42 Hillgrove Crescent is a detached bungalow in a quiet residential street providing a home for five people who have learning disabilities, all of whom also have a physical disability and one of whom is over 65 years of age. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection. What has this told us about the service? The registered manager of 42 Hillgrove Crescent sent us their annual quality assurance assessment (AQAA). It gave us some information together with real practice examples about the services offered. It included details of the improvements they have made in the last twelve months and their ongoing plans for the future. The registered manager has highlighted in the AQAA barriers faced by this service when trying to recognise and meet peoples individual needs and preferences in areas, such as, religion, gender and age to ensure that peoples lives are meaningful to them. The AQAA confirms, Diversity training is not easily available, which makes it difficult for staff to remain up to date. However, the registered manager has shown us in the AQAA that people living in this home are being encouraged and supported to have their say in respect of their support plans with regular meetings to discuss these. Family members, advocates and other professionals are able to express their views to ensure individuals preferences and needs are met in their best interests. We are told that people living in the home take part when interviewing new members of staff. Also people have an identified member of staff who becomes their key worker so that any worries and or concerns people may have can be shared with a staff member who has built up a close relationship with them. The key worker can also make sure individuals are receiving the support and encouragement required to achieve their goals and aspirations. This gives people who live in the home more control over their own lives and empowers people to feel comfortable in stating their likes and dislikes. At the last key inspection the registered manager was asked to focus upon the health care records of people who live in the home. For example, we shared with the registered manager that records should be developed to show when a health concern is raised such as, if a person has sore skin the records need to reflect staff practices to ensure the persons skin heals. These processes should be in place so that it can be seen that peoples health needs are being met and by whom together with the management of identified health conditions. We can see from the information supplied in the AQAA that all individuals have a health action plan where peoples health needs Annual Service Review Page 4 of 7 are noted, monitored and reviewed. The registered manager also states the following to show what practices are in place to reflect what staff do well in respect of meeting the health needs of people who live in this home, Guidance from professionals is incorporated into individual support plans Support plans document individual health needs and how these are to be met Staff support individuals to regularly attend clinics and appointments for health needs Health action plans are in place for individuals. The registered manager has shown in the AQAA that there is a continuing programme which makes sure that people living in this home have a comfortable homely environment that meets individuals needs. For example, the AQAA tells us that there are plans to turn the garden into a bright sensory area so that people are encouraged, stimulated and can feel a sense of relaxation within the garden of their home. Inside the home we are informed that peoples bedrooms reflect their indivdual tastes and preferences. One of the ways this is acheived is by staff supporting people to purchase their own goods. We are also told by the registered manager that there are plans to build a new bathroom and turn the existing bathroom into a shower room. This shows that the home is being well maintained together with the environment inside and outside being changed where appropriate to meet individuals physical needs. This should make sure that all facilities at 42 Hillgrove Crescent give people not only choices but are easy to use whatever the persons abilities are. The AQAA states that the home are striving to improve the knowledge of staff who work at the home. This is reflected in the fact that all eight staff members have acheived Level Two in the National Vocational Qualification, (NVQ), in care and progressing towards gaining NVQ Level Three. The AQAA also informs us that the registered manager is making sure that people who live in this home are supported and assisted by a consistent and familiar staffing team. The registered manager highlights what staff do well and confirms, Positive person centred approach and culture. This means that staff should be able to meet peoples individual needs aswell as the shared needs of all people who live in this home. This goes some way to recognising the differences and similarities when giving support and assistance to people living at 42 Hillgrove Crescent. These improvements should ensure that staff have and carry on gaining the necessary skills and knowledge to provide a good standard of care and protect the people who use the service. The home continues to let us know about the things that have happened since our last key inspection and they have shown that they have managed the issues well. Where necessary we have received notifications of incidents that have happened in the home in the last year together with the appropriate action taken. We have received no complaints and the AQAA also confirms that the registered manager has also not had any complaints during the last twelve months. We looked at all the information we had concerning 42 Hillgrove Crescent, and our judgement is that they are still providing a good service and that they have a clear vision of what further improvements they wish to make. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
