Conditions of registration: 1. Up to 5 service users aged 60-65 years may be accommodated. Date of last inspection 31st August 2005 Brief Description of the Service: Whitelodge is a large care home situated in Southampton and is close to local facilities. The home is registered for twenty-seven service users within the categories of old age, dementia and mental health. The home is owned and managed by Mr Foot. The home provides accommodation in a range of double and single rooms and some rooms are on the ground floor. The home has two lounges and dining rooms a kitchen and suitably located bathrooms and toilets. To the front of the house is a small garden and to the rear is a nicely maintained garden and patio, which is accessible to service users. This is an overview of what the inspector found during the inspection. The second inspection of the inspection year took place over 2 hours and the purpose was to ensure compliance with previous legal requirements brought to the homes attention at the first inspection in August 2005 and to complete the inspection process for the year. The inspector audited 4 standards and reassessed 5 standards, of which the inspector had raised 1 requirement at the last inspection. All key standards have now been assessed throughout the year. The inspection involved a walk around the home and an audit of some of the homes documentation. The inspector was assisted by the homes staff. Much of the inspection was spent talking to the service users who clearly displayed their involvement in the home. Service users spoken to advised the inspector that they enjoy living at the home and that the homes staff are ‘lovely’, ‘kind and caring’ and ‘fun’. Several service users commented on how homely the home is and that they feel happy because the staff respect them and their views. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 All service users have an adequate care plan. EVIDENCE: Previous inspections found this standard to be fully met. One service users care plan was audited by the inspector, which was found to include all relevant information relating to care needs. The home had undertaken an assessment and this had been incorporated in the service users plan. The home had undertaken risk assessments and had involved all relevant health care professionals. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 Service users confirmed that they enjoy living in the home and that the home meets their needs, provides suitable activities and supports their choices in their lifestyles. EVIDENCE: The inspector spent the majority of the inspection talking with service users and participating in the homes activities. At the time of the inspection the home had a visiting musician, who service users stated they enjoyed. Service users advised the inspector of the large range of activities the home provides. Individual choices in hobbies and leisure are supported by the home, which was witnessed by the inspector. Several service users were seen participating in the home as they wished and these service users advised the inspector that the home supports their choices. Several service users advised the inspector that the home asks them what they want and then tries to provide it. Service users advised the inspector of the range of activities that were planned for the Christmas period, which included parties and visiting musicians. The home had organised several events for the Christmas period including visiting musicians for Boxing Day. The home stated that this is usually a quiet day and that they wanted to ensure service users enjoy the season. The inspector was advised that the home supports access to service users families and outside venues of their choice where possible. The inspector had the opportunity to speak to two visitors in the home and both spoke very highly of the care their relatives receive and the attitudes of the homes staff. Visitors also stated that they are invited to join in the activities of the home, which they felt is exceptional. The home has built a new bathroom for service users and refurbished the staff toilet since the last inspection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 35, 36, 38 Service users live in a well managed home that is run for their benefit. Service users are consulted about the running of the home, however, this needs formalising. Adequate financial records safeguard service users money. Some staff are supervised and appraised within their jobs, however, these need developing to ensure all staff receive supervision. Mainly service users and staff are protected by policies and procedures, however, these need further development. EVIDENCE: Service users and visitors to the home all spoke positively about the home and the care they received. Service users confirmed that the providers are

The home occasionally holds service user meetings and on a daily basis service users are spoken with to see if there are any issues. The meetings held had been documented. Service users confirmed that staff act on their wishes and this was evidenced through menus, activities and how care is provided in the home. The home has started to look at ways to formalise how they consult with service users and visitors to the home. Service users spoken to confirmed that their views are listened to and that the home acts on their concerns. Following discussions with the homes staff it appears the home does not seek the views of all stakeholders of the business. The home is required to formalise their monitoring of the quality of the service it provides. The home does not manage any of the service users money and only holds one service user’s money in the homes safe. The home keeps adequate records. The home had started to implement staff supervisions, which was found to be in the early stages. Only some of the staff had received supervision. A further requirement has been made. The home has undertaken some risk assessments and implemented controls for the identified risks, including radiators, hot water, chemicals and moving and handling. Some areas of risk had not been documented and controls implemented, for example the risk of falls from a height (first floor windows), and young staff. The risk assessments have not been recently reviewed and had not been signed. The home has an adequate infection control policy and has implemented procedures as currently the sluice facility is in the laundry room, whilst the sluice room is being refitted. On audit of the homes certificates all were found to be up to date, however, the home does not undertake Portable Appliance Testing. The home is to review its risk assessments and implement risk assessments for any further identifiable risks. The home is required to consult with Environmental Health regarding risk assessments and for guidance on Portable Appliance Testing. STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
