If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Troutstream Hall is a residential home for 21 people over 65 years of age. It is owned and run by Troutstream Hall Trust, a Christian Charitable Trust. The building is a large detached house that has been converted into a care home. It is situated in a quiet residential area near to the village of Rickmansworth. The M25 and M1 motorways are a few minutes drive away. There is a driveway with parking spaces to the side. The spacious entrance hall leads to the dining room and kitchen. The administrative office, staff room and the laundry room are all on the ground floor. The bedrooms, of varying sizes, are on all three floors. Each bedroom has a hand basin with toilet and bathroom facilities in close proximity. Four rooms have been converted to ensuite bedrooms. The communal rooms include two well proportioned lounges. There is an attractive front and back garden, surrounded by mature trees and bushes. The patio areas have Annual Service Review Page 2 of 6 2 1 0 1 2 0 0 9 garden furniture and potted plants. The home has a lift to all the floors. All the rooms and gardens are accessible to wheelchairs. The managers office and an equipped training room is on the top floor and there is a large room on the ground floor where medication and records are stored. The weekly fees range from £554 to £623 according to the size of the room and the facilities in the room. (Correct at January 2010). Fees are subject to review where the dependency level of people living in the home is assessed as having changed significantly. Charges for additional services such as hairdressing and chiropody are priced individually and set out in the individual service contract. Copies of the latest inspection report from the Care Quality Commission (CQC) are available in the home. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all of the information we have received about Troutstream Hall since the last key inspection in January 2009. This includes the Annual Quality Assurance Assessment (AQAA) submitted by the home in November 2009. The AQAA is a selfassessment document that focuses on how well care outcomes are being achieved for people living in Troutstream Hall, it also includes some useful statistical information, for example about staff training and complaints. We have sent surveys to people who live in Troutstream Hall, relatives, carers and advocates of people who live there and to care staff who work there. Where these have been returned already we have taken them into account in this review, any surveys received after the review is finalised will be read, recorded and assessed and will be used to inform the future regulation of Troutstream Hall. We have also made use of any comments or information received about Troutstream Hall since our last key inspection; this could be from health or social care professionals associated with the home or any other people or organisations who are involved with the home in any other capacity. We have also regularly received information from the home in the form of notifications of significant events affecting the home or people who live there. What has this told us about the service? There is a very friendly atmosphere and a homely feel with friendly staff and very good quality of food and It gives the best care in a warm and happy atmosphere. More like a family home than a residential home for the elderly were some of the comments made about Troutstream Hall in surveys returned to us. The AQAA was reasonably well-completed and provides an adequate and realistic assessment as to what the home does well and what are the areas where performance could be improved in order to provide an even better care experience for the people who live in Troutstream Hall. Plans for improvement include further work on the homes built environment to update and improve it and further work to expand the activities on offer, for example by increasing the number of people able to drive a mini bus and by developing a more person centred, individualised programme of activities. This reflects one of the few consistent themes in surveys received, which concerned what was said to be a limited activities programme. One suggestion made in answer to the question; What could the home do better was employ an activities co-ordinator. The AQAA informed us that 100 of all permanent care workers have achieved the required level of NVQ training in care. It also informs us that there have been no complaints received in the past 12 months. There are currently no outstanding concerns raised with the CQC by other health or social care agencies or professionals. Since the last inspection one concern was raised with the CQC about the homes policy and practice in respect of criminal record bureau Annual Service Review Page 4 of 6 checks. The Trustees and the Manager co-operated fully in addressing this matter satisfactorily and the AQAA includes details of enhanced policies and procedures that have been subsequently put in place to offer enhanced protection to people who live in Troutstream Hall. The assessment of Troutstream Hall following the last key inspection in January 2009 was that it was a home that achieved good care outcomes for the people who live there. No events have occurred or information been received since then to suggest that assessment is still not valid and appropriate. What are we going to do as a result of this annual service review? We can inspect Troutstream Hall at any time if we have concerns about the care, welfare or safety of people who live there. There is nothing to indicate that an inspection is required in the immediate future. The next key inspection of Troutstream Hall will take place in line with the developing methodology of the CQC. As with all registered care services information about Troutstream Hall will continue to be gathered and the performance of Troutstream Hall will continue to be subject to a continual process of assessment, evaluation and review, with appropriate regulatory activity being undertaken as indicated.
