert. Under safeguarding protocols the home has investigated the issues and responded to the actions required in an appropriate manner. There is a complaints log in place which records all concerns no matter how minor. Further detail is required in the recording of concerns/complaints to indicate what if any action is required. In view of this the Registered Manager states that she will review the complaints recording system to include a separate section to clarify what action is taken. One resident says that she did raise a concern with the Registered Manager, who responded appropriately, and everything is fine now. None of the other residents spoken to had any complaints about the home. The Registered Manager stated that she will carry out an audit of the complaints and revise the recording procedure. The home has polices and procedures in place to ensure that residents are protected from abuse. They have a qualified in house Protection of Vulnerable Adult trainer and Care Homes for Older People Page 4 of 8 all staff have received training. Some of the training courses were completed some considerable time ago. Staff can demonstrate their awareness of adult protection procedures but would benefit from an update in training to keep up with current practice.
