West House provides the services and care at 47 Sandy Lonning for six people who have a learning disability, some of whom may have a physical disability. The home is located in a quiet lane half a mile from the West Coast Town of Maryport. Community facilities and amenities are within easy reach. The premises are purpose built and have been progressively extended and adapted. Recent extension work has provided additional living space for service users. This adds to the environmental standards that were already met by the home. Very comfortable and pleasant accommodation is provided with a high standard of furnishings and decorations. All but two of the private bedrooms are on the ground floor and all have en-suite toilet and bathing facilities. The lounge, dining room, kitchen and utility room are also on the ground floor. Additional toilets are available and separate bathroom and shower room have specialist facilities adapted to assist people with a physical disability. The office is situated on the first floor. The garden area is well maintained and there are car-parking facilities to the front of the home. What the care home does well Sandy Lonning provides a high standard of personal, social and health care to people who may have complex needs. Care is delivered in a sensitive and person centred way and families are invited to become involved if they wish. People report high levels of satisfaction with this service. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: Prior to this inspection we, the Care Quality Commission (CQC) asked the manager to complete an Annual Quality Assurance Audit (AQAA). It is a legal requirement that this is filled in by every registered service. It contains details of what has improved in the past 12 months and what the service hopes to achieve during the coming year. The form was comprehensively completed and returned to CQC within the allowed timescale. We also sent surveys to residents, staff, relatives and health care professionals that may visit the home. We had a very positive response and some of the comments are included in this report. Based on the information contained in the AQAA, the feedback received in our surveys and observations during a visit to the home we judged that Sandy Lonning was continuing to provide a safe service for those living there. We, therefore, decided to carry out a short random inspection rather than a full key inspection at this time. Margaret Drury made an unannounced visit to the home on the 6th May during which we were able to speak to residents, staff, the home manager and the services manager who was visiting the home at the time. We looked at a selection of care records and discussed the management and administrations of medicines. The focus of this inspection was personal and health care support and complaints and protection. When we arrived the staff were just finishing a meeting with the manager and services manager and some of the residents were getting ready to go out. We undertook a short tour of the building with the manager looking at the environmental standards within the home. We looked at the care records for 2 of the people living in Sandy Lonning and found them to be relevant, informative and up to date. The care plans are a working document that give all the necessary information to the care staff in order that they can provide the very high level of care required to meet the needs and expectations of the residents. Each care plan is tailored to the individual and supports the provision of care and lifestyle of choice. People receive care and support in a way and at a time that is acceptable to them. Personal wishes and choices are documented and all residents are involved with the preparation and review of their individual plan. If communication or involvement is difficult an advocate is called on to act for the resident. Relatives are invited to be part of the review process but the manager told us that, in the main, they are happy to leave this to her and the care staff. We had a positive response to surveys sent to residents and they told us that; Sandy Lonning looks after me well. There is good friendly and helpful staff. The home is clean. This home makes you feel happy and I go out shopping with the staff. I like it the way it is. Staff take a lot of interest in my choices and support me with activitied. I enjoy the Care Homes for Adults (18-65 years) Page 4 of 9 food. Staff comments included; There is an excellent staff team but we could do with more staff. The home provides good care for everyone who lives there. If there were more staff they could take the residents out for more social events. We take on board everyones individual needs and choices which is really important. Relatives surveys told us that The general care is very good and they treat each client as an individual. We discussed with the manager the comments raised by staff about the staffing levels within the home. She confirmed that the earlier meeting had been about this subject and that measures were being put in place with regards to the deployment of staff hours to allow more time for activities and outings. Risk assessments are well documented and cover times when residents are inside the building and out in the community. These, like the care plans are reviewed and updated regularly especially if there has been any change in residents circumstances. Nutritional assessments are completed and if special diets are required the advice of the district nurses and dietician is requested. Weights are also monitored. Each resident has an health action plan that gives detail of all health care needs and how these are met. Access to chiropody, dental and optical treatment are available to all and details of visits and appointments are well recorded. Every effort is made to organise different outings and activities for the residents. This is discussed with them and they make their own choice about the activities they enjoy most. There are those who prefer to spend time in the home and choose to go out infrequently. The home has a complaints procedure and log in place. There have been no complaints to record and we have not received any from other external sources. The manager told us that all those living in Sandy Lonning are very vocal and make their wishes known. We did observe interaction between staff and residents during our visit and we found this to be warm, friendly and supportive. Medicines are well managed although the amount prescribed is quite high as residents do have complex needs. The medication is securely kept in a locked cabinet and all the staff have completed relevant and certified training. We checked the records and found these to be accurate and correct. This means that residents receive their medication in the prescribed way and at the right time. There have been no safeguarding matters to report and the staff are well aware of their roles and responsibilities in this matter. There are policies and procedures in place and there is a copy of Cumbrias policy available for the staff to read. We have not received any referrals from social services or any other external sources. Staff attend training in safeguarding procedures and this subject is also covered in the induction programme.
