What the care home does well The home is safe, resident`s rooms and communal areas are clean. The premises provide people with a homely and comfortable environment to live in. Residents have personalised their rooms, which gives them a sense of ownership. People living at the home and their relatives say that visiting is flexible and that visitors are made to feel welcome. Evidence and feedback from staff and residents concluded the standard of care in the home is good under the direction and leadership of the manager/provider. Residents, staff and relatives are encouraged to be involved in the running of the home. Staff are supported in their roles. Records and administrative systems are well maintained. All residents spoken with praised the manager and staff. Overall feedback was very complimentary about the service. What has improved since the last inspection? Since the last inspection the provider has taken on the role as manager and is now registered with the Commission. Feedback from staff and residents demonstrate that this is working well and the manager is competent as well as supportive. The environment at Arun Lodge continues to undergo major improvements. Some bedrooms and communal areas have been refurbished as part of an ongoing improvement plan for the home. The heating system for the home has now been installed. Staff have completed Dementia care training, which they reported gave them a better understanding of the needs and appropriate responses to some residents. Although the home does not take individuals with dementia, some residents demonstrate short-term memory loss and some confusion. The training has given staff confidence to understand these behaviours better. Residents living at the home are now aware of the terms and conditions of living at the home and given a copy of their contract on admission. Residents and their relatives are encouraged to contribute to their care plans to help build a picture of their lives and interests prior to moving to the home. What the care home could do better: The supervision records for the home are out of date and staff have not received supervision on a regular basis. A requirement has been made in respect of this. The statement of purpose is currently being updated to include recent changes to the home. The Quality Assurance questionnaires for relatives and residents are in the process of being sent out. This information, once collated, will contribute to gain an overall quality assurance action plan for the home for 2008/09. This means the people who use this service experience good quality outcomes. The unannounced inspection visit was carried out by Ms Beth Tye and was arranged to assist the Commission in assessing the home’s compliance with the key standards of the national minimum standards for care homes for older people. Planning for the visit took into account information received on the service since it opened. The Annual Quality Assurance Assessment was returned to The Commission for Social Care Inspection (CSCI) and informed us areas of improvement, which have been carried out and also identified areas for further improvement. Survey forms received from people living in the home, relatives and members of staff also contributed to our planning. On the day of the visit the inspector spoke at length with the manager/provider who was at the home on the day. Residents living at the home and staff were spoken with to gain their views of the service, overall comments were positive and all residents spoken to said they enjoyed at the home and the standard of care was ‘extremely good’. Three sets of admission assessments and the individual plans of care for people living in the home were looked at. A case tracking exercise for these residents was undertaken to examine how their assessed needs were being met. Other records sampled included recruitment and training records for three members of staff, supervision and annual appraisals and records relating to health and safety issues in the home. The premises were viewed including communal areas, kitchen, bathrooms and bedrooms. A number of interactions between people living in the home and staff, arrangements for lunch and medication storage were observed. Since the last inspection the provider has taken on the role as manager and is now registered with the Commission. Prospective residents needs are identified and planned for, before they move into the home and then reviewed on a monthly basis. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Arun Lodge has a Statement of Purpose and Service User Guide, which describe the facilities and services available at the home. The most recent version is now being updated to include recent changes to the home. Residents spoken to confirmed they are given up to date information about the home prior to admission, including a Service Users guide and complaints procedure. This information helps them (and their families) to make an informed decision about moving to the home and what to expect. Arun Lodge Rest Home DS0000014367.V367046.R01.S.doc Version 5.2 Page 10 The Annual Quality Assurance Assessment (AQAA) returned to CSCI stated that prospective residents are fully assessed prior to admission by the manager. This ensures the home will be able to look after them. All residents have pre admission assessments and updated care plans. Care records showed that information gathered through the initial assessment informed the care plans. These identified relevant areas of need including, health and social needs. The manager reviews and updates this information on a monthly basis. Risk assessments are in place for each of the residents and these contained information relating to their specific needs and assessed areas of risk. This promotes independence for residents in all aspects of daily living. The home now has new Terms and Conditions in place, which is signed by the resident/relative. A copy of this is given to the resident following their move to the home. This contract ensures they are aware of their rights and expectations of the home. All records are kept in a locked cabinet only accessible by care staff to ensure confidentiality. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good Care records set out residents needs in full and are reviewed regularly. Health needs are met with the support of health professionals. Medication is managed safely by the home. Residents’ privacy and dignity is respected. