This inspection was carried out on 9th November 2005.

The inspector found no outstanding requirements from the previous inspection report, but made 2 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The Cave appears to provide support and personal care in comfortable homely surroundings. Many of the staff team have worked in the home for several years and are aware of each residents needs. The manager is experienced having worked in the home for 13 years and a good standard of recording is maintained. The majority of the residents have active lives attending day services and maintaining contact with their families. They are encouraged and supported to use community facilities.

What has improved since the last inspection?

Some improvements have been made to the home with a laundry and food store having been upgraded. Some carpets have been replaced and fire authority approved door restrictors have been installed.

What the care home could do better:

The manager should demonstrate that residents are involved in more in their care planning. The three monthly reviews should include their comments and any goal plans should demonstrate how residents fulfilment is achieved. The home should consider improving infection control by using liquid soap and towel dispensers. Some privacy and health assessments should be addressed.

Date of last inspection 2 March 2005 Brief Description of the Service: The Cave is converted from a large detached domestic residence set in its own grounds on the outskirts of Allendale. Care and support is provided for twelve residents with a learning disability, three of whom are elderly. Two bedrooms are shared and non have en-suite facilities. There are two communal rooms used as lounge and dining rooms. The home has two bathrooms one of which also has a separate shower cubicle. Outside a summerhouse is used for activities and crafts. This home would not be suitable for anyone with a physical disability as there are short flights of stairs between areas on the ground floor and first floor and no lift between floors. This is the home’s first annual inspection. The inspection was unannounced and took 3 ½ hours. In that time the manager and both staff on duty were spoken to, a tour of the premises was undertaken and a selection of the records held in the home were reviewed. Three residents were available during the inspection and although their ability to communicate was not good, time was spent in their company to assess the quality of care provided. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2. Residents are provided with good information regarding the home and each resident has a full assessment of their needs in place. EVIDENCE: The home has been full since the last referral in 1997. Each resident was provided with a copy of the home’s Statement of Purpose. Four case files reviewed each contained a full assessment of needs that was well written and evaluated recently. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 9. Comprehensive plans are in place detailing the support each resident requires. The residents’ involvement in these plans appears limited. Risk assessments to support residents’ independence are produced. EVIDENCE: Four case records reviewed contained comprehensive information relating to the assessed needs of each resident. At three monthly intervals the key worker reassesses these plans and makes any necessary adjustments. The plans do not appear to be done in consultation with the resident and little use is made of goal plans to enhance fulfilment and meet changing needs. Comprehensive risk assessments are used to promote independence both within and outside of the home. These assessments are regularly reviewed and updated. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15, 16. No resident is involved with paid or unpaid employment, however they generally have active social lives and take part in community activities. Residents have regular contact with their families and have varying daily routines. EVIDENCE: The case records reviewed identified a resident group that are involved in varied regular activities. The range of activities residents are involved with outside of the home includes; day centre services, outings with an enabler, a drama group, swimming and shopping. Some residents can travel independently and visit the village centre and take part in local events. Holidays taken by residents this year were generally organised individually with family or staff. Residents visited; Spain, Crete, Paris and Scotland. Many residents are close to their families and have regular contact both by visiting and by telephone. Residents use community facilities alone or with the support of staff if required. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20. Good personal and healthcare support is in place in the home. The frequency of some community support should be reviewed. Staff are supported by good records and community professionals. The home’s medication system is good. EVIDENCE: The residents’ have all lived in the home for several years and have used community services throughout this time. Staff in the home regularly monitor each residents health and all interventions are recorded. Regular appointments to maintain health are in place although the manager was asked to review some appointments related to opticians and dentists. The personal support required by each resident is detailed in an individual plan, mobility is assessed and each residents dependency is monitored. Staff are very experienced and reacted well to one resident who was having an emotional moment. The home has recently changed the medication system. The system for storage and administration was reviewed and was found to comply with current pharmacy guidance. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23. A complaints procedure is in place in the home. Staff are well trained and aware of issues relating to vulnerable adults. EVIDENCE: The home has a complaints procedure and a log to record any matters of this nature. No complaints have been made to the home since the last inspection. Staff have received training in issues relating to abuse awareness and the protection of vulnerable adults. Appropriate policies and procedures are available to staff in the home. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 30. The home is comfortable, safe and homely. There is a reasonable standard of cleanliness. EVIDENCE: On a tour of the building the home was found to be generally warm and homely. Residents’ bedrooms were individual in style with a general standard of cleanliness. Recently fire authority approved door restrictors have been fitted to some doors to assist movement through the home, some carpets have been replaced and the laundry and foodstore have been improved. The manager was advised that liquid soap and towel dispensers should be available at communal washing points to promote infection control. The manager considers this inappropriate in this home due to the actions of a resident. A risk assessment could be produced to determine the best course of action. A screen was not available for use in shared rooms should privacy be required. A training and development plan is in place for each staff member and these show that a good standard of training is in place. Should any new staff be required the home has an induction system that meets the desired standard. JUDGEMENT – we looked at outcomes for standard(s) 42. Residents live in a safe home. EVIDENCE: The standard of safety in the home is good. Staff are trained in health and safety. Fire logs are well recorded and staff instruction in fire safety is in place. Electrical items are checked annually and a competent person has assessed the home’s wiring. Water temperatures are safe and fridge temperatures are regularly checked. The water system in the home has not yet been assessed for risks posed by Legionella.
