What the care home does well The staff have embarked on external accredited medication handling training. When asked a member of the staff who regularly gives out medication, she was able to describe the scenario when a person who is prescribed a "when required" medicines would be given this particular drug and what documentation would be made. What the care home could do better: The staff need to see and sign to say will follow, a home`s medication policy that covers the most up to date practises. Be more consistent in maintaining accurate records relating to why "when required" medicines are given ,when medicines in general are given and any returns. Maintain records of any medication audits and competency checks that are carried out by the management. Date of last inspection Brief description of the care home Anita Janes Lodge is a care home providing care for sixteen adults with mental health difficulties, in addition it provides a respite service (short stay) for up to one service user and offers a day care service. There are eight single and four shared bedrooms all without en-suite facilities, located on the ground and first floor. There are bathing/showering and toilet facilities on both the ground and first floor. Communal areas consist of a lounge and dining room/recreational room and kitchen on the ground floor. A copy of the latest published report from the Commission for Social Care Inspection can be found in the entrance lobby. Fees at the home are currently £309 per week. Care Homes for Adults (18-65 years) Page 3 of 12 What we found: This Pharmacist Inspection was carried out to confirm compliance to the requirements issued from the last Pharmacist Inspection on the 18th May 2010. Controlled drugs were handled and stored safely. The manager explained that one out of the two people on duty on the day of this inspection, who give out medicines is now on an external medication handling course. She said that all staff would have had in house competency assessments and that she would also discuss this in their supervision. This would enable her to identify training needs. No notes were available to confirm these discussions. The medication policy had been changed since the last visit but had not been adapted for this particular home as it had another homes name quoted within the sheets. In addition the policy had not been shared with the staff that give out medicines. This could make their systems inconsistent and unsafe. The manager stated that she has just had sight of this policy ( within days from the May inspection) but discusses medication practises with staff on a daily basis. Some of the current medication sheets had been received in to home with the wrong start date. This had shown to have caused confusion to staff and also meant records were not accurate. The manager had noted this and was putting measures in place to avoid any drug errors. On the day of the inspection the manager requested new record sheets to show the correct dates. Records are maintained for medication that are returned to the Pharmacy for destruction. There was one anomaly noted from the most recent records which indication a tablet was missing. The manager could not conclude what had occurred. It was also noted that when required medicines were bing returned monthly and then re-ordered. It was explained that this was wasteful and unnecessary. It was clear through taking to staff that medicines that are given as a when required basis were given appropriately but insufficient documentation was being made. There was no explanation given in the daily notes. A member of staff had a very vague idea how they can assist when a person living at the home is suffering with a minor ailment. A medication round was not observed but a verbal explanation given confirmed that a safe process would be followed by this member of the staff. This requirement has been re-issued as a reult of this inspection.
