What the care home does well

It was apparent that the home encourages a good rapport to be maintained between the relatives, residents and the staff. The manager and the staff were very aware of the residents` care plans. There was good improvement in the training especially the NVQ training and supervision of staff. The home provides a comfortable, clean, calm and caring environment. The staff are very friendly to each other and worked well in the team.

What has improved since the last inspection?

The requirement that was left to update the medication procedures has been met. The home has maintained good standards of care.

Date of last inspection 27/09/04 Brief Description of the Service: Whitegates is a privately owned nursing home which provides care for 38 residents with old age and situated in Laleham near Staines. It is well appointed to the major main routes by car. The river is within walking distance from the home. The home employs nursing and care staff and have internal catering. The home has ample car parking facilities and also have a garden which is accessible. The bedrooms are arranged over two floors which can be accessed by a lift. The inspection was announced which means that everyone was aware that an inspection was taking place. This is the first of two CSCI inspections to be carried out this year before the end of March 2006. The inspection was undertaken in the daytime and the inspector had opportunity to meet with several residents, a few relatives, receive comments cards (completed beforehand by the residents/ relatives), look at documentation and the premises and discussing with the manager and the staff. A pre-inspection questionnaire was also submitted to the CSCI ahead of the inspection with relevant details of how the home operated. The inspector was made very welcome by all. The owner also telephoned the inspector from his holiday abroad for a quick feedback on the day, which demonstrated that everyone cared very much for this service and were keen to know how it was operating. The home was being run efficiently. Staff were observed dealing with residents in a very encouraging, positive and sensitive manner. The staff knew their residents well and were able to talk about their documentation and care with ease. There was a calm atmosphere throughout the home. Several relatives were around and gave very good feedback on the home and the staff. The comments received on the day included: “The home does very well. The room is slightly small. The staff do all they can for…(relative). We get on with everyone” “I join in activity when I want. I have everything I need right here” “I like all the staff, they do everything I want”. There is open visiting and relatives are included in the activities. One relative was participating in the quiz on the day. Another relative stated that the home had purchased a birdbath for the resident to be able to observe the birds from the bedroom window. Comments from staff expressed that they liked working there, had access to relevant training, felt supported and were able to discuss their working experiences with the manager and the deputy. The inspector wishes to thank all the residents, the staff, the relatives (who helped with providing feedback), the management team for their kind hospitality and assistance on the day. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 There is a new statement of purpose, which complies with the regulations and standards. The assessment procedures were in place and were working well to ensure all applicants are assessed thoroughly and the home is prepared to meet the needs of the applicant before a placement is confirmed. Standard 6 was not relevant to this service. EVIDENCE: The assessment procedures were looked at and the inspector inspected two recently admitted residents’ admission notes and care plans (which are discussed further in the report). The assessments were conducted by either the manager or the deputy manager and covered a range of documentation. The manager stated that the home encouraged the prospective residents, their relatives and care managers to visit and meet staff and fellow residents and also to view the home and ask questions. The relatives of residents in the home during this inspection also confirmed that this was the case when the inspector spoke with them. Sometimes this is not possible due to the distance for travelling or if the prospective resident is too unwell in hospital. The needs are individually assessed and included physical/ nursing needs, the social care and any psychological or emotional needs. The nursing assessments include Whitegates Private Nursing Home h09-h58 s17651 Whitegates v225241 160605 stage 4.doc Version 1.30 Page 9 nutrition, moving and handling, procedures for the use of any equipment or nursing interventions, falls, wandering and pressure sore risk assessments. Any correspondence from the hospital or district nursing staff was present in the file. Some hospitals also offered very thorough written assessments prior to transfers, which is very useful. A new care plan is then generated once the admission is complete. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 There are care plans available for each resident, which clearly sets out their care needs. Residents were looked after well and had access to all the relevant health care professionals. EVIDENCE: The inspector inspected 4 care plans, which were written very clearly. There was a lot of information relating to their actual needs and also the risk assessments for various tasks considered to pose a risk, such as moving and handling or taking medication. There was evidence that the home referred to the relevant health care professionals regularly such as the tissue viability nurses, consultants, doctors, chiropodists, dentists, opticians and dieticians. Daily notes are maintained and both night and day staff were expected to document their observations from their shifts on individual logs. Staff spoken with during this inspection were very aware of the residents’ needs. They were observed looking after the residents with confidence whilst undertaking their tasks with sensitivity. They were not rushing around unnecessarily despite being very busy and they helped each other. This gave more confidence to the residents and