The home is registered to accommodate up to 56 older people of both sexes whose primary care need relates to dementia and mental health. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included, the Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys were returned to us by people using the service, staff and from other people with an interest in the service. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. And the previous key inspection. What has this told us about the service? The home sent us their AQAA when we asked for it. It was clear and informative. The AQAA states that the service provides specialist care for older people who have mental health needs, especially dementia. Potential residents are assessed by a Registered Nurse prior to admission to see if the service can meet their needs. Care plans are person centered to be individual for each resident. They are up-dated monthly or sooner if necessary. Residents views are actively sought allowing them to feel involved in their care. Incorporated in the risk assessments are prevention of falls, tissue viability, nutritional needs (MUST Assessment Tool), eye care and personal preferences such as bath or shower, how often etc. Activities are varied and devised to help residents retain their skills and memories for as long as possible, and to provide fun and entertainment. The service has strong links with local community, schools, colleges and the Alzheimers Society. The home is clean and has benefited from a total refurbishment, which has included upgrading lighting to provide a dementia focused environment. The garden is contained which provides a safe outside space. Skill mix and ratio of staff meets the residents needs. Robust recruitment procedures safeguard residents. Staff receive good induction and ongoing training. The service has received three complaints, all were responded to within twenty eight days and all were upheld. No safeguarding referrals or safeguarding investigations have been made. Annual Service Review Page 4 of 7 There are quarterly relatives meetings and newsletters plus an independent satisfaction survey. Relatives are invited to take part in care reviews and can meet with staff as they wish. The service regular audits of care plans, medication and financial procedures. And monthly health and safety meetings promote safe working practices. The service has identified in their AQAA the following areas for improvement, for more residents to access NHS physiotherapy services. To increase the number of staff supervision sessions. (We highlighted this as an area for improvement at the last key inspection in October 2008). Make contact with relatives within seven days of the admission to check they have all the information they require. The last key inspection stated people using this service experience good quality outcomes in all areas accept environment. No requirements or recommendations were made. The AQAA confirms that the environment both within and outside the home has been improved. One service user was helped by a relative to respond to our survey, stated they did not receive enough information to help them decide if this service was the right place for them. They agreed they have been given a contract. Always receives the personal and healthcare support they need and staff are always available when they need them. The service takes great care of residents, always friendly and willing to talk to residents at any time. They are aware of who to inform if they are unhappy and staff usually listen to and act on what is being said. Agreed the home is always clean and fresh and likes the meals. Out of the five staff who responded to our survey, two strongly agreed they are given up to date information about service users that enable them to provide appropriate care and support. Two stated usually and one was left blank. All agreed the service had carried out Criminal Record Bureau (CRB) and relevant pre employment checks before employment started. Staff are confident they receive relevant training on a regular basis and meet regularly with their manager for support and to discuss how they are working. Comments received included We are kept up to date with knowledge relating to working. The manager is available if I have any concerns, I feel part of a team and have done my NVQ which has progressed my career and training is provided. Good person centred care relating to residents. Residents are treated well as individuals. Activities are well organised and varied. Special diets are catered for. All staff agreed to knowing what to do if someone has concerns about the home. Opinions varied as to whether staff thought there were enough on duty to meet the individual needs of all people at Bereweeke. Three stated usually and one sometimes Comments received included The service is good at covering shifts when illness means people cant come to work. Overall staff felt the home is clean but one member of staff stated it could be tidier. One member of staff stated that staff need training in how to present food in an appetising manner and to be aware of portion control. One healthcare professional responded to our survey and was generally satisfied with the service provided by Bereweeke. Comments included The service knows its users well and interact well with Annual Service Review Page 5 of 7 them. As all are RGNs they should be able to give injections and vaccinations as prescribed and insert and maintain urinary catheters. The Registered Manager responded to the draft ASR and informed us that all RGNs administer injections and are competent to insert and maintain urinary catheters. BUPA has a contract with the GP surgeries to deliver the vaccinations within Bereweeke. The service continue to notify us of things that have happened since our last key inspection. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 27/10/10. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
