The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home does well at maintaining contact and communication with relatives, involving them in all aspects of their family members care. There is a high emphasis on treating service users with dignity and respect and ensuring that their needs are met. Several comments made by service users and their relatives highlighted the care provided and the cleanliness of the home and quality of food served.

What has improved since the last inspection?

The manager has worked well to meet the requirements and recommendations made at the last inspection. Staff especially in studies leading to NVQ qualifications has undertaken further training.

What the care home could do better:

There were some areas that may need to be redecorated in the near future to maintain the homes appearance but generally the home was well maintained. Hot water temperature records showed some fluctuation and this needs to be looked at to ensure temperatures do not exceed the recommended level.

The home is situated in a small residential thoroughfare within the County Durham village of Billy Row, approximately 1.5 miles north of Crook Town. The semi rural setting around the village green features the Post Office, general store, church and pub and is on a regular bus route to Crook, Bishop Auckland, Weardale, Darlington, Durham and Newcastle. West Lodge was originally a large stone built country house and has been extensively altered and extended to provide accommodation on the ground and first floor. The home offers single and double room accommodation and all bedrooms are equipped with nurse call alert, plug-in points for TV and telephone. Communal TVs are provided in every lounge, a hi-fi system in the main lounge and the piano and organ in the dining room/conservatory on the ground floor. A passenger lift provides access to the first floor, as does the central stairway. Throughout the home is well equipped, decorated and furnished to a very good standard whilst retaining a domestic, homely environment. The home is surrounded by attractive well cared for gardens with seating and good access. There is ample parking space for visitors. There is a friendly and welcoming atmosphere and good links are well established with the local community in which the home enjoys a well-deserved reputation for providing good care. The inspection took place on 25th January 2006 over a period of 4 hours. During the inspection time was spent talking to service users, relatives, staff and management. A number of records were looked at and the grounds and building itself were inspected. Discussions with service users and their relatives found a high degree of satisfaction with the care provided and the rapport that had developed between themselves and the staff. Comments made by relatives suggested that the communication between the manager and relatives was “extremely good” and that the staff are “always caring, patient, willing to listen” and demonstrated “excellent care practices”. Service users comments were also favourable. One service user said, “quality care, top class” and “the staff are very protective, I feel secure”. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 5 and 6 The home offers the opportunity of trial visits and assessments to ensure the home is suitable. EVIDENCE: The Director said prospective service users were invited, with their relatives, to come to the home for a visit to see the facilities offered, before making the decision to live there permanently. The manager would also conduct an assessment using a pre admission checklist and provide information about the home and useful telephone contact numbers. The home did not provide an intermediate care facility. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Arrangements are in place should a service user wish to control their medication. EVIDENCE: Currently there are no service users staying at the home who have expressed the wish to control their own medication. The Director said if a service user wished to control their own medication, a risk assessment would be carried out to ensure their safety. A metal safe had been purchased to contain any medication required by the service user on completion of the risk assessment. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 and 15 The home encourages service users to retain control over their lives and provides a good variety of satisfactory meals. EVIDENCE: Service users said they could make choices in their lives including when to get up and go to bed and added that there were always plenty of things to do if they wished. Service users said they had been encouraged to bring their own possessions to the home including furniture and to personalise their rooms to their own tastes. Service users agreed that the food served at the home was of good quality and was always offered with an alternative course and catered for special dietary needs. The Director said the menus were currently being updated and advice had been sought from a dietician. The home had recently been involved in the Focus on Food project and had carried out a survey of service users asking about the quality of food served. The Director said the results of the survey were very satisfactory. JUDGEMENT – we looked at outcomes for the following standard(s): 17 The home promotes and protects the legal rights of service users. EVIDENCE: In discussion, the Director said that service users families were encouraged to conduct their relative’s legal affairs if necessary, including voting in local and general elections by using the postal voting system if required. An advocacy service was available to service users and had previously been used at the home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 24 and 25 The home provides a safe, comfortable environment that is well maintained. EVIDENCE: The home offers spacious and homely lounges for service users to meet with each other and to entertain visits from relatives in private if desired. The gardens are spacious and provide a safe and tranquil area to use in the warmer days. Service users rooms were seen to be clean and tidy, and contained personal possessions and items from their own homes. Service users said they liked their accommodation and that they were nice and clean. Checks on fire systems and bathing equipment were carried out annually by professional services and as part of the homes maintenance process. Safety records were well documented and up to date. Communal areas were well used and pleasant to sit in and enjoy socialising. It was noted that water temperature records showed that at times water temperatures fluctuated and were sometimes quite high. The manager should arrange for temperature regulator valves, where fitted, to be checked and to fit them where they are required. The Director said staff, prior to bathing, rigorously checked water temperatures. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 and 30 The home promotes and protects the safety of service users through training and recruitment and selection procedures. EVIDENCE: Staffing levels at the home remain constant and included six 1st level nurses. Recently the home recruited a nurse from overseas and the Director was able to outline the processes involved in offering employment. The home employed 22 care staff and dedicated kitchen and domestic staff. Examination of staff files found evidence of satisfactory checks being carried out prior to the commencement of employment including Criminal Records Bureau checks and Protection of Vulnerable Adults checks. Staff training was progressing well, with all but two of the care staff having achieved NVQ2 in care, and 6 care staff had progressed to level 3. The Director said two of the care staff had shown an interest in carrying on their studies to NVQ level 4. Specialised training courses had been undertaken including peg feeding, tissue viability and dementia care. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33 and 38 The home is run in the best interests of service users and health and safety issues are addressed. EVIDENCE: The home carried out an annual quality assurance survey in the form of an evaluation questionnaire, the results of which were displayed on the homes notice board. Meetings were also arranged for service users to give their views and opinions on the running of the home. The manager was able to demonstrate that as far as reasonably practicable the health, safety and welfare of service users, staff and visitors to the home were maintained.
