What the care home does well The home continues to meet the assessed needs of the people who use the service Service users who spoke with the inspector felt that staff treated them with dignity and respect. The home offers a wide range of leisure activities. Those who live in the home enjoyed food offered by the home. Appropriate complaints systems are in place. What has improved since the last inspection? Since the last inspection, the home has ensured that all previous requirements and recommendations have been met. The responsible person has ensured that all care staff receive all mandatory training, as required by law. The home`s statement of purpose has been updated to include information about the new registered manager. Individual care plans have been further developed, in order to demonstrate that all aspects of care included in the pre-admission assessment have been taken into consideration. Service users are now offered keyworking sessions in accordance with their care plan and/or their Care Programme Approach minutes. The registered manager has ensured that all relevant risk assessments are drawn up and reviewed on regular basis, in order to ensure that any activities in which service users participate are as far as reasonably practicable free from avoidable risks to health and safety of the people who use the service. Records in relation to the food offered in the home are now appropriately maintained. The registered manager has ensured that appropriate systems are in place to monitor and record weight of any service users, as previously required. Improvements have been noted to the home`s medication systems, as well as recording of any accident/incident affecting the wellbeing of service users and staff. Some improvements have also been made to the home`s premises, however further work is required to update the home`s kitchen. The registered manager has ensured that staff personnel files contain all information listed in Schedule 2 of the Care Homes Regulations; in order to evidence that appropriate recruitment systems are in place. The registered manager has also ensured that staff receive supervision and annual appraisals in line with the National Minimum Standards. What the care home could do better: Improvements are required to ensure the safe storage of food in the home. The home must also install a controlled drugs cabinet, in order to comply with the safe storage of medication. Further improvements are needed to the home`s environment. The home is situated in a residential street in Stamford Hill, within walking distance from an overground train station, shopping facilities and other local amenities. The home is located in a domestic property that blends in with other houses on the street. The premises are not wheelchair accessible. This is an overview of what the inspector found during the inspection. The quality rating for this service is 2 stars. This means the people who use this service experience good quality outcomes.
This inspection visit took place over one day and was unannounced. During this visit, the inspector spoke to all service users living in the home, the home’s registered manager, deputy manager and one member of care staff working in the home. He also conducted a tour of the premises and viewed various records. Prior to this inspection the home was asked to complete the Annual Quality Assurance Assessment. This was returned within given timescale and was completed by the Home’s Deputy Manager. Some of the information provided in the assessment has been incorporated into this inspection report. The aim of this unannounced inspection was to check the home’s compliance with the National Minimum Standards and the Care Homes Regulations. The inspector would like to thank service users and staff for contributing to this inspection. What the service does well: What has improved since the last inspection?

This judgement has been made using available evidence including a visit to this service. Prospective service users have the information they need to make an informed choice about where to live. Good admission systems were in place and prospective service users have an opportunity to “test” drive the home. EVIDENCE: Since the last inspection, the home has updated its statement of purpose and the service users’ guide to include information about the new registered manager. Appropriate admission systems were in place. Each care plan that was checked during this inspection contained appropriate pre-admission assessments. Service users who spoke with the inspector commented that they were happy with their admission process. The following information was provided by the home in their Annual Quality Assurance Assessment: “When we receive a referral, we carry out a very comprehensive assessment on the client. This assessment is usually carried out by our most experienced and senior managers. The client is allowed to view the home before he decides to try it out. If he agrees to try our home, he is then put on a program where he would visit the home during the day eventually leading to overnight visits. The client can change his mind at any

During this period we will also be monitoring to see whether there are any needs which have been omitted during the assessment. the client is also provided with a service user guide whose contents is explained to the client verbally as well. If the client decides he wishes to choose our home, he then goes through a trial period which is usually followed by a permanent move. During the trial period the client is still under no pressure to accept the placement. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People who use the service benefit from a good care planning process. They are encouraged to take part in planning their care and contribute to other aspects of life in the home. Appropriate risk management strategies were in place. EVIDENCE: During this visit the inspector reviewed care plans of four randomly chosen service users. At the last inspection, it was noted that care plans viewed by the inspector were quite basic and required further development to include issues such religion, culture, sexuality, assessment and management of risks etc. There was evidence that the service users were involved in the decisionmaking. Care plans were not always reviewed following important events in service users’ lives. There has been a significant improvement since the last visit and the inspector was satisfied that appropriate improvements have been made to each service user’s file. Care plans were now reviewed on a monthly basis or more often where it is necessary.

Each person had a designated keyworker, who was responsible for coordinating care of their keyclient. Keyworking sessions were also organised, during which any individual goals, progress and areas that needed further support were discussed. There has good an improvement since the last visit and the registered person has ensured that each service user is offered keyworking session in accordance with their care plan and/or Care Programme Approach minutes. Any refusals from service users to attend keyworking sessions were now being recorded. Regular service users’ meetings were also held, during which service users could make any comments and suggestions as to how the home should be run. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16, 17. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users continue to take part in appropriate leisure activities. They are encouraged and supported to develop and maintain friendships and family links. Service users enjoyed food in the home, however improvements are required in storage of food products. EVIDENCE: Documentation viewed and conversations with the service users confirmed that those who use the service are encouraged to take part in the local community and to maintain and develop social, emotional and communication skills. Records viewed showed that service users visit their families and friends, attend cultural centres, do some of their own shopping and go out to cinemas, theatres and pubs. Some of these events are organised and facilitated by the home. The following information was included in the home’s AQAA about improvements in the last 12 months: “We have encouraged the clients to have

