Briarfied is a residential care home providing accommodation for six people who have a learning disability. West House, who have a number of similar homes, operates the home. The home is large detached bungalow, situated in a residential area on the outskirts of Workington. There is ramped access to the front and rear of the building. Garden areas are to the front and rear. There is also a large patio area to the rear of the home. Residents have access to a wide range of both in-house and community based activities. The home has a wheelchair accessible vehicle for residents use. West House is a voluntary organisation providing a range of community based support services and residential homes to people with learning disabilities living in West Cumbria and Carlisle. The full range of West House services are open to residents, and includes day services, art, craft and music workshops, work placements and a wide social network. The current scale for charging can be discussed with the manager or a representative from West House. What the care home does well Briarfield provides a high standard of personal, social and health care to people who may have complex needs and /or physical frailty. Care is delivered in a sensitive and person centred way and families are invited to become involved if they wish. People report high levels of satisfaction with this service. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: Prior to this inspection we, the Care Quality Commission (CQC) asked the manager to complete an Annual Quality Assurance Audit (AQAA). It is a legal requirement that this is filled in by every registered service. It contains details of what has improved in the past 12 months and what the service hopes to achieve during the coming year. The form was comprehensively completed and returned to CQC within the allowed timescale. We also sent surveys to residents, staff, relatives and health care professionals that may visit the home. We had a very positive response and some of the comments are included in this report. Based on the information contained in the AQAA, the feedback received in our surveys and observations during a visit to the home we judged that Briarfield was continuing to provide a safe service for those living there. We, therefore, decided to carry out a short random inspection rather than a full key inspection at this time. Margaret Drury made an unannounced visit to Briarfield on the 26th April during which we were able to speak to residents, staff and the manager. We looked at a selection of care records and discussed the management and administrations of medicines. The focus of this inspection was personal and health care support and complaints and protection. When we arrived some residents were just finishing their breakfast and others were getting ready to go out. We undertook a short tour of the building with the manager looking at the environmental standards within the home. We looked at the care records for 3 of the people living in Briarfield and found them to be relevant, informative and up to date. The care plans are a working document that give all the necessary information to the care staff in order that they can provide the very high level of care required to meet the needs and expectations of the residents. Each care plan is tailored to the individual and supports the provision of care and lifestyle of choice. People receive care and support in a way and at a time that is acceptable. Personal wishes and choices are documented and all residents are involved with the preparation and review of their individual plan. If communication or involvement is difficult an advocate is called on to act on behalf the resident. Relatives are invited to be part of the review process but the manager told us that, in the main, they are happy to leave this to her and the care staff. We had a positive response to the surveys sent to residents and they told us that; Briarfield gives a good quality of care and the home is always clean and tidy. They keep my room tidy and take me out. Briarfield does everything well. Looks after me and cares for all my needs. Comments from staff included; Care Homes for Adults (18-65 years) Page 4 of 9 The home meets the clients needs in a supportive, emotional and physical way. It offers a wide range of activities/outings to enhance their lives. Everything meets needs and safety for all residents and staff. The local doctor and district nurse both agreed that the home met all the needs with very caring staff. Risk assessments are well documented and cover times when residents are inside the building and out in the community. These, like the care plans are reviewed and updated regularly especially if there has been any change in residents circumstances. Nutritional assessments are completed and if special diets are required the advice of the district nurses and dietician is requested. Weights are also monitored. Each resident has an health action plan that gives detail of all health care needs and how these are met. Access to chiropody, dental and optical treatment are available to all and details of visits and appointments are well recorded. The home makes every effort to organise different activities but this is very much up to the individual. One goes horse riding and another goes swimming. All go out to help with shopping for the home and their own personal shopping. Residents go out to day care on most days and are able to take part in activities at each centre. They also like to go out for meals. There is a complaints procedure in the home and all residents and their relatives are given a copy. There is also a complaints log in place but there have been none to record. The commission has not received any either. As this is a small home the staff know the residents extremely well and any concerns are dealt with and settled immediately. Medicines are well managed although the amount prescribed is quite low and at least 1 resident has no prescribed medication at all. All medication is securely kept in a locked cabinet and all the staff have completed relevant and certified training. We checked the records and found these to be accurate and correct. This means that residents receive their medication as prescribed and at the right time. There have been no safeguarding matters to report and the staff are well aware of their roles and responsibilities in this matter. There are policies and procedures in place and there is a copy of Cumbrias policy available for the staff to read. We have not received any referrals from social services or any other external sources. Staff attend training in safeguarding procedures and this subject is also covered in the induction programme. We judged that residents were able to voice their concerns and were safeguarded at all times.
