This inspection was carried out on 25th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The personal care and assistance provided to the service users have been maintained in a positive way. The service users appeared well cared for. Their personal preferences are respected and choices are given.

What has improved since the last inspection?

The standard of service, especially the administration of medication and record keeping is well maintained since the management of services was restructured with the appointment of a new deputy manager to assist. Staff have well defined roles and responsibilities and they are encouraged and supervised to perform those tasks to a high standard. The administrative office has been moved to the top floor. The vacant office is currently being renovated to provide an additional bedroom with en-suite facilities. The training room on the top floor is now in full use, with teaching aids and other equipment in place.

What the care home could do better:

The external ground is tidy and accessible to the service users. However, the fencing section of the back garden needs more attention to ensure that the newly planted trees will fill the gaps in the metal fence to ensure safety. The registered manager has assured the inspector that more evergreen trees will be planted over the next few weeks. The management must ensure the safety of the service users and staff while renovation work is carried out to transform the vacant room into an en-suite bedroom.

This home is registered for 21 people with old age. Date of last inspection 15th July 2005 Brief Description of the Service: Troutstream Hall is a residential home for 21 service users in the Old Age category. It is provided by Troutstream Hall Trust, a Christian Charitable Trust. The building is a large detached house that has been converted into a care home. It is situated in a quiet residential area near to the village of Rickmansworth. The M25 and M1 motorways are a few minutes drive away. There is a driveway with parking spaces to the side. The spacious entrance hall leads to the dining room and kitchen. The staff room and the laundry room are also on the ground floor. The bedrooms, of varying sizes, are on the ground and first floor, which is served by a lift. Each bedroom has a hand basin. The toilet and bathroom facilities are nearby. The communal rooms include two well proportioned lounges. The administrative office and the staff training room are on the top floor. There is a lift to the second floor There is an attractive front and back garden, surrounded by mature trees and bushes. The patio areas have garden furniture and potted plants. All the rooms and gardens are accessible to wheelchairs. The unannounced inspection was carried out on 25th January 2006. The registered manager and the deputy manager were present. There were 19 service users in the home. Most of the service users were resting in their bedrooms when the inspector arrived. A number of them were interviewed about the service and care provided. They responded with positive comments. All the service users appeared well dressed and well cared for. The care plan files and medication charts were examined. The inspection included a conducted tour of the premises, including the new office and the training room, both situated on the top floor. (Please see below for details of the inspection findings). No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5. A comprehensive assessment is carried out before the prospective service user is admitted into the home. A trial period is arranged before the contract is agreed. EVIDENCE: The Statement of Purpose has been updated. Each service user has a Service User Guide given at the time of admission and it is kept in their bedroom. A service user who was recently admitted confirmed that a full assessment was carried out before she moved into the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10. The care planning system ensures that the health and personal care needs of the service users are being met. They are treated with respect and their right to privacy is upheld. The administration of medication is in accordance with legislation. EVIDENCE: The written care plan of each service user has been reviewed and updated since the last inspection. A senior carer has been assigned to check all the care plans to ensure that they are consistently maintained and updated. The administration of medication is well maintained. All records are accurately completed and they are up to date. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15. The service users are given every opportunity to participate in all aspects of routine living. They are encouraged to maintain links with their family and friends. The meals provided are nutritious and balanced. Choices are given. EVIDENCE: The home has no activity co-ordinator. Staff are involved in the daily activity. One service user interviewed said that she prefers to do her daily walking exercise outside the premises. Her wishes were respected. The service users interviewed are pleased with the meals provided. The menu is reviewed regularly. The preferences of service users are taken into consideration. The service users interviewed confirmed that their family members do visit them. On the day of the inspection there were no visitors in the building. EVIDENCE: All concerns raised are dealt with immediately and appropriately. The facilities provided are adequate and well maintained. The management must ensure the safety of the service users and staff while renovation work is carried out to transform the vacant office into an en-suite bedroom. EVIDENCE: The premises appeared homely and clean. There is an ongoing maintenance programme. The administrative office has been moved to the top floor. The vacant office is currently being renovated to provide an additional bedroom with en-suite facilities. The training room on the top floor is now in full use, with teaching aids and other equipment. The gardens are well maintained. The registered manager has assured the inspector that more evergreen trees will be planted in the coming weeks to make the back garden more attractive and to provide additional fencing to the existing metal fence. The home is run in the best interests of the service users. Their health, safety and welfare are promoted and protected. EVIDENCE: The management restructuring has improved the administration and management of the service, which is now well maintained with the assistance of a deputy manager, appointed since the last inspection. Staff are given specific tasks and they are encouraged and supervised to perform those tasks. In particular , the standard of administration of medication and record keeping, including written care plans has improved. Service users interviewed gave positive feedback regarding the care and service provided in the home.
