Conditions of registration: 1. Service users under the age of 65 years are restricted to one (1 ) whose DOB is 02/04/1943. Date of last inspection Brief Description of the Service: Wantsum Lodge provides residential care for up to 32 older people, the Home is attached to a large building providing accommodation for the elderly and accesses Wantsum Lodge for catering and laundry services. The Home is situated in a residential area in the coastal town of Ramsgate, within easy walking distance of shops, post office and churches, and within a short drive of the town centre with community facilities such as health centres, library, railway station and a shopping centre. There is no off street parking but ample parking is available on the streets to the front of the building. The fees for support from the home are set during the assessment period and are very individual to the needs of the service user, depending on the level of support required and the staffing numbers provided. The average fee levels, as a guide at this time, are between £303.00 and £520.00 per week. Wantsum Lodge DS0000023619.V307465.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection and the process consisted of speaking with the manager, staff members, service users and also included a review of records, documents and undertaking a tour of the premises. The inspector spoke to 11 service users during this inspection and all comments were positive and confirmed that staff are caring and supportive. What the service does well: What has improved since the last inspection?
Some areas have been redecorated and as rooms become available, redecoration is carried out before a person moves back into the room. The dining room has non-slip flooring, there is new carpeting in two service user rooms and all hallways have been redecorated. The hairdressing room has been redecorated and the front of the building is also due to be painted. This redecoration programme needs to continue to ensure all areas are at an acceptable level. What they could do better:
While the medication storage room has been de-cluttered to some extent, this area is still not in a condition that supports safe and appropriate practice. The sink was covered with items that should have been stored away and the sink was giving off offensive odours. The acting manager explained that this has been investigated and no cause has been found, however, this is not the appropriate environment for medication storage. The home would benefit from an alternative, appropriate storage area that provides adequate space to support appropriate practices. The medication fridge showed temperatures that are very erratic and are often above advised levels. Staff must be aware of acceptable temperature levels and know what action to take if these are not being met. In house procedures are required to ensure all practices in the home fully support the health and well being of service users. Discussions were undertaken with the acting manager about procedures, particularly with regard to medication procedures and adult protection matters. The medical administration record (MAR) sheets were up to date and fully completed but some scribbling out was found. Anything written in error must be crossed through with one line and explained at the bottom of the page. These are official records and need to be completed as such at all times. Daily records are completed and dated and signed but these observations are not ensuring the full well being of service users. Comments state that food is eaten and the person ‘appears fine’. This is not evidencing that observations and conversations have been undertaken. Other signs of well being are not written in the daily reports and while these might be undertaken, files do not show this. Fuller and more appropriate information needs to be recorded on care plans to ensure all needs are being met. The flooring in the laundry area is old and very damaged. The flooring does not meet with standards that state flooring must be impermeable to ensure the control of infection. There is a room allocated for staff if they wish to use it, but again, this area is in need of redecoration and refurbishing to make this more suitable for staff to take their breaks. The kitchen, freezer area and dry storeroom were found to be in need of cleaning. Floors were dirty and empty boxes had been stored next to the freezers. The kitchen area was very cluttered on all surrounding surfaces, with

The flooring in the kitchen and adjoining areas is old and very marked. Floors do not appear to have been regularly cleaned and there is a need for a cleaning schedule to be in place to ensure hygiene in all areas. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3&6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Pre admission assessments are carried out to ensure any decision to move into the home is appropriate. Intermediate care is undertaken with appropriate support to maximise independence and facilitate a speedy return home. EVIDENCE: All new admissions are undertaken following a pre admission assessment to ensure the placement is appropriate. Files seen at this time did contain pre admission assessments that are undertaken by the manager or deputy manager. Service user guides are available in each individual room to give additional information and support to those moving into the home. Intermediate care and rehabilitation is undertaken using appropriate equipment plus assessed support from care staff and healthcare professionals.

