Conditions of registration: Date of last inspection 9th December 2005 Brief Description of the Service: The Rosewood is registered as a residential care home for three older people, one of who may be physically infirm. The service is provided by the owner/manager and her husband, (who does not provide personal care) and their daughters will assist if required. Whilst the Home is respectful of all religious beliefs, service users need to be sensitive to the religious traditions and beliefs of Jehovah Witnesses. The home is a detached bungalow set in its own grounds and is of domestic style and design. All three bedrooms are for single occupancy, one overlooking the garden at the rear is particularly spacious: another has a small conservatory and the third bedroom has recently been turned into a study/quiet room. There are no en-suite facilities, at present but the design of the bungalow means that each room is in very close proximity to the spacious and well-fitted bathroom. The area in front of the bungalow provides for a small car park, whilst the rear garden is attractively block paved, with a bird table and plants in pots. The Rosewood has attractive views of the countryside. There is a small shop within the immediate vicinity, and there is accessible public transport to the larger village of Milton approximately two miles away. A pub and church are close by, situated within Bagnall itself. The proprietor and her husband provide transport to health appointments, activities and outings in the family car. The fees charged are presently £300 per week. At the time of the inspection there was one person living in the Home and receiving a service. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over two hours. The total time spent, including pre-inspection preparation and writing the report was eight hours. This was a ‘key inspection’ and the core standards were assessed. The methodologies used were discussions with the person living in the Home and the owner/manager, Mrs Molly Hill. The medication systems were checked and a tour of the Home undertaken. The Home’s records were checked including care plans, risk assessments, maintenance records, training certificates and some policies and procedures. What the service does well:
The Home provides very personalised care and support in a homely and ‘family like’ setting. The person living in the Home receives individual attention and her needs and wishes are catered for. Past evidence suggests that this would be the same for any person living in the Home. Health needs are closely monitored and medical intervention sought appropriately. The routines in the Home are flexible, with the person choosing when to rise and when to go to bed and what to do with her day. The person living in the Home goes out into the community regularly, accompanying the manager and her husband on shopping trips, visiting friends and attending religious events. The records in the Home are well-maintained and are regularly reviewed. The manager ensures that her training is regularly updated. The environment is comfortable, clean and homely. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 4, 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People would not be admitted into the Home unless an assessment had been undertaken, ensuring that their needs can be met. EVIDENCE: Although the Home is registered to provide support and accommodation to three people, there is presently one person living at The Rosewood. She originally came for respite care and was known to the Home because she used to visit a friend who was resident there. The manager obtained an assessment and care plan from the Local Authority before the resident moved into the Home. She reported that the Local

The manager is not intending to admit any further people into the Home, although she has received enquiries from the Local Authority. The Home is owned and managed by Mrs Molly Hill assisted by her husband. They are practising Jehovah’s Witnesses and so is the person living in the Home. This gives them a mutual understanding and they enjoy attending the local Kingdom Hall services and bible classes together. The manager has kept her training updated and has the skills required to meet the needs of the person living in the Home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Health needs are closely monitored and medical attention sought when necessary, affording the person living in the Home confidence in the support received. The medication systems are managed safely and efficiently. EVIDENCE: Care records are well maintained and reviewed regularly. The manager has also assessed any risks associated with providing the care; this includes a moving and handling assessment. The manager has developed a ‘Health Profile’ for the person living in the Home and there is evidence that health concerns are closely monitored. The person living in the Home is enabled to access medical support, if required. She is

The privacy of the person living in the Home is respected. She is able to make private phone calls and see her visitors alone. Medication is stored, administered and recorded appropriately. The manager keeps good records and she has received medication training. The general practitioner reviews medication annually. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The person living in the Home is supported to be involved in her local community and follow her religious beliefs. The routines in the Home are flexible and based on the choices of the person living there. EVIDENCE: The person living in the Home is treated like a member of the family and as such regularly accompanies the manager and her husband when they go out. This includes shopping trips and visiting friends. They are all Jehovah’s Witnesses and attend religious meetings regularly. The manager keeps records of all trips and activities partaken in. Visitors are made welcome in the Home and they are able to meet privately. The person living in the Home chooses to eat in her room, but said that she could eat with the manager and her husband if she wished. The manager has a current food and hygiene certificate and she records the food provided daily. The routines in the Home are flexible and the person living in the Home can get up and go to bed when she chooses. A bell has been provided in case she needs assistance in the night. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The person living in the Home said that she felt safe and did not want to live anywhere else. EVIDENCE: The Home has a complaints procedure, a copy of which has been given to the person living there and is also displayed in the hall. There have been no complaints to the Commission for Social Care Inspection or the Home. Because of the nature of the Home any concerns would normally be discussed and resolved. The Home has a written policy and procedure regarding Adult Abuse and the manager covered this subject in her Registered Managers Award and National Vocational Qualification 4 training. The manager, her husband and their two daughters all have Criminal Records Bureau disclosures and financial transactions are recorded. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The Home provides a clean, safe and homely environment for the person living there. EVIDENCE: The Rosewood is a well-maintained and nicely presented family bungalow. The person living in the Home has her own bedroom and at present uses the family bathroom across the hallway. The manager is planning to refurbish one of the bedrooms with an en-suite bathroom, which will be offered to the person living there. Should she choose to have it, she will be able to choose the décor. The Rosewood DS0000008315.V336332.R01.S.doc Version 5.2 Page 16 Since the last inspection, a new kitchen has been fitted and the conservatory has been furnished to provide a really nice space looking onto the garden area. The manager has a three-year plan for the Home, which includes re-fitting the bathroom. There is an assisted bath chair. Maintenance records were seen to be in order, including Control of Substances Hazardous to Health information, fire safety, gas and electric records and Health and Safety risk assessments. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager updates her training and skills to ensure that she can continue to provide support safely and appropriately. EVIDENCE: The manager, her husband and two daughters work in the Home, although now that there is only one resident the manager provides most of the care. The daughters will support the Home if their Mother goes on holiday or has some time off. The manager has kept her training updated. In the last twelve months she has attended manual handling, first aid, medication, fire safety, continence care and she would like to attend a course on dementia care needs. The manager and one daughter have achieved National Vocational Qualification 4 and the Registered Managers Award and the other daughter has National Vocational Qualification 2. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Management of the Home is ‘personal’ in its approach, whilst maintaining an appropriate emphasis on record keeping and Health and Safety. The person living in the Home is well cared for and supported. EVIDENCE: The manager and owner, Mrs Molly Hill obtained National Vocational Qualification 4 and the Registered Managers Award in 2004. She provides most of the support to the person living in the Home. Policies and procedures are in place, which the manager reviews annually. Any financial transaction is recorded and signed for. The quality of the service is checked on an on-going basis, with open discussion between the person living in the Home and the manager and her husband. The manager has, in the past given out Quality Assurance questionnaires. The Home is well maintained and Health and Safety a priority. Accidents are recorded.
