This inspection was carried out on 14th December 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The Residents are treated with respect. Good opportunities are provided regularly to socialise with other residents through a variety of activities. Residents enjoy the high quality of food provided. Good quality assurance systems are used.

What has improved since the last inspection?

A record of all complaints is kept. All the required information about new staff is available. Sufficient staff members are trained in first aid. The home`s adult protection policy has been developed to include local details. The sensory garden has been installed and will be usable by the Spring. The new contractor has improved the housekeeping and catering arrangements. New weighing scales have been purchased. The deputy manager has started management training, and is responsible for training of staff. Training on dementia care has continued.

The total number of persons that may be accommodated at any one time must not exceed 50 of which up to 10 may be in either the DE(E) or MD(E) category 30th August 2005 Date of last inspection Brief Description of the Service: The Old Vicarage is a care home owned and managed by the charity Friends of the Elderly. The home is situated in a residential area of Moulsford and is set in attractive grounds accessible to all the residents. Accommodation is provided over two floors in single rooms with en-suite facilities. Residents have use of the communal facilities, including two day rooms and two dining rooms. The registered manager and her deputy home manage the home, with a team of registered nurses, carers, kitchen and housekeeping staff. A physiotherapist and an activities organiser are also part of the care team. This unannounced inspection took place one morning and early afternoon during a busy pre-Christmas day. Residents were entertained during the morning by a carol concert and had enjoyed mulled wine and mince pies. The inspector visited several areas of the home, assessed individual files, records and met with several residents, staff and management, then discussed issues arising with the registered manager and her deputy. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10. Residents are treated with respect. EVIDENCE: Several residents told the inspector that staff treated them with respect and that staff understand their need for privacy. The inspector saw the minutes of a recent residents’ meeting showing that issues raised by residents were seriously addressed. New staff are instructed on how to treat residents with respect during the induction period. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All of these standards were assessed. The activities coordinator understands the needs of the residents and provides a varied programme of activities to meet the needs of all the residents. Visitors are welcome at any reasonable time, and residents are helped to keep control over their lives. Good food is provided in pleasant surroundings. EVIDENCE: Residents told the inspector that they were satisfied with the routines of the home. The inspector met the activities coordinator who is in the home every weekday. The activities coordinator meets each new resident and is able to plan with the individual how to meet his/her social and recreational needs. A varied programme of activities is provided, and residents choose to take part in some all or none of the activities. Large group, small group and individual sessions are available, and take part in different locations around the home. Families usually manage financial affairs when the resident can do so no longer, and information about advocates is available at the home. Residents told the inspector that the food is usually good, and some said that it is very good. New providers manage the provision of food since October and the chef consults regularly with the residents for feedback about mealtime issues. The dining rooms are well planned and residents sit together in small groups at mealtimes. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 30. The home has a structured induction programme for new staff and regular staff training is provided. EVIDENCE: The deputy manager oversees the home’s induction programme for new staff, and allocates a mentor from the senior care staff to provide direction to new care staff. The inspector saw the induction record for a new member of staff, that included structured weekly meetings to discuss progress with the deputy manager. NVQ training is encouraged and four candidates are enrolled to begin the NVQ Level 2 course soon. Staff have an individual assessment of training needs annually, and regular training on appropriate topics such as continence care and tissue viability is provided. The inspector witnessed one incident of poor practice in moving and handling which was discussed with the deputy manager, who said she would address it with staff. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The home’s quality assurance system takes the residents views into account, but the results need to be shared with residents. Residents’ finances interests are safeguarded. EVIDENCE: An annual satisfaction questionnaire about the home’s services and facilities is provided to the residents and relatives, and a separate review of medication is carried out. The results are analysed and action plans are developed to address any issues identified as falling short of expectations. The inspector recommends that the results of the surveys be shared with residents so that residents and relatives know how the home is using the information to improve services and facilities.