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Three care plans were examined as part of the case tracking process. Each care plan contains relevant details relating to the residents health needs and social well-being. The care plans seen detailed all aspects of health, personal and social care and the actions staff needed to take to meet these needs. Information seen on care files was up to date and easily accessible. There was evidence to demonstrate the manager undertakes monthly reviews and up dates the care plans as changes occur. Staff were asked about their care of residents and all those spoken to were knowledgeable about the residents and their specialist care needs. All action taken is recorded on care files and daily records. This gives staff a better understanding of need and responses in addition to supporting residents to maintain independence safely where possible. Records showed that residents have access to other community based health professionals as required. Health professional visits are recorded. Outcomes and action required by staff is recorded on individual care plans. This provides staff with up to date knowledge about appropriate care practice. Staff complete a full induction and mandatory training programme. This provides staff with the skills and knowledge base to respond appropriately to resident’s specialist health care needs. Case tracking, feedback, observation and discussion with the residents and the staff confirmed good practice is maintained in the home and residents are treated with dignity and respect. Residents commented ‘the staff are wonderful, they can’t do enough for us’ and ‘the staff here are really good. They know everybody’s little habits and we are treated like part of a family’ Medication dispensed at lunchtime was observed. Policies and procedures are in place to ensure safe medication administration. Medication administration charts are completed correctly, which indicated that residents are receiving their medication as prescribed. Photographs of residents are with their MAR charts as an extra safeguard towards safe administration. Records seen were in good order and up to date, demonstrating that staff follow appropriate procedures. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good The home has flexible routines which allows those who are able, to exercise choice and some control over their lives. Activities are offered and residents who are able are encouraged to be part of the community. Residents are encouraged to maintain contact with their family and friends. Home cooked food is provided to a good standard with choices of alternatives available on a daily basis. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Activities are organised at the home on a regular, offering stimulation to those residents who are less able to explore interests outside the home. These include art and crafts, pampering sessions, music, quizzes, exercise and entertainment. Care planning includes assessment of each resident within the home to build a picture of their preferences for activities and interests. Relatives are encouraged to contribute to this process. Flexibility enables individuals to have choice and express a preference in their daily routines. Staff escort residents to community events and appointments as required. Residents and relatives confirmed that the visiting arrangements for the home are open and visitors can come and go as they please and are made welcome by the staff. Residents meetings are no longer held at the home as these proved unpopular with the residents. However the manager has an open door policy and encourages residents and relatives to contribute their views on the home outside the more formal process of Quality Assurance questionnaires. The manager also meets the residents individually on a weekly basis to address any issues and gain feedback. This gives the residents the opportunity to comment on how they view the home and contribute to decision-making. The menu offered at Arun Lodge offers a wide range of balanced, home cooked food. The cook is experienced and qualified to fulfil his role. The menu offered takes in to account the preferences of residents and specialist dietary needs. This promotes choice for the residents and provides an opportunity for them to eat what they prefer. An alternative meal is on offer at lunchtime and teatimes. Residents spoken with said they enjoyed the food and there was plenty to eat. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good Residents who use this service or their representatives are able to express concerns, and have an access to a robust, effective complaint procedure, are protected from abuse and have their rights protected This judgement has been made using available evidence including a visit to this service. EVIDENCE: Residents spoken with during this visit were aware of the homes complaints procedure and told the inspector that they always raise any concerns which they have directly with the manager or the owner who is on the premises most days. The complaints procedure is on display in the home and a copy is included in the Statement of Purpose and Service User Guide. Both resident surveys returned to the inspector after the visit reported that the resident knew how to make a compliant. There have been no complaints made to the manager or provider since the last visit to the home by the inspector and no complaints made to CSCI. Staff induction and training records indicated that all staff have received training in safeguarding vulnerable adults and staff spoken to, did know what to do if they suspected abuse. The home has a robust recruitment procedure, which ensures all staff undergo appropriate checks prior to employment. Three staff files were examined and all held the correct checks and information. This promotes the welfare and safety of residents in the home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good Continued improvements to the home have provided residents with a homely and safe environment. Bedrooms are comfortable and meet the needs of the residents. Residents are encouraged to contribute to making their bedrooms their own, by having their personal possessions around them. The home is clean and residents take pride in their home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Some