What the care home does well

The focus of this inspection was to check if requirements had been met. Because of this we did not look at all aspects of the service and may therefore not have identified some positive things about the home.

What the care home could do better:

Some risk assessments are not up to date, which could place people at risk of harm or injury. We have asked the manager to make sure that a full risk assessment is carried out whenever potential risks are identified, and that strategies are put in place to manage any risk.The arrangements for calling staff at night could mean that on occasion, staff might not know that people need assistance. We are making a requirement for the manager to carry out individual assessments for everyone who lives in the extension, to ensure that people can be confident that staff will know if they need assistance during the night.

It is within walking distance of Ross-on-Wye town centre, and is also on a bus route. The home is registered to provide personal care for up to thirty-six people who are over sixty-five years of age. West Bank has thirty-three bedrooms, most of which are en-suite. This is the first inspection of West Bank since the change of ownership earlier this year. The home is now owned by Buckland Care Ltd. Care Homes for Older People Page 3 of 8 What we found: We carried out this inspection to check if the home had complied with the requirements which we made at the inspection on 7 October 2009. We also wanted to look at how the home makes sure that people are looked after at night. We looked at care plans for some of the people who live at the home. We saw that some care plans had been reviewed recently, and there were improvements since the last inspection. The manager is clearly working hard to make sure that all the care plans are up to date. Although some care plans had not been reviewed for a while, the records of care provided by staff showed that people were being well looked after. One persons records showed that they had been assessed as being at risk of falling. There was no current risk assessment to take into account their changed circumstances, following an illness and subsequent change in mobility. We also looked at the homes arrangements for night-time care. Records showed that the manager has taken on new night staff to bring staffing up to four each night. This means that there are two staff in each part of the home during the night. The manager told us that the home no longer uses sleeping-in staff. We saw that the home keeps records of when staff check people during the night. The record sheets were seen, and showed that checks are made two hourly throughout the night. We saw that every bedroom has a call system. In the bedrooms in the extension, the call bells are wall switches, some of which were not accessible. For example, we saw that, in one bedroom, the wall switch was positioned on a wall behind a chest of drawers, and in another the wall switch was behind a heavy armchair. The manager told us that not everyone living at the home would be able to use a call bell, due to their dementia illness. In these circumstances, it is very important that staff are able to hear when someone needs assistance. The manager told us that the night staff do the laundry during night, and she agreed that staff in the extensions laundry room would not be able to hear calls from some rooms. The manager told us that, for most of the night, staff base themselves in the lounge, which is by the staircase and from where they should be able to hear anyone who needs help.