encouraged them to participate in their care and also in conversation. This made communications easier when health care needs are discussed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13 The home seeks information about each resident involving their relatives and friends to enable them to participate in activities of their choice and maintain all their past contacts in the community. EVIDENCE: The activities coordinator who is employed by the home sees each resident, shortly after admission. The task involved seeking all the information about previous interests, wishes and important events, dates in the life of each resident. Birthdays are celebrated and in fact on the day of the inspection one resident received a beautifully personalised cake from the chef. This particular resident liked a game of cards and the chef decorated the cake with playing cards. The chef later spoke about her skills at cake decorating, which the residents enjoyed very much. Some residents were able to talk about the different styles of cake decoration done at events such as Easter, Christmas, parties and birthdays. There were several photograph albums available to demonstrate the events, which were shown to the inspector (intended for residents and visitors to see). Daily activities are provided and a programme is available. Each activity participated by residents is clearly logged. There was a quiz being facilitated on the day of the inspection and several residents took part with the visitors. The community support is reported as “tremendous” by the activities organiser. A local grocery provides a bunch of flowers daily to the residents. A few relatives are reported to take residents out regularly to the local pub or the river (which is a short walk away). Some received regular visitors and were able to share a meal with them if they wished to. Relatives and friends are invited to all the parties organised by the home. There have been entertainers also organised to come to the home including the animal farm. The home also owns a taxi car that can take two wheelchairs. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 a 18 The home has a complaints procedure in place, which is used. The home has procedures in place to deal with the protection of residents in their care. EVIDENCE: The home has a procedure to deal with complaints. There was no open complaint at the time of inspection. Each complaint is clearly logged and all information is available in a dedicated file. The CSCI has not been contacted to deal with any complaint relating to this home under this legislation. The manager and the deputy stated that they aimed to resolve issues as soon as they are raised and they felt that they were proactive in their management style. They also knew their residents and their relatives well and made themselves available to talk to everyone regularly. The home has procedures in place to deal with any suspected cases of abuse. The staff had training in the protection of vulnerable adults and are able to use many contacts to whistle blow should they feel the need to. There have not been any cases of abuse investigated under the protection of vulnerable adults procedures under the new legislation. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 a 26 The home has not undergone any building work of late. It still provides a comfortable, decorated and maintained, homely, clean and safe environment. EVIDENCE: The inspector looked around the home inside and outside and found no obvious health and safety issues that could pose a risk to those who live and work there. There were no health and safety issues raised to the inspector’s attention. Residents felt that they were safe and were looked after well. There have been no structural changes to the premises since the last inspection last year. The bedrooms and communal areas are decorated in a homely manner. There were ample areas to relax and eat. Residents were able to choose if they wanted to stay in their rooms to receive visitors and have privacy or sit in the communal areas. The home was clean and tidy throughout during the inspection. Staff wore protective clothing as necessary and as appropriate to the tasks they performed at the time. All the disposal of clinical waste was handled appropriately and in accordance with the home’s clinical waste management procedures. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 30 The home provides a range of training and encouraged staff to undertake relevant training to ensure a competent workforce. EVIDENCE: The home employs a large number of workers including first level nurses, carers, a deputy manager, an activities organiser, catering and cleaning staff. The home facilitates NVQ qualification and so far achieved 9 staff members with at least level 2 NVQ. A further 4 members of staff are about to commence this course. The deputy manager is undertaking level 4 NVQ in Management. Other training courses offered and covered by staff include manual handling, protection of vulnerable adults, heath and safety and food hygiene. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 and 38 The home was operating well and to full capacity at most times indicating that there were no financial issues. There were procedures to ensure that residents and staff were living and working in a safe environment. EVIDENCE: The home employs an accountant and a business manager to take care of the financial issues. The manager is involved in setting the budget. Funds are made available for all necessary expenses to enable the manager and her team to provide a quality service. The comments received from both relatives, residents and staff were encouraging and were very positive. They all felt that this was an efficiently run service which the owner took pride to visit regularly and give practical help to achieve a reasonable standard of care. There are procedures in place to enable staff to deal with emergencies including fire and first aid. The equipment is serviced regularly and the home is well maintained. There were no health and safety issues reported or observed on the day. A recommendation was made for these forms to include a clearer description of the risks involved from the use of cot sides for those who were asked to sign it (residents/ relatives) to make an informed choice.
