Breage House is one of seven care homes owned and managed by Swallowcourt Ltd. Breage House is registered with Commission For Social Care Inspection (CSCI) for 16 people with a learning disability whose ages range from 18 to 65 years of age. The main bus route from Penzance to Helston/Camborne runs through the village, which makes getting to the home easy for parents and relatives. The home has two styles of architecture an older main house, which is linked to a large modern extension. Internally the premises provide extensive communal areas and ensuite rooms. The company have invested substantially in the home recently and provides a high standard of accommodation for the people that live there. The home has been adapted for wheelchair users with ramped areas, wide corridors and a wide variety of fixtures and fittings for the more physically dependent residents. There is a call bell system available for those people wishing to use it. There are two shaft lifts, which serve ground to first floor. There is parking to the front and side of the home and spacious grounds to the rear of the property. The home has its own vehicle which is used daily to access a full range of resources in the community in accordance with residents assessed needs and personal preferences. Each resident has a structured timetable of activities, many of which make use of community facilities. The home also arranges less formal activities, regular outings and trips to the shops daily. The residents living at Breage House have been placed by the department of Adult Social Care following a thorough assessment of their needs. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: the annual quality assurance assessment (AQAA) that was sent to us by this service (the AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service, and it also gave us some numerical information about the service); Surveys returned to us by people using the service and from other people with an interest in the service; Information we have about how the service has managed any complaints; What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement; The previous key inspection and random inspection. What has this told us about the service? This service sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us the information we asked for. We looked at the information in the AQAA and our judgement is that the care home is still providing a good service and that they know what further improvements they need to make. The AQAA told us the home does the following to incorporate the views of people that use the service: The views of all people who use our service are important to us. Quality assurance audits are used to plan and project so that the service we offer meets the needs and wishes of people we support, their families, advocates, relevant professionals etc. Quality assurance questionnaires are sent out annually to residents, staff, family members/advocates, and professionals involved (i.e. social workers, health professionals etc). The questionnaires are confidential, and information is collated by Swallowcourt. These questionnaires have been returned recently, and we will use these to identify areas which need improvement and to develop an action plan based on the data obtained. The residents Care Plans are reviewed monthly and with additional updates when required. We have six Monthly care plan Reviews with our Residents and with their choice of representatives, such as ASC social workers, CPT and any professionals who are involved with their care, their families, their key workers. We have regular clinics for residents who have complex needs ie epilepsy, dementia, which involve liasion with local G.P. C.P.T. specialists and families. We are working with the Cornwall first team and some residents on communication passports and other forms of augmented communication systems ie. Keyword. We have monthly residents meetings and regular individual meetings, this gives the resident their opportunity to discuss their individual needs and to address any issues privately, this is also the opportunity to discuss with them the keyworker system, Annual Service Review Page 4 of 6 reminding them what it means to them, how they control this, and to ensure they have the proper support from their keyworker. The Annual Quality Assurance Assessment also told us the home made the following changes as a result of listening to people that use the service: Person Centred Plans are in progress with everyone we support. Residents are involved as much as possible in compiling these, to ensure that their plan is personal, accessible and meaningful to them. Information is clear but detailed and precise. Person Centred Planning assists people to express their needs, wishes and choices by finding out what activities people enjoy and helping them to record goals, and have thus been instrumental in helping people to get out and about more. Breage House provides an excellent service for this particular resident group, with specialist facilities to match. We have a keyworker system which supports residents to plan and action dream. This year some residents, with their keyworkers, have organised weekends away, theatre break, camper van holiday and day trips. Residents have plans for more activities in the pipeline. We have had further external work in respect of grounds and garden. The gardens and outside facilities are actively used by residents and families alike, with the completion of the pergola, the mill house, the wild garden, and the herb garden making a further activities area that is accessible to all residents who enjoy this pastime. Swallowcourt are a company committed to providing quality outcomes for our residents at Breage House and this we feel underpins the ethos of the home and our commitment to the Valuing People white paper. Representatives of the company visit the home regularly and Regulation 26 reports are completed, reviewing the conduct of the service which includes spending time with residents, listening to their views and giving feedback on observations. The AQAA also informed us that staff recruitment, retention, and training is a priority. Reviews of policies and procedures occur. The AQAA also told us that they have received one complaint since the last service review, which was resolved within 28 days. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 18th November 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