We have also encouraged all our clients to interact with other clients from Cascade Care Ltd who may have shared interests. We also now provide funding for outings for meals to places of their choice.” Annual holidays are organised and available to the people who use the service. This year some of the service users went to Butlins in Minehead. Families and visitors are welcome to the home. The home had a visitor’s policy in place. Visitor’s book was also maintained. Service users said that they were able to invite their friends and families into the home, subject to the home’s visitor’s policy and individual agreements as agreed by individual placing authorities. Meals offered by the home can be eaten in the dining room with other service users. There were appropriate food supplies in the home and people who spoke with the inspector said that they enjoyed food in the home. Fridge and freezer temperatures were recorded. Records of food offered to those living in the home was viewed and it was appropriately maintained. Improvements are required to the way the food is stored, as at the time of this inspection, some perishable products, such as salad cream and tomato ketchup were not labelled when opened. In addition some meat products were kept in the top drawer of the freezer and this could cause cross-contamination. The inspector shared a lunch meal with the people who use the service and staff working in the home. Food prepared on the day of this unannounced inspection, was tasty, attractively presented and nutritionally balanced. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff working in the home ensure that service user’s emotional and healthcare needs are going to be met. Good medication systems were in place. EVIDENCE: The inspector was informed that all service users who lived in the home at the time of this inspection attended their own personal care and staff only offered verbal prompts as and when required. There was evidence of an input from Psychiatrist, Community Psychiatric Nurses and Social Workers. The majority of the psychiatric reviews were carried out promptly. The majority of the service users choose to attend their GP surgeries and hospital appointments independently. In some cases staff accompanied service users to appointments. At the last inspection visit, it was identified during the case tracking exercise, that staff did not always monitor the weight of a service user, where concerns have been noted in relation to his food consumption and obesity. As part of this visit, the inspector checked the home’s medication systems. The home uses a Monitored Dosage System and there were appropriate records of medication received into the care home, administered to each person who used the service, and returned to the pharmacist. At the time of this inspection visit, one person was self-medicating. One of person at the home was administered medication, which required storage in a controlled drugs cabinet. Whilst the inspector was satisfied that appropriate recording systems were in place in relation to this medication, in accordance with change in legislation, a cupboard is required to be installed for the storage of Controlled Drugs. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22, 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Appropriate complaint systems were in place and those who used the service were aware of their right to complain. People who used the service were protected from abuse, neglect and self-harm. EVIDENCE: The inspector was informed that there have been one complaint made to the home since the last inspection. This complaint was received two days prior to this inspection and was being dealt with. The home had a complaints policy in place, which included details of the Commission for Social Care Inspection. People who used the service were aware of their right to complaint. People who use the service told the inspector that they would feel confident to talk to the manager if they were not happy about anything in the home. Most of the service users manage their finances, however support and guidance is offered to those service users who may require help in managing their finances, due to risk of substance abuse or money mismanagement. Improvements have been made since the last inspection to the way staff record financial transaction in relation to the people who choose to keep some of their finances in the staff office. There has been one adult protection issue in the home since the last inspection. The home informed the relevant authorities and the matter was appropriately dealt with. The home informed the Commission of any events that adversely affected the wellbeing of those accommodated in the home and any other issues which required to be reported in line with the relevant regulation, as previously required. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 28, 30. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Although some improvements have been made to the home’s environment, further work is needed to ensure that all parts of the home are decorated and maintained to a good standard. EVIDENCE: The home is an ordinary domestic property situated in a residential street. The inspector carried out a tour of the premises, as part of this visit. There have been some improvements made to the home, some of the service users’ bedrooms have been repainted and new furniture has been purchased. One of the staff office areas located on the top floor has been converted into a bathroom/shower room and an additional toilet. One part of the home, which has begun to show its signs of wear and tear, is the kitchen area, as well as the home’s conservatory. Some of the cupboards

The registered person must ensure that the home’s kitchen and conservatory areas are upgraded/improved. People who use the service who spoke with the inspector said that they were happy with their bedrooms and the communal lounge. The home has a large garden at the rear of the home, which is sometimes used by the people who live in the home and staff working there. At the time of this inspection, the back garden fence required repair/replacement. The premises were found to be clean and hygienic at the time of this unannounced inspection. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35, 36. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are supported by an effective staff team, which is appropriately selected, managed and supervised. EVIDENCE: As part of this visit, the inspector viewed duty rosters, which showed that there were sufficient numbers of staff on duty. There are two members on each shift, including nights. Night staff remain awake throughout the night. Staff who spoke with the inspector stated that staffing levels were sufficient to meet the needs of the current service user’s group. Staff working in the home were observed working with the people who use the service in a respectful and professional manner. The majority of staff either have obtained the NVQ qualification or were in the process of obtaining one. Since the last inspection, the registered person has ensured that staff receive mandatory training, as previously required.

JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 42. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Those who live in the home benefit from a well-run service, which is managed by a competent manager. The health and safety records and checks were generally up-to-date. EVIDENCE: There has been a change of the home’s manager since the last inspection. Following the departure of the previous manager, a new manager has been appointed. The home manager is a Registered Mental Nurse and she has recently been registered with the Commission as fit to manage a home for people with mental health issues. There was good evidence to demonstrate that both the registered manager and the home’s deputy manager (who is a qualified Social Worker), have made a good progress to ensure that all previous requirements and recommendations have been met. Appropriate quality assurance systems were in place. The home received regular visits from the “registered provider” on a monthly basis. After each visit, the registered provider produced a report, copy of which was sent to the home and to the Commission. The majority of health and safety checks were in place, however the home’s Landlord’s Gas Safety Certificate was out of date and required renewal. The home was adequately insured for its purpose.
