Date of last inspection 1 8 0 2 2 0 1 0 Care Homes for Older People Page 2 of 9 Brief description of the care home Willow Lodge is registered to support and accommodate up to 26 older people any one of whom could have a diagnosis of dementia. The home is in the residential area of Southbourne, close to Emsworth. There are 14 single bedrooms and six double rooms, over three floors. The majority of the rooms have en-suite facilities, although most of en-suite baths are not currently in use. There is one communal bathroom with an adapted bath on the second floor as well as a shower room. The ground floor has a walk-in shower room. The home has a large open plan dining room / lounge which opens out onto a large patio area and secure rear garden. Care Homes for Older People Page 3 of 9 What we found:
The inspection consisted of a site visit of 3 hours and 15 minutes. Discussions took place with the manager and the 2 owners of the home. Two staff were interviewed. Staff were observed working with residents. Records, documents and policies and procedures were looked at. Choice of Home The process of assessing someones needs before they are admitted to the home was looked at for 1 person. This showed that the home carried out a comprehensive assessment of needs, which included consideration of assessments provided by the social services care manager. The homes pre admission assessment included mobility needs, and the persons capacity to make decisions. Health and Personal Care A residents care plan was looked at. This included assessment of mobility and instructions for staff to follow in providing the required support. The care plan included assessments and guidance on support for the following needs: general health, mobility, memory, daily routines and preferences, nutrition, medication, interests/activities, spiritual/religious/cultural, end of life care, washing, dressing, continence, hair care, bath/shower, optical/hearing, foot care and oral hygiene. There was a separate care plan for night time. Risk assessments were recorded for the following areas: the likelihood of falls, self harm, harm to others, abuse, nutrition, aggression, fire safety, wandering, ability to self medicate and risk of pressure sore. A corresponding care plan with times of observation needed by staff was recorded. The manager stated that the home has introduced a revised risk assessment format. Care records also include a nutrition screening tool and a Barthel assessment. Daily running records are maintained of daily events and were seen to be of a good standard with dates and signatures of staff recorded for each entry. Since the last inspection the home has installed a controlled drug cupboard. It was noted that the location of this could be improved as it requires staff to use a step ladder. Daily Life and Social Activities Since the last inspection the home has appointed a staff member specifically for providing activities for the residents. At the time of the visit, the activities co ordinator was observed interacting with residents by giving hand massages and conducting a quiz, which residents engaged in. Complaints and Protection Since the last inspection social services have been involved in 2 investigations under the safeguarding procedures. One of these is concluded and action has been taken regarding
Care Homes for Older People Page 4 of 9 the other incident. Records show that the home has worked with social services in providing information. The home has a copy of the local authority multi agency procedures for the safeguarding of vulnerable adults. Environment Four bedrooms were seen during the visit. Each was found to be clean with the exception of the carpet in 1 room. The home owners plan to redecorate this room when it is vacant. As required by the previous report damaged furniture has been replaced. Secure lockable storage is provided in bedrooms. A requirement made in the previous report regarding storage and safety in bedrooms had not been fully complied with. A storage cupboard in an en suite bedroom, which was not secure and contained paint pots and broken tiles had been made partly safe by the removal of a handle. The door had been made secure by the use of a screw, which was protruding and posed a risk of injury to staff or residents. A mattress was propped up on its side in the same bathroom posing a hazard. In another bedroom there was a similar cupboard containing Christmas decorations. Whilst these risks are not substantive, and the home has written to confirm that the matters have been addressed, the situation reflects a lack of understanding of minimising environmental risks in the home. Staffing The staff rota was checked for 5 specific weeks. This was to check whether the home has had sufficient numbers of staff to meet residents needs and to check the hours being worked by staff as a complaint was made to the Commission that 1 staff member had worked 24 hours non stop including a waking night. The home confirmed that 1 staff member did work these hours, but that this was an isolated incident due to staff shortages. The rotas showed that the home had sufficient numbers of staff on duty with between 4 and 5 care staff on duty from 8am to 2pm each day and 3 to 4 care staff from 2pm to 8pm. Additional staff include the manager and the catering staff. The rota showed that some staff were working up to 66 and 60 hours on a regular basis. The Commission consider that this is too many hours and may affect staff performance and the care of residents. Staff working long hours said that they did not feel it was a problem, that they dont get tired, that they choose to work these hours and are able to take frequent breaks throughout the day. Staff said that they feel supported in their work and are encouraged to say if they feel the working hours are too long. The manager stated that 2 new staff are to shortly start work at the home, which will reduce the need for staff to work long hours. Staff confirmed that they are provided with frequent training, including access to National Vocational Qualifications in care, pressure area care, dementia care, medication procedures and moving and handling. Staff also said that they have one to one supervision. Management and Administration The home liaises with the appropriate health and safety agencies such as the local

A recent Environmental Health Officers inspection made recommendations for improving health and safety, which the homes owners said are either completed or are being completed. It was also advised that the homes risk assessments are reviewed and updated. Training records show that 8 care staff have completed first aid training.
