This inspection was carried out on 17th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Following observation of the care and interaction between residents and staff and also from discussion with residents care is offered in a skilled manner and residents are treated with respect and dignity.

What has improved since the last inspection?

There has been an improvement in the way residents records are stored and the manager is also piloting a new care plan, which seems to have clarity and ease of use. Much training has been put in place to ensure that residents receive care from competent staff. Attention is being paid to activities for the residents and there are plans for two raised beds in the garden for residents use. The home now has two cats about which the residents spoken to expressed pleasure.

What the care home could do better:

Whilst there is a well-developed activities programme one resident felt that some of the regular music is old fashioned. " Pack up your troubles" she said is a bit dated. As the home has within category 6 places for people aged 50 and over and consideration could be given to providing some more up to date music sessions.

Bedrooms 3-7 and bedrooms 15-20 will not be able to accomodate service users after 1st august 2006 should they not meet the care home regulations 2003 and the national minimum standards for older people in full by this date. 2. Up to 6 users in the category Physical disability (PD) aged over 50 years may be accomodated. 3. Up to 6 users in the category Physical disability (PD)(E) aged 65 years and over. may be accomodated. 4. A maximum of 34 service users may be accomodated at any one time. Date of last inspection 21 02 05 Brief Description of the Service: The Old Malthouse is a care home with nursing and is registered to provide accommodation for up to thirty four service users in the category older persons, six of who may also be aged 50 years and over in the category physical disability. It is a large detached premises, which is located off the main road in to Selsey village. The local shops and amenities, and the sea front are within easy walking distance of the home. Accommodation is provided in thirty single and one double bedroom located over two floors. There is a passenger lift between the floors and levelled access to all rooms. All bedrooms, except for one, have en suite facilities and in addition, assisted bathing facilities are also provided. There is a choice of sitting areas including two lounges and a dining area. The home is about to undergo extensive refurbishment. Timothy Whites Ltd is the registered provider and privately owns the service. The responsible individual, acting on behalf of the company is Mr David Appleby; Mrs Glynis Whitmore is the registered manager and is responsible for the day to day management of this care home. This was an unannounced inspection carried out on May 11th 2004. The manager, director, administrator and several staff were spoken to. The majority of the service users were also spoken to. All expressed satisfaction with the care offered here. The staff stated that it was a supportive environment to work in and the general attitude to the change of ownership and management was positive. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The resident’s health, personal and social needs are set out in a plan. These needs are fully met. The out come for residents is that their individual needs are met. The underlying philosophy of the home is that of respect and dignity and residents are treated with this in mind. EVIDENCE: New care plans being piloted were seen and address all the residents’ needs. Resident’s needs are met and appropriate entries made in the care plans. That residents are addressed and treated with respect and dignity was observed during the inspection. Residents spoken to confirmed this. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 Resident’s lifestyle in the home meets their expectations and mainly satisfies their recreational need. Residents maintain contact with relatives and friends and community activities and they have choice and control in their lives. An appealing and nutritious menu is served. EVIDENCE: The kitchen was not inspected as it about to undergo refurbishment but the meals observed being served were well presented, nutritious and were enjoyed by residents. Those who chose to do so had their meals served in their bedrooms. All residents spoken to expresses satisfaction with the meals, which they say, have improved since the new management took over. This is an improved outcome for residents. There is a well-developed activities programme with a choice of activities, musical events ands shows. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16-18 There is a complaints procedure on display. Residents are mostly aware of the complaints procedure. Residents are given the opportunity to vote and are not abused in any way. EVIDENCE: There is a complaints procedure in place and residents stated that their complaints would be listened to and acted on although many stated that there would be no need to complain. Residents stated that they are always treated with respect and that they are not harmed in any way or hurried. This was observed during the inspection. The outcome for residents is that they are protected from abuse. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,24,25,26 Residents live in a safe and comfortable environment. There are appropriate toilet and bathing facilities. Residents can have their own belongings around them if desired. The home is clean pleasant, hygienic and there is not any unpleasant odour. EVIDENCE: There is ramped access to all parts of the home, indoors and outdoors all of which is well maintained. The residents own furnishings were observed in their rooms. All but one room has ensuite toilet facilities at present and this will change following the refurbishment when all rooms will have toilet facilities. There are appropriate assisted bathing facilities and there will also be changes to these. The refurbishment will mean that those residents that do not currently have windows opening to the outside will do so in the future. The manager stated that the necessary statutory bodies are being consulted in the carrying out of this refurbishment. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28,30 Residents are in the care of safe and appropriately trained staff at all times. EVIDENCE: The residential forum or staff rota were not inspected on this occasion but discussions with the manager and the staff evidenced that there are appropriate numbers on duty at all times. Staff stated that there is good continuity in care and a happy working environment. Residents spoken to stated that there are adequate numbers of staff, one residents stated that there is sometimes a delay in staff attendance following using the call bell but that this is not excessive. The enhanced training programme was inspected and staff confirmed that there is a comprehensive training programme and good attendance. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32, 38 The manager is suitably qualified to run the home and residents and staff benefit from the management ethos. The health safety and welfare of the residents is protected EVIDENCE: The manager is a Registered Nurse and has many years experience in care home management. The residents and staff spoken to stated that the registered manager is approachable and has achieved many improvements in the home. One member of staff although not entirely happy with the changes in the home stated that the residents are well cared for and that staff can approach the manager to discuss problems. The manager has ensured safe working practices by drawing up and implementing policies and procedures covering moving and handling, fire safety, first aid, food hygiene and infection control. This was evidenced by looking at training records and by discussion with staff. The health, safety and welfare of residents is protected.
