0 0The Rosewood is registered to provide care for three older people, one place can be offered to a person with a physical disability. The service is provided by the owner/manager and her husband, (who does not provide personal care) and their daughters will assist if required. The home is a detached bungalow set in its own grounds and is of domestic style and design. All three bedrooms are for single occupancy, one overlooking the garden at the rear is particularly spacious. A conservatory is also available for people to use. The Rosewood has attractive views of the countryside. There is a small shop within the immediate vicinity, and there is accessible public transport to the larger village of Milton approximately two miles away. A pub and church are close by, situated within Bagnall itself. The proprietor and her husband provide transport to health appointments, activities and outings in the family car. The current fees for the service were Three hundred and seventy seven pounds per week. These fees were current at the time of the inspection. What the care home does well The Rosewood stands in its own grounds with clear views overlooking the village of Milton and Bagnall. The service provides a homely, comfortable, well furnished environment. The person has personal space and access to all the other areas in the service. Arrangements were in place for the continued health care and consultant appointments from other professional agencies. This was evidenced from the persons records during the inspection. The provider/manager provides a ambiance that is person centered, the AQAA told us that the person came first, this was obvious from observations and a discussion with the person. The person using the service is provided with an outstandingly well maintained environment, the recent total refurbishment of the bathroom and the provision of an shower en-suite facility for the person has enhanced her personal well being. The bungalow has a number of well appointed features which includes a small study and large conservatory, each area was well maintained. The provider uses extensive policies and procedures to ensure the safety and health of the person. She maintains a detailed care plan including the monitored dietary needs of the person. From the details in the AQAA and records provided the provider has undertaken an excess of thirteen training courses during the last twelve months including the Mental Capacity Act, Deprivation of Liberty Safeguards, Mental Care in Older life. The required mandatory training is also on going. What has improved since the last inspection? Since the last inspection the service has completely refurbished the main bathroom. The person has an assisted portable bath chair for her use. Within her large bedroom an en suite has been completed with a walk in shower fitted. This means that the person using the service experiences excellent quality outcomes. This unannounced key inspection was carried out by one inspector between the hours of 9 am and 1pm who used the National Minimum Standards for Older People as a basis for the inspection outcomes for the person who uses the service. Prior to the inspection we had sent an Annual Quality Assurance Assessment (AQAA) for the manager to complete, this was returned to us prior to the visit and within the requested timescale. The document was detailed and contained information relevant to the service. The completion of the AQAA is a legal requirement and it enables the service to undertake a self-assessment, which focuses on how well outcomes are being met for Care Homes for Older People Page 6 of 22 the person who was using the service. It also gives us some numerical information about the service. We were provided with documents which included, managers training record, menus, care plan, and financial records. We spent some time with the person using the service. We were told Its a pleasure to live here I dont know where I would be if I did not live here. They reflect the things that people have said are important to them: People are confident that the care home can support them. This is because there is an accurate assessment of their needs that they, or people close to them, have been involved in. This tells the home all about them and the support they need. People who stay at the home only for intermediate care, have a clear assessment that includes a plan on what they hope for and want to achieve when they return home. People can decide whether the care home can meet their support and accommodation needs. This is because they, or people close to them, have been able to visit the home and have got full, clear, accurate and up to date information about the home. If they decide to stay in the home they know about their rights and responsibilities because there is an easy to understand contract or statement of terms and conditions between them and the care home that includes how much they will pay and what the home provides for the money. This is what people staying in this care home experience: Judgement: People using this service experience good quality outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. People would not be admitted to the service without a full assessment of their needs being carried ensuring that their needs can be met. Evidence: The Rosewood is registered to offer care and support to three older people, there was at the time of this inspection one person accommodated and supported with her daily life skills and routine. The manager is also the provider, who knew the person from previous visits to the service to visit her friends. The care plan contained a social workers assessment and the assessment by the provider. There was a detailed history of the persons needs, physical, emotional, dietary and medical. The care was made readily available to us. The person was prone to falls this was identified and contained in the risk assessment. We evidenced the manager had planned and recorded regular monthly reviews. The AQAA told us that the provider makes sure the person is put first, this was evidenced Care Homes for Older People Page 10 of 22 Evidence: from observation of her daily routine, the manner in which the provider provides for the persons dietary needs. We were told that the provider does not at this time expect to admit any other person to the service. They reflect the things that people have said are important to them: People’s health, personal and social care needs are met. The home has a plan of care that the person, or someone close to them, has been involved in making. If they take medicine, they manage it themselves if they can. If they cannot manage their medicine, the care home supports them with it, in a safe way. People’s right to privacy is respected and the support they get from staff is given in a way that maintains their dignity. If people are approaching the end of their life, the care home will respect their choices and help them feel comfortable and secure. They, and people close to them, are reassured that their death will be handled with sensitivity, dignity and respect, and take account of their spiritual and cultural wishes. We have made this judgement using a range of evidence, including a visit to this service. Arrangements were in place for the service to meet the personal, health and emotional needs of the person living here. Evidence: The care plans are very well maintained and reviewed monthly. Within the plan were moving and handling assessments. We evidenced contacts with other professional agencies. The providers husband provides transport when necessary to any medical appointment. The care, privacy and dignity of the person was respected and protected at all times, the person was able to make private phone calls, the person makes a personal choice when riseing and is assisted by the manager. The provider ensures the safety of medication with the appropriate storage facilities, records and by the accredited training she has undertaken. The person chooses not to self administer her medication. We observed the person rise during the inspection we were told by her This is my normal time I will have