The home applied to the CQC for a major variation to their registration numbers. This application was approved following a site visit by a CQC registration inspector. The

Amended certificate issued on 11/06/2009 to reflect changes recorded above -certificate number 2000012914 Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Buxton Lodge is a large detached property located in a quiet residential area of the town of Caterham on the Hill in Surrey. The property has been refurbished and extended to provide accommodation and nursing care for up to 44 older people, some of whom may have dementia. The registered provider is New Century Care Ltd, an organisation that has 27 registered care services throughout the UK. Service user bedroom accommodation is arranged over two floors, served by passenger lifts. The bedrooms are for single occupancy and most offer en-suite facilities. A communal lounge and a separate dining area for service users are on the ground floor. There is an attractive and well-maintained garden. The garden is not enclosed. This is taken into account during the assessment process when considering any new admissions to the home. There is limited parking for visitors to the side of the property. Fees charged are based on personal assessed need but range from £800.00 - £1,000.00 per week. What did we do for this annual service review? We looked at all the information we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) sent to us by the service. The AQAA is a self assessment that focuses on how well outcomes are being met for people using the service. It also gives some numerical information about the service. Information we have about how well the service has managed any concerns or complaints. What the manager has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection report dated 4th November 2008. Relevant information received from other organisations or agencies about the service. What other people have told us about how the service operates. We had a brief discussion with the registered manager during the review process to clarify points raised in the AQAA and to further update us about the service. What has this told us about the service? This is the second annual service review (ASR) of this home and was undertaken on the 23rd October 2009. A previous ASR of Buxton Lodge was undertaken on 15th May 2008. The ASR confirmed our initial judgement that the home was providing a good service, but in light of issues raised with us regarding inadequate staffing levels and no registered manager, it was recommended that a key inspection of the home be brought forward. The last Key Unannounced Inspection of this service was therefore undertaken on the 4th November 2008. The last key inspection report confirmed the quality rating of 2 stars - Good and was overall positive about the service provided at the home. The home had continued to work within its planned programme of service development, refurbishment and redecoration. Building work to extend the communal areas and bedroom capacity of the home was well underway at that time. A new manager had been appointed by the provider and had made application to register. As a result of the inspectors visit there were some identified areas for further improvement. Three statutory requirements were awarded and two good practice recommendations were made. The manager confirmed with us in preparation for this review, that the issues raised in the previous inspection report have now been addressed and resolved. In preparation for this annual service review the home completed and returned their AQAA (annual quality assurance assessment) when we asked for it. The document was seen to be comprehensive and provided us with a good picture of what was actually Annual Service Review Page 4 of 7 happening in the home. It identified specific areas where the service had improved in the last year and others where further developments were being planned. It included evidence that the service regularly seeks the views of service users and other stakeholders to inform their planning. For example, there have been recent changes made to the menus and the quality of food provided. There have been more activities arranged and a programme of regular exercise and pamper days have been introduced for service users. In addition, Do not disturb signs have been provided to service users. These may be displayed on the front of their bedroom doors if they wish, to further promote their privacy and dignity in the home. Some staff have registered as Dignity Champions to challenge existing practices that could be improved upon for the people who live in the home and to encourage and support good practice. The home has now undergone a complete programme of refurbishment throughout and a large extension has been built to provide six new bedrooms, the remodelling of two existing bedrooms to reduce occupancy to single accommodation and the addition of an extra bathroom, sluice, hairdressing/therapy room, lounge and dining room. The six new bedrooms and the two remodelled rooms are located on the first floor and are all the same size and layout, in excess of 12 square metres and are furnished and decorated to the same specification. The first floor is reached by two passenger lifts and staircase. All areas of the home are fully accessible to wheelchair users. The home makes contact with the commission if there are any significant events that affect the well being of the people who live there. The service also has strategies in place to ensure that those who live there and those that visit can make their views and complaints known. In the last 12 months six formal complaints have been received by the home and dealt with effectively using their own complaints procedure. In addition, there have been 3 adult protection investigations in which the CQC and the home have been involved. These have now been closed. The provider cooperated fully and dealt with the issues raised in a satisfactory manner. Evidence shows that robust staff recruitment procedures are in place, which also ensure that residents are safe and protected from any potential for harm. Services that provide social care must be sensitive to people of different cultures, age, gender, faith and sexual orientation. Throughout the homes annual quality assurance assessment, it was clear that equality and diversity issues are consistently being promoted by the manager and incorporated within the service. The home takes the training and development of staff very seriously and looks for individuals who are able to undertake all aspects of mandatory training and prove competency at NVQ Level 2 or above. The AQAA records the home exceeds the 50 standard required for trained staff on duty rosters, which further ensures that the home safeguards the welfare of residents. The manager also records in relation to staffing.....there have been improvements made to night staff routines and rosters, which have made the service user group feel safer. We keep our staffing levels constantly under review to ensure we meet service users needs. We have recruited into our vacant staff posts and built up a bank of cover staff to eliminate agency use, thus improving continuity for our service users. We provide training in all mandatory areas and will including Mental Capacity Act 2005 and Deprivation of Liberty Safeguards training this year. This will ensure that staff are aware of new legislation and the good practice demands placed upon them. What are we going to do as a result of this annual service review? There will be no change to our current inspection plans for this service. A key unannounced inspection will be undertaken by the 3rd November 2010.
