Conditions of registration: Date of last inspection 22nd July 2005 Brief Description of the Service: The Old Dairy is a three-storey house with accommodation for six adults with Aspergers Syndrome. The home offers a very specialised, individual service for people with Aspergers syndrome, and staff receive training to support service users appropriately. The home is conveniently situated in Nailsworth, which enables service users to access local community facilities. Service users also have access to transport that is provided by the home and this enables them to access facilities in several other local towns. The home is staffed 24 hours a day, seven days a week. Family and friends are welcome to visit the home at any time and service users can meet them in private if they wish to. This is an overview of what the inspector found during the inspection. This was an unannounced inspection and it was completed in one afternoon. The inspection consisted of talking to staff on duty and service users who were in the main house. The inspector also visited the adjacent flat and spoke to the service user and member of care staff who were on duty. A number of records were also seen and they included care plans, annual reviews and risk assessments. There was a relaxed and informal atmosphere in the home and flat, service users spoken to said that they enjoyed living in the home and that staff were always available to give support and guidance if needed. In some cases independence was encouraged and this was being developed with appropriate assistance from staff. What the service does well: What has improved since the last inspection? Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 9 Service users needs are continually reviewed and wherever possible met. EVIDENCE: Service users needs are assessed and care plans are written. The plans indicated how the current and, where necessary, the anticipated service users needs are to be provided. They included planned interventions and some structure to their daily lives. There is also an annual review and regular Individual Personal Programme meetings. The plans are wherever possible drawn up with the involvement of the service users and where appropriate their families. The home operates the key worker system. There is a regular review of the problems/needs and if appropriate this would involve other health care professionals. Independence is encouraged and supported and service users have a number of links with the local community.

Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15 Service users are provided with a range of appropriate activities and are supported to maintain other social relationships. EVIDENCE: The service users have a busy and varied programme of activities. Staff continue to make great efforts to identify and wherever possible meet the wishes and needs of the individual. The centre at Spring Mill has a day service provided by the Trust and this provides a range of appropriate interests and activities for the service users. The home supports service users in respect of contact with friends and families, and the inspector was given examples of the kind of support provided. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 20 The healthcare needs of the service users are met in a manner which is sympathetic to individual preferences and needs. EVIDENCE: The home maintains a monthly record of all healthcare received. This is updated from the daily records. The inspector was advised that there is a record of the receipt, administration and disposal of medicines. At the time of the inspection there was no selfmedication. The system of recording the administration of prescribed medicines now included the signature of staff. JUDGEMENT – we looked at outcomes for the following standard(s): 22, 23 Service users live in a safe environment and are encouraged to comment on issues, which affect their day-to-day lives. EVIDENCE: The inspector felt that service users were listened to by staff and that any reasonable request would be considered and wherever possible met. The atmosphere in the home was relaxed and this, with the willingness of staff to listen, provided an environment where service users felt able and comfortable to offer their views on matters that affected their day-to-day lives. There is a written complaints procedure, however, the inspector felt that staff would deal with any concerns/complaints quickly and effectively without recourse to any formal procedure. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 35 Staff were seen as competent and providing a personal and dignified service. EVIDENCE: At the time of the inspection there was one member of staff in the main house and one carer in the flat. Both staff were seen and spoken to individually and they had a good knowledge of the needs of the service users. The inspector was advised that the Trust are looking at training options to include a part-time distance learning module managed by Birmingham University.
