Conditions of registration: 1. PD - 2 beds minimum age 55 years and over. Date of last inspection 9th September 2005 Brief Description of the Service: Wetley Manor Residential Care Home is a care home providing personal care and accommodation for up to 22 residents. The home is dedicated to caring for elderly people who require residential care with a Mental Disorder, excluding learning disability or dementia (2), Old age, not falling within any other category (22), Physical disability (2), Physical disability over 65 years of age (8). The home is privately owned by Wetley Manor Care Homes Limited. The home is located in the rural village of Wetley Rocks, Staffordshire and is accessed from the road via a long driveway. Amenities can be reached by transport but are not within walking distance. The home is laid out on one level and has 20 single and one double bedroom all with en suite facilities. There are five communal rooms including a conservatory and sun lounge area, and a separate dining room. The grounds are spacious, attractive and accessible to residents. Views from the home are of the surrounding countryside and there is a car parking area at the entrance to the home. This is an overview of what the inspector found during the inspection. This unannounced visit was made on 22nd February 2006 at 10.00 a.m. The inspection was carried out by one inspector who used the National Minimum Standards for Older People as the basis for the inspection. The total time spent for the inspection, including pre and fieldwork, amounted to 5 hours. The inspection included a part tour of the building, inspection of records, observation, and discussions with residents and staff. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2 and 4. Potential residents received detailed information about the home and the service it provided to enable an informed choice to be made about the suitability of the home for them. Each resident had a contract with the home and they were assured that the home could meet their assessed needs before moving into the home. EVIDENCE: The inspector examined the home’s Statement of Purpose and Service User Guide and found they contained all of the required elements to provide prospective residents with a clear understanding of the service provided at Wetley Manor. A resident’s contract was also inspected and included details of: the trial period, fees, scope of the fees, those items/services not included in the fees, smoking and fire regulations, visiting and mealtimes, termination conditions etc. Wetley Manor Residential Care Home Limited DS0000005033.V281002.R01.S.doc Version 5.1 Page 8 A standard letter used to confirm to prospective residents that the home could meet their needs following assessment was examined and found to be welcoming, detailed and informative. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 11. Care planning processes within the home provided staff with sufficient information to meet resident’s needs satisfactorily. Health care needs were met very well and resident’s needs at the time of their death were treated with dignity and respect. EVIDENCE: Resident’s care plans were examined and found to provide a range of information about activities of daily living. Each care plan was reviewed monthly and risk assessments were completed and also reviewed monthly. The daily report provided an up-to-date picture of the current status of the individual resident. Access to other health care professionals was recorded and evidenced that resident’s health care needs were met well. The care plans evidenced that resident’s wishes at all stages of their lives were upheld and respected, including end of life needs which in some cases detailed specific funeral arrangements. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14. All other key standards were examined at the last inspection on 9th September 2005 and were met well. Residents were supported to make their own decisions and choices in their everyday lives. EVIDENCE: Discussions with residents and observation throughout the inspection confirmed that they were supported and enabled to make their own choices and decisions about their day-to-day lives and to maintain contact with their relatives and friends. Residents stated that they were very happy in Wetley Manor and were glad that more activities had been put on recently. They were very complimentary about the staff and the food, and wine was served with lunch for those who were able to have it if they wished to. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 and 18. Residents were listened to and their views taken into account. Residents were supported to participate in the electoral process and were protected from abuse by on-going training and supervision sessions for staff on the protection of vulnerable adults and the home’s Adult Protection procedure. EVIDENCE: The Commission had not received any complaints or allegations of abuse since the last inspection and residents confirmed that they were listened to and any issues resolved quickly. The registered manager confirmed that residents were enabled to exercise their legal rights directly and were supported by staff to participate in the electoral process if they so wished. Councillors also attended the home during an election period and talked to residents if they wished to. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21, 22, 23 and 25. Satisfactory toilet and washing facilities were available for the residents and specialist equipment was provided to support independence if required. Rooms were suitable to meet needs and the home provided a safe, comfortable and very clean environment for the residents. EVIDENCE: All bedrooms had en-suite facilities and communal bathrooms and toilets were satisfactory for the residents and very clean. Specialist equipment and adaptations was provided for the residents to promote their independence and included 2 hoists, walking frames, grab rails etc. The layout and location of the home was suitable for the residents and was well maintained and safe both externally and internally. Dimensions and layout of rooms were sufficient to meet the needs of the residents and emergency lighting, smoke alarms and covered radiators throughout the home maintained the safety of the residents. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30. Staff were trained and competent to carry out their roles and responsibilities and residents were in safe hands at all times. EVIDENCE: Discussion with the registered care manager confirmed that the home currently had 17 care staff working in the home with 3 having achieved NVQ Level 2 in care, 1 who had almost completed NVQ Level 3 in care and 9 other members of staff had applied to undertake the NVQ Level 2 in care. Once they had completed this training, the home would exceed the required 50 of trained members of staff with NVQ level 2 or above working in the home. The inspector examined the records of the type and frequency of training undertaken by the staff within the home, which evidenced that mandatory training, had taken place. In addition to this, the registered care manager confirmed that the pharmacist for the home as part of the regular medication reviews at the home would be providing medication training for staff. A training planner for 2006 was in the process of being collated and the registered care manager had made attempts to find an available course for abuse awareness training for staff. It was confirmed that contact would be made with the Vulnerable Adults Co-ordinator to obtain advice on the training available for the protection of vulnerable adults. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 34, 35, 37 and 38. The registered care manager was fit to be in charge, responsible and of good character. Financial procedures were robust to protect resident’s financial interests. Records held were accurate and secure and the health, safety and welfare of residents and staff were upheld and protected. EVIDENCE: The registered care manager for the home had worked at Wetley Manor Residential Care Home for 3 years and had previously worked as assistant manager and deputy manager in other care homes. She was a qualified enrolled nurse and had also achieved the D32-D33 Assessors Award and undertaken the N.E.B.S Management course and E.N.B 941, Care of the Elderly. She was an experienced manager who was very capable in managing her responsibilities and staff to meet the needs of the residents. Records of service user’s personal allowances were examined and found to balance with monies held.

Policy and procedural documentation was examined and covered a range of topics to protect residents. A range of records and documentation was examined and provided evidence that the health, safety and welfare of residents and staff were protected. Fire safety records showed that the fire alarm and fire extinguishers were tested on 25/08/05. Other records included: gas safety check on 14/03/05, and nurse call alarm service on 25/08/05.