areas of the home are in need of refurbishment and updating. These include some carpets and décor. However the manager/provider is fully aware of this and an improvement plan to maintain and improve such areas of the home is underway. Since the last inspection some major refurbishments and improvements to the home have been completed. These touches make bedrooms look homely and give the residents a sense of ownership. There are aids and adaptations to meet the needs of the residents presently accommodated. The communal areas are clean and furnished on a homely fashion with plenty of suitable chairs for residents. There is a pleasant, decked area in the garden with ample seating for those who like to go outside. Policies and procedures are in place for infection control, and all of the staff are due to attend refresher training for infection control and health and safety procedures in 2008. This promotes good practice in the area of safety and welfare for the residents and reduces the risk of infection spreading within the home. There is a passenger lift for residents with limited mobility to access all floors of the house. A new heating system has been installed in the home. All radiators throughout the home have been covered. The fire procedures, environmental risk assessments and safety checks were recorded and detailed. A new call bell system is provided in every room so staff can attend an emergency situation swiftly, should it arise. The maintenance log showed all maintenance is completed as required on a regular basis. This means the resident’s environment is kept safe and well maintained at all times. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good Staffing levels at the home are adequate to meet the needs of the residents. Recruitment procedures safeguard and protect residents at the home. Staff have taken part in training and Vocational Qualifications to maintain their skills and ensure that residents are safe. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Duty rotas and information provided on the Annual Quality assurance Assessment confirmed there are enough staff on each shift to meet the resident’s needs. A full time vacancy has recently been filled and the manager is awaiting CRB clearance before a start date can be issued. During the visit it was observed that staff were readily available to attend to residents needs when needed. Feedback from residents confirmed this. Staff records and information provided by the manager of the home prior to the visit about the qualifications of staff show that over 50 of staff have an NVQ qualification. The records of three staff were examined during the visit. Information seen confirmed recruitment policies of the home are being followed and those Arun Lodge Rest Home DS0000014367.V367046.R01.S.doc Version 5.2 Page 20 employed had Criminal Record Bureau Clearance and Protection of Vulnerable Adult Clearance. Records show that staff complete a detailed induction pack at the start of their employment, then go on to undertake a mandatory training programme appropriate to the needs of residents. This includes training in moving and handling, first aid, food hygiene, fire safety, health and safety, infection control and safeguarding vulnerable adults. The manager confirmed that infection control and health and safety training are due to be updated this year. All staff have completed training in dementia awareness and the manager is exploring local courses in respect of equality and diversity and palliative care. This will enhance the existing skills and knowledge base of the staff team. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good Residents live in a home, which is well run and resident’s benefit from a caring and well-trained team of staff. Resident’s financial interests are safeguarded. The home operates a quality assurance system. The health and welfare of residents is promoted and protected. Staff have undergone an annual appraisal but supervision of staff does not occur on a regular basis. A requirement has been made in respect of this. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Residents spoken with feel that the provider/manager acts in the best interests of residents living at the home. Arun Lodge Rest Home DS0000014367.V367046.R01.S.doc Version 5.2 Page 22 Since her registration as manager of Arun Lodge, last year. Mrs Rawlings has acquired an NVQ Level 4 qualification and she is planning to undertake the RMA award in Sept 08. The manager demonstrates experience to run the home and provide good leadership for the staff team. Staff stated they felt ‘extremely supported’ by the manager. All staff spoken to said they felt able to approach the manager at any time and she was ‘always on hand to deal with problems’ A management on call system is in place for out of hours emergencies. Staff meetings are regularly held to enable staff to discuss practice issues and contribute to the way the home is run. Records demonstrated that annual staff appraisals for 2008 have been completed. However regular supervision sessions (at two monthly intervals) have not been kept up to date. The manager acknowledged that this is an area she intends to address at the earliest opportunity. A requirement has been made in respect of this. A Quality Assurance policy is in place. The introduction of quizzes is an innovative way of getting residents/relatives to give feedback about different subject areas within the home. The quality assurance for 2008 is underway as at the time of the visit, the manager was in the process of posting out questionnaires to involved parties. Arun Lodge holds the Investors in People Award. A few residents have monies kept by the home on their behalf. The records of records of incoming and outgoing expenditures are recorded appropriately. Financial arrangements are recorded as part of the care planning process. The inspector examined health and safety records. Records required to be kept by legislation are in good order and up to date. All care records are kept in a locked office to maintain confidentiality. Well-maintained administrative systems and regular management audits at the home promote all aspects of health and safety. Residents were observed expressing their views confidently and staff responded with consideration and respect, reflecting a positive ethos and high standard of care, within the home.
