Date of last inspection Brief Description of the Service: St Martin of Tours House is a registered Care Home for men with a forensic psychiatric history between 18 – 60. The home has been open since 1987. Many of the residents have a serious forensic history. All residents are supported in the poject within the multi-agency framework of the Care Programme Approach. This requires extensive and continual liaison with all external professionals involved. The home is divided into two Units: 1) Wilton Villas which has 26 single bedrooms and 2) New North Road which has 18 single bedrooms. Each unit is staffed independently and has a manager with a dedicated staff team. Also, the two units share 3 ancillary workers. The focus of the service is on rehabilitation and partnership working with the forensic services and multi-disciplinary teams. The home is not set up to meet the needs of residents who wish to stay permanently or long terms. Stays are generally up to 2 years. There is a strong emphasis on risk assessment, care planning and structured individual sessions with key workers. Project and support workers run a range of group activities, relevant professionals are available to support therapeutic groups. The home is on 4 floors, including the ground floor where the offices are based, the communal lounge and kitchen which is used for supervised cooking groups and where residents have supervised access. Each floor has a kitchen, toilets and bathroom. Both units are self-catering. The home has a pleasant courtyard garden.

It is within walking distance of Essex Road, and bus routes to Dalston, Highbury & Islington Tube Station and Angel Tube station. Parking is not available. This is an overview of what the inspector found during the inspection. This was the second statutory inspection over the year April 2005 – March 2006. The inspection was unannounced, took place over one day and lasted 6 hours. All of the key standards had been assessed at the previous inspection. The purpose of this inspection was to check on the progress made in relation to the requirements made at the previous inspection. The rest of the time was spent speaking with the deputy manager, manager of each unit, interviewing a staff member and speaking with the one resident who agreed to speak with the inspector. The inspector would like to thank residents, staff and management for their hospitality and co-operation. What the service does well: What has improved since the last inspection?
The home has introduced an audit system – a check list - upon referral of a resident to ensure that comprehensive assessments from the referral authority

That the home’s own assessment is properly recorded, that records are up to date so that appropriate and relevant information is available. So that residents are appropriately placed and staff are have the necessary information to undertake their tasks to the benefit and safety of residents. The manager(s) produced notes of a review of the ways in which the referrals process could be enhanced. The environment has improved with curtains in communal areas, framed pictures on walls and large pots with foliage, which serves to soften, to some extent, the institutional feel of the building and create a slightly more homely environment for residents. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 & 2. The New North Road Unit does not meet all the requirements regarding their Statement of Purpose. (This refers to New North Road Unit). Wilton Villas Unit responded well to a requirement made under standard 2. EVIDENCE: A requirement in terms of a comprehensive assessment and risk assessment, under standards 2, 9 and 41 was acted upon and the assessment and care plan was appropriately completed. Service users may feel confident and assured that staff and the resident have the appropriate information to enable them to be safe. The registered manager should ensure that the service user whose first language is not English is made aware of the terms and conditions and their care plan. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9. The manager responds well to a requirements made at the last inspection. EVIDENCE: A requirement was made under standards 2, 9 and 41. The requirement was made in terms of Care plans at Wilton Villas to be appropriately completed with detail and relevant information. This was done. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24. The internal appearance has improved since the last inspection. EVIDENCE: A requirement had been made at the last inspection to add homely touches to relieve the institutional appearance of the home especially so on the New North End Unit. The communal areas had curtains to match the decoration. There were large pots with realistic looking foliage and various types of pictures hanging on most of the communal areas and stairway walls. The colours for decoration on the walls are different on each floor, some vivid and some calming and may, or may not be to everyone’s taste. The appearance of the home was much improved. The manager(s) produced the Provider’s draft proposal for refurbishment works to bring the building up to a higher standard both in its general fabric and appearance. Residents may look forward to experiencing a pleasant and homely environment.

Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31. The home’s new managers are making sure that staff receive regular supervison and are aware of their roles and responsibilites, according to their job description. EVIDENCE: The inspector interviewed the new deputy manager of one month who confirmed that they had been fully informed of their roles and responsibilities and had been on a continous month’s induction. Supervision had been planned for that day when the unannounced inspection occurred. One resident said they felt the staff were well trained and doing a good job, but was disappointed that staff remained in their office and only met with them on a formal basis. They said they would appreciate it if staff came in to the lounge or any communal area like the kitchen to chat on an informal basis with them. In discussion with the manager, the manager accepted that this was the practice in the past, but that he was encouraging staff to be more proactive in working with residents. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 41 & 42. Generally, the home responds well to requirements made. EVIDENCE: A requirement made at Wilton Villas in terms of record keeping under standards 2, 9 and 41 was acted upon and the record has been appropriately updated. Service users may feel confident and assured that staff and the resident have the appropriate information to enable them to be safe. The manager(s) produced notes of meetings where the enhancement of assessments were being reviewed to ensure the safety and well being of residents. The managers at each unit has put in their application and undertaken their fit person interview with the Commission’s Central Registration Team. They are awaiting their registration certificate. The Regulation 26 monthly visit reports was examined. They were readily to hand and it was evident that the manager goes through the report and acts

A requirement made at the last inspection about hazardaous substances being stored in a locked cupboard was not met.