Healthcare appointments are fully recorded and show that GPs and district nurse provide support in the home and all outcomes are also recorded. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. All the appropriate information is recorded within the service user care plans, however, these have not been reviewed regularly to ensure the well being of service users. Medication checks are not fully compliant with set guidance, this does not ensure the well being of service users. Service users are treated with respect and the right to privacy is upheld. Staff must recognise that these are official records and need to be completed as such at all times. Senior staff are responsible for dispensing medication following an assessment of competencies. Boots provide training on dealing with medication in the home. Records contained some photographs of service users and stated personal details and any allergies. The manager explained that staff are in the process of providing new photographs for medical records. Medication was found to be locked, all controlled drugs that were checked at this time, matched the levels recorded and all returned medication is recorded and signed for. As stated, the acting manager has been in post for four months, however, she is aware of the need to review care plans and is in the process of doing this at the time of this inspection. The review of care plans must be an ongoing routine that is regularly undertaken and clearly evidenced on files. Some staff are still not taking full responsibility for this practice and it is hoped that the current review of files will continue on a regular basis. Daily records are completed and dated and signed but these observations are not ensuring the full well being of service users. Comments state that food is eaten and the person ‘appears fine’. Other signs of well being are not written in the daily reports and while these maybe undertaken, evidence is not found on files. Fuller and more appropriate information needs to be recorded on care plans to support care needs are being met. Records sampled at this time, evidenced access to health professionals, with full recording of any change in medication. CPN, GP and care manager visits, consultation and telephone calls are all clearly recorded on care plans. The acting manager reviews any continued cases of falls on a monthly basis. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Choice and autonomy are supported and encouraged, ensuring individuals have control over their own routines and life. Service user needs are met by the home and family, friends or advocates are encouraged to maintain contact. EVIDENCE: Service users spoken with at this time stated they are well cared for and are comfortable in the home. Staff are always available to help when needed and are ‘very caring’. Staff and service users were seen to be enjoying a good relationship and one visitor stated staff are ‘welcoming and caring’. Celebrations are enjoyed in the home for special occasions and one person was celebrating her 105th birthday on the day of this inspection. An afternoon buffet was planned and family and friends were visiting, as were the local mayor and a reporter from the local newspaper.

There is a choice of food and service users spoken to at this time stated meals are not rushed and there is always an alternative if requested. Meal times are staggered and service users were seen to be arriving at different times for meals. The acting manager stated that those wishing to eat in their rooms are supported to do so and service users confirmed this. Notices are posted in the home to inform visitors and family members of service user meetings. The manager also explained that she speaks to service users individually on a regularly basis. Service users have the opportunity to undertake exercise weekly, music is provided regularly and quizzes, beauty sessions, outings with Age Concern and concerts are also undertaken. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. A copy of the service user guide is available in each room and there is a complaints policy in place. There are no procedures for staff in the event of any abusive situations being suspected or reported. This is not ensuring the safety of service users or supporting staff to undertake appropriate action. EVIDENCE: A complaints policy is in place to support anyone who may have any problems. Service users spoken to at this time confirmed they can talk to the manager of staff and feel that they will be listened to. Visitors were seen being welcomed into the home and appeared comfortable when moving about the building. The manager explained that a good working relationship has developed between the home, visitors and family members. This does enable discussions to be undertaken when any matters arise and this supports a quick and appropriate outcome for all concerned. All visitors support special events in the home and have undertaken fund raising in the past to help the home with special purchases. A comments book is available for use in the front entrance. Money is securely stored, records contain two signatures at all transactions, receipts are kept on file and audit checks are carried out every two weeks. There are no abuse procedures in place to support staff and protect service users in the event of any abuse being suspected or notified. Policies are not specific in all areas and this is not supporting staff, service users or ensuring actions meet current legislation. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 24, 25, 26 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Rooms, communal areas, bedding and bathroom areas were, in the main, found to be clean and odour free at this time. While redecoration and some refurbishment has enhanced areas of the home, other areas remain in need of total refurbishment before the home reaches an acceptable standard throughout. Some areas of the home required thorough cleaning. EVIDENCE: Wantsum Lodge DS0000023619.V307465.R01.S.doc Version 5.2 Page 18 Bedrooms are being decorated where possible but some minor repairs that have been outstanding for some time will make rooms more homely and comfortable. Some hallways have been redecorated, the dining room has new, non- slip, flooring and the ground floor toilets have been painted. The lounge area has been painted and new blinds are on order, however, the carpet is frayed and is to be replaced. The external area of the building is also to be completely repainted. Hand written notices are stuck around rooms and in bathroom areas. These do not ensure a homely or appropriate environment for service users and visitors. If essential, typed and laminated notices would be more professional and the placement of these notices must be considered and the impact on the environment recognised. Bathroom and toilet areas have plastic bags that are tied to towel rails for use by staff. These should be in dispensers that are fixed in place to ensure an appropriate environment is created in all areas of the home and risk assessments undertaken. Flooring in some toilet areas is heavily stained and requires replacing. One large room has a stained ceiling and another room has a missing ceiling tile. This has been missing for some time, while a tile was obtained and found to be too heavy, this patch is in need of attention. Some form of regular maintenance checks need to be carried out to ensure all areas are kept in an acceptable state of repair. The flooring in the laundry area is old and very damaged. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Staff are caring and consider the dignity of service users, creating a relaxed environment. Training is now monitored throughout the year and training needs are being identified and booked. EVIDENCE: Staffing at the time of this inspection consisted of the acting manager, the deputy manager, two senior carers, two care assistants, two domestic staff, one cook, one kitchen assistant and one laundry person. The deputy manager was additional on shift at this time as part of an induction programme. Service users confirmed in conversations that staff are available when needed and do provide appropriate support. One visitor also confirmed that the person living in the home is well cared for and happy and staff are always available and welcoming. New staff have recently been recruited and will start work following checks and an induction programme is then completed. The acting manager stated that all new staff work with experienced staff until full CRB checks are received.