breakfast later I do not have much appetite, I used to come Care Homes for Older People Page 12 of 22 Evidence: here before I moved in to see friends, I cannot imagine living anywhere else, I have everything I need. They reflect the things that people have said are important to them: Each person is treated as an individual and the care home is responsive to his or her race, culture, religion, age, disability, gender and sexual orientation. They are part of their local community. The care home supports people to follow personal interests and activities. People are able to keep in touch with family, friends and representatives. They are as independent as they can be, lead their chosen lifestyle and have the opportunity to make the most of their abilities. People have nutritious and attractive meals and snacks, at a time and place to suit them. There are no additional outcomes. We have made this judgement using a range of evidence, including a visit to this service. The person is supported by the flexible relaxed ambiance of the service. The routines are based on choice and meals provided are appropriate to meet her needs. Evidence: The person living at the Rosewood is treated as a member of an extended family and goes into the community with the providers. Her privacy is important when she has family visiting her. We were told in the AQAA that refreshments were provided this was confirmed by the person. I go to my room when my sons visit, I have a drink when I want one I only have to ask. We were told and observed breakfast being served when the person rose. I dont eat much but enjoy the meals I have. A comprehensive record of the meals taken by the person using the service was made available. This record ensures that the health of the person is monitored. The daily routine for the service was observed to be relaxed and flexible. They reflect the things that people have said are important to them: If people have concerns with their care, they or people close to them know how to complain. Any concern is looked into and action taken to put things right. The care home safeguards people from abuse and neglect and takes action to follow up any allegations. People’s legal rights are protected, including being able to vote in elections. This is what people staying in this care home experience: Judgement: People using this service experience good quality outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. The person using the service was protected by robust policies, staff awareness and training. The person was aware of the complaints process and felt safe. Evidence: Within the numerous policies made available to us was a procedure regarding Adult Abuse. All of the people involved in the operation of the service and the care of the person have Criminal Record Bureau disclosures, and these were evidenced in the files. The services complaints process was displayed in the entrance porch, Statement of Purpose and Service Users Guide. Details had been changed to reflect the new Care Quality Commission. We were told by the person that she knew who to speak to I would speak to the manager, I could always speak to my sons. The AQAA told us that the service had received no complaints. They reflect the things that people have said are important to them: People stay in a safe and well-maintained home that is homely, clean, pleasant and hygienic. People stay in a home that has enough space and facilities for them to lead the life they choose and to meet their needs. The home makes sure they have the right specialist equipment that encourages and promotes their independence. Their room feels like their own, it is comfortable and they feel safe when they use it. We have made this judgement using a range of evidence, including a visit to this service. The service provides an exceptionally clean, safe, and homely environment for the person using the service. Evidence: When entering The Rosewood it presented as an extremely well maintained service, the service was the family home of the providers. Located overlooking Bagnall at the top of a rise the home has a public transport route outside the door. The service was immaculate throughout a credit to the owners. The recently refurbished bathroom was impressive in its size and design. The person using the service had an en-suite facility in her large bedroom, she told us I like my bedroom, there is space and I like to watch the television. The conservatory at the rear of the service was comfortable and well furnished giving a view over the structured garden. They reflect the things that people have said are important to them: People have safe and appropriate support as there are enough competent staff on duty at all times. They have confidence in the staff at the home because checks have been done to make sure that they are suitable to care for them. Their needs are met and they are cared for by staff who get the relevant training and support from their managers. There are no additional outcomes. This is what people staying in this care home experience: Judgement: People using this service experience excellent quality outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. The manager and staff are well trained to support the person using the service ensuring their safety and appropriate care. Evidence: There are no external staff employed at The Rosewood. The provider/manager with her husband and two daughters work and cover holidays at the service providing daily support to the one person. We saw evidence from the records of the impressive training undertaken by the manager in the last twelve months this has included: Nutrition in the Elderly, Mental Capacity Act, Risk Assessments, Mental Care in Later Life, Stroke awareness, Deprivation of Liberty Safeguards. The manager had plans to update her Moving and Handling training, other mandatory training has been undertaken. The provider/manager has achieved the Registered Managers Award level 4. This qualification has also been achieved by one daughter. The remaining daughter has a National Vocational Qualification level 2. From the records provided each person involved in the running of the service had a current Criminal Records Bureau clearance. They reflect the things that people have said are important to them: People have confidence in the care home because it is led and managed appropriately. People control their own money and choose how they spend it. If they or someone close to them cannot manage their money, it is managed by the care home in their best interests. The environment is safe for people and staff because appropriate health and safety practices are carried out. People get the right support from the care home because the manager runs it appropriately with an open approach that makes them feel valued and respected. The people staying at the home are safeguarded because it follows clear financial and accounting procedures, keeps records appropriately and ensures their staff understand the way things should be done. They get the right care because the staff are supervised and supported by their managers. We have made this judgement using a range of evidence, including a visit to this service. The service is operated to the best interest of the person using the service by a qualified competent manager. Evidence: The manager and provider has achieved the Registered Mangers Award and a National Vocational Qualification level 4. With the support of her husband, she is the main carer for the person using the service. The AQAA told us that the person is always put first. This was confirmed by the person who told us Its a pleasure to live here I dont know where I would be if I was not here. We were provided with an impressive policy and procedure file, while the service remains small the policies covered all the Codes of Practice. We evidenced from the records that the policies written by the provider were reviewed annually. Financial transactions were signed by both parties. The provider has been in the caring profession for a number of years and continues to provided quality care while recognising the person rights, privacy, dignity, and safety.
