Cherry Orchard is a care home with nursing situated in Dagenham. It is some way from public transport, and can be difficult to find. The home is situated in the part of Dagenham Avenue that is next door to the Goresbrook Sports Centre (opposite EustowAnnual Service Review Road). Care UK Community Partnership Ltd, a large, private, provider, which has many similar homes across England, manages the home. It is a single storey, purpose built home, which is divided into two units. Castle Green is a 12-place unit for people over the age of 65 who have functional mental health problems. Thames/Ripple is a 28place unit for people aged over 65 who have dementia; two of these places are for people under 65. All places at the home are block purchased by Barking and Dagenham Primary Care Trust, with two beds on Thames /Ripple being for respite care. All bedrooms are single and have en-suite toilets and wash hand basins, and all are a good size. Castle Green has a small kitchenette and open plan dining and lounge area, leading onto an enclosed patio and garden. There is also a separate conservatory. Thames/ Ripple comprises of two corridors, connected by a large dining room. Each corridor has 14 bedrooms, a lounge, small conservatory, bathroom, toilets and shower. All areas of the home including the garden have full disabled access. The Statement of Purpose and the Service User Guide are issued to every prospective resident and both of these documents are displayed in the entrance hall of the home. A copy of the most recent inspection report is also available. A resident or relative/representative could ask for his or her own copy, which the manager would make available. The weekly fees for the home are #715 for people with dementia and #898 for people with mental health issues. This information was given by Violet Kotu (manager) on 25th November 2007. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for since the last key inspection November 2007 This included: the Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self assessment document that focuses on outcomes for people using the service. it also provides us with statistical information about the service. Information we have about how the service has managed any complaints What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection (November 2007) What has this told us about the service? At the last key inspection this care service had met all of the National Minimum Standards for older people in all areas. This home was assessed as being an excellent service - 3 star. The home sent us their AQAA when we asked for it. It was comprehensive and gave us all of the information we asked for. We looked at the information in the AQAA and our judgment is that the home is still providing an excellent service and that they know what further improvements they need to make. The AQAA identified that during the past twelve months the home has improved in the following areas: Staff have undergone training so that they are now more confident in showing potential service users and their families around the home and to answer any questions they may have. Relatives are encouraged to visit the home on more than one occasion and if appropriate for a potential service user to spend time meeting staff and other service users. The home now has a dedicated GP who is very familiar with the service users medical needs. All service users have been assessed and reviewed by the Community Dentist, who has also provided training on oral hygiene for the staff. Daily progress reports are now more detailed and reflect on how the service user has spent their day and choices they have made. The activity programme has been adapted to reflect the service users abilities and all staff are encouraged to join in the activity sessions. Some of the service users spend a Annual Service Review Page 4 of 7 short amount of time helping out in the laundry or making a cup of tea in the kitchen. All bedrooms and communal areas have been redecorated and the flooring replaced. the home also has many attractive murals and paintings which have created a more stimulating environment for service users. No agency staff have been used which allows service users to receive continuity of care from staff who are familiar to them. Five members of staff have completed the four day first aid course and the Activity Co-ordinators role has been further developed to include training and guidance for Activity Based Care. Nurses are now taking on more responsibility for their own units and developing their leadership skills. The AQAA identified that their improvement plans for the next twelve months include: The service is looking to establish or gain access to a simple information leaflet for prospective service users and their families dealing with different types of funding and other information that would assist them with the process of choosing the right home. The home has adopted the principles of the Gold Standard Framework; this is to ensure that service users receive the best quality care in their final days but are looking to become an Accredited Home. An end of life specialist nurse has been employed by the local authority and she is going to provide training and support to the staff. To continue and to increase day trips for smaller groups of service users and to explore other outdoor activities. The service will continue with the personalisation of service users bedrooms and lounges. To revamp the garden so that there is a secluded quiet area as well as a sensory and vegetable garden as well as replacing the paved area with tarmac to improve safety. To encourage and implement a relatives support group and to have more combined social events such as afternoon teas and barbeques. To increase the number of staff meetings and to vary the times to suit family commitments. For the manager to complete her leadership course. The home continues to let us know about the things that have happened since our last key inspection and they have shown that they have managed these issues well. there has been one safeguarding investigation in the past twelve months and this has been investigated by the local authority and concluded. The service works well with us and have shown us that their service continues to provide an excellent outcomes for the people who use it. What are we going to do as a result of this annual service review? However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