She is also to undertake a training the trainer qualification in the area of abuse. A training matrix is currently being developed in the home to evidence training that has been undertaken. The area manager monitors training and ensures all staff are trained in core areas and refresher training as necessary. Staff meetings are regularly undertaken with minutes being signed by all staff when read. A member of administration staff from another home in the group was reviewing staff files to check all documentation was in place. Staff files sampled showed that most required paperwork is on records, but one file did not have proof of address and one other did not have a picture to confirm identification. While this had been obtained for CRB checks, copies of these are also to be kept on file in the home. Wantsum Lodge DS0000023619.V307465.R01.S.doc Version 5.2 Page 21 Management and Administration

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,38 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home is run in the best interests of service users and financial interests are safeguarded. While there are areas of the home that do require attention and action, the manager and staff group do, in the main, consider the health and welfare of service users. EVIDENCE: The current acting manager has worked in care for the past ten years, progressing to senior carer and has now been in the post of acting manager in
Wantsum Lodge DS0000023619.V307465.R01.S.doc Version 5.2 Page 22 the home for four months. She is booked to start RMA training and also to train as a trainer regarding adult abuse. The manager hopes that this training will then fully inform the development of full procedures. The fire record book was up to date and regular fire safety checks are undertaken. Discussions about the new fire safety legislation and the need for proactive assessments, such as yearly fire safety assessments, were undertaken. The need for developing ‘In house’ procedures was discussed, especially in the areas of safety such as abuse and medication. There is a comments book available for visitors to use in the main entrance hall. All incident reports are reviewed on a monthly basis and regular supervision sessions are being booked with the manager. There are clear signs for emergency and fire exits, fire extinguishers are appropriately placed, clearly signed and serviced. There were no COSHH items in evidence, and cupboards with materials posing a risk were appropriately locked at this time. Other checks confirmed that regular maintenance is carried out regarding gas equipment and parker baths. The staff group and service users were observed to be relaxed and confident when dealing with the manager during this inspection. In the main, staff do try to ensure the safety and well being of service users. This was confirmed through discussions with service users at this inspection, who stated they feel cared for and staff are always available when needed.
