What the care home does well The care plans were informative and advised staff what the strengths and abilities of the individual are as well as what they need to do support individual needs. Where individuals wished to be self caring or carry out an activity, we saw that there were risk assessments to lessen any identified problems. We saw that medication administration, storage and management was carried out safely. What the care home could do better: There are no requirements for action following this visit. The challenge is for the home to embed changes and develop the service. Care Homes for Older People Page 3 of 8 What we found: he purpose of the inspection was to assess how well the home is doing in meeting the key National Minimum Standards and Regulations. The visit was carried out on the 1st June 2010 between the hours of 10:30 and 1:00 pm. Prior to the visit to the home we reviewed, previous inspection reports and information received from the home since it was last visited in September 2009 and January 2010. We met with the registered manager, deputy manager and spoke with the new administrator. We looked at four care plans and medication records. We had made several requirements regarding care planning and medication following the visit in September 2009. The purpose of the visit was to see what action the home had carried out in these areas and whether they had embedded these actions. It was seen that the requirements have been met. We looked at four care plans and saw that there was information on family and social support. There was information on how staff should support the individual. For example frail diet has been introduced to raise calorie intake has small appetite and prone to weight loss. There was information on care when an individual was being looked after in bed to help with pressure relief. Staff were seen to sign when they had carried out support and what they had done. We saw assessments for risk, moving and handling, nutrition, and tools used to monitor individuals pressure areas and support needs such as Barthel and Waterlow. These documents had been reviewed monthly and as needed. There was information on what was important to the individual with the addition of likes and dislikes these included environment and activities as well as food. There were detailed information on how staff were to support the individual with their physical difficulties and what action they should take medically. We saw that staff have been trained in techniques specific for the individual. We saw plans of care where an individual required pain relief this included where they needed pain relief before they had a dressing on a wound changed. We looked at the medication storage and administration records. We saw the medication records for the month up to and including the day of the visit . There are two medication trolleys; one on the nursing floor and one downstairs, both were attached to the wall. We saw that the home has Butrans, Buprenorphine and Ora Morph stored as a controlled medication and these were kept locked in cupboard near a small office. We saw a list of staff names who are deemed competent to administer medication. This includes nursing staff and senior care staff in the residential area of the home. We saw copies of Guild Care policies and procedures for medication in both medication record files. This included a policy on out of stock medication and an as required medication protocol. We saw that the community pharmacist had visited the home in December 2009, one recommendation they had made was for the home to contact them regarding allergies people living at the home had. There was a list of homely remedies and instructions. We had seen in the care Care Homes for Older People Page 4 of 8 plans that there was information on as required medication that the individual could have and the side effects. We saw that this was repeated in the medication records. In the residential part of the home we saw that where nursing support is needed for example insulin administration, people receive support form the district nursing team. There were assessments in place for individuals who wished to administer their own medication. We saw in one case that staff had not recorded the reasons, cause and effect when they administered an as required medication. This was the only example of non recording. The commission had received two notifications regarding concerns about the home since the last visit. We discussed this with the manager at this visit and confirmed that socials services had completed their investigation and the issues were unsubstantiated.
