The Pharmacist Inspector from CQC visited this home to check compliance with requirements made in relation to medicine management at an inspection earlier this year We conducted this inspection by talking to four members of staff and two residents We watched medicine being given to 7 people We also looked at records relating to medicine management and checked care plans for information relating to medicine use for some people The homes medicine storage and recording systems are of expected and required standard The home uses medicine dosage system We saw that the nurse giving medicines did not wash her hands once while giving medicines In this time she handled medicines with her fingers and also administered eye drops for one person On three occasions out of the seven peoples medicines administration we watched, we saw that the nurse signed for the medicine as she removed the medicine to give and not after she had given it This shows some poor practice around the procedures used when giving medicines to people We noted that the medicine round did not finish until 11:00 a m and had taken 2 hours We discussed this with the managed and staff as the records indicate medicines to be given at 9:00 a m when in fact they were given for some people at 11:00 a m We asked to see training update records for this member of staff and we were informed the this member of staff was deployed from another service on a regular basis and there was no information available at this service In the main the records for recording of medicines being given were complete except for a few Gaps where there should be a signature [initials] for medicines administered or a reason recorded for non administration We found one medicine left from the previous morning for one person by their bedside This medicine was for constipation and this person had not taken this medicine although it was signed as given This person had said if I could only walk a bit I will be alright but they dont allow me to walk She was referring to her constipation This shows that the people in the home are not involved in their care decisions One persons medicine dosage directions were incorrectly printed on the Medicine Administration Record [MAR] chart by the pharmacy The staff had not corrected this at the start of the cycle and as a result there were two signatures [initials] against this medicine which was not prescribed or available These signatures indicate false records A check of two separate medicines against records showed there were less signatures than doses available for this medicines which would be labeled complete the course as these medicine were antibiotics For two people we saw medicines being left with the person to take later There were no supporting information in the care plan for this process In one room we saw a medicine which was one that can be bought over the counter There was no supporting guidance in the care plan on the use of this medicine Care Homes for Older People Page 4 of 12 We checked the medicine management policy of the service and found that these were the corporate ones There were no additional policy on local procedures added to the corporate ones Local policies and procedures give clear guidance to staff of exact processes and in home and lead to safe, consistent methods in practice Following our previous inspection in June 2010, we had received an improvement plan from the manager which stated that trained staff will receive refresher medicines training by the 17/07/2010 We asked for evidence of this training and were shown certificates of training that had taken place in May 2010 We were informed of a training session that had been booked for the 4th August 2010 for some members of trained staff but not all We received correspondence from the manager following the inspection to confirm that named nursing staff had received training update on medicine management from their pharmacist We checked controlled drugs and the records relating to controlled drugs These are medicine that require stricter controls by law We found Controlled Drugs stock and records to be in order What the care home does well: What they could do better: If you want to know what action the person responsible for this care home is taking following this report, you can contact them using the details set out on page 2 Care Homes for Older People Page 5 of 12 Are there any outstanding requirements from the last inspection?
