What the care home does well Residents spoken with said that they were very happy at the home and were receiving the care they needed. They were complimentary about the nature and approach of staff to their needs and were happy with the food provided and the activities available. Residents could receive visitors whenever they wished and stated that their visitors were always made welcome. Residents had regular meetings with the owners to share their views on the service they receive and to make suggestions for improvement. The owners were said to respond very positively to any comments or concerns raised with them. Examples of comments made by the residents during the site visit: `The home is always nice and clean and warm and I am very happy here- the food is good and there are no restrictions on anything.` `The staff are very good and caring and have time for you. I can ask then anything and they always do it.` `They look after me very well and I am looking forward to Christmas here- it was very good last year.` `I can get up and go to bed whenever I like- there are no restrictions and I like that, I like to keep independent where I can.` What has improved since the last inspection? The owners have complied with all the requirements made at the last inspection. Paper towels and liquid soap are now used in the communal toilets and this aids infection control. The owner confirmed one new hoist has been provided to ensure the safe moving and handling of some residents. This makes a total of three mobile hoists and one ceiling hoist. Bath hoists are also provided. Some new dining room furniture has been purchased and new table linen and kitchen equipment has also been purchased. A new commercial washing machine is now in use in the laundry The external security lighting has been improved. Staff supervision has also improved and is recorded. The care plans are more comprehensive with staff undertaking regular comprehensive reviews. What the care home could do better: It is required that at least one member of staff completes a comprehensive medication course as discussed during the inspection. Staff should then undertake regular assessments to ensure they remain competent to administer the medications safely. This was a key unannounced inspection that took place on 23rd November 2007 and was conducted by Sue McGrath, Regulation Inspector for the Commission for Social Care Inspection. The key inspections for care home services are part of the methodology for The Commission For Social Care Inspection, whereby the home provides information through a questionnaire process and further feedback can be gained through surveys sent to service users and relatives and information provided from professionals associated with the home, wherever possible. The actual date of the site visit is unannounced. Additional information was also obtained through the Annual Assessment Quality Assurance (AQAA) review, which all services registered with the Commission for Social Care Inspection (CSCI) must now complete on a yearly basis. At the site visit, service users and staff were spoken to, records were viewed and a tour of the environment was undertaken. Judgements have been made based on records viewed, observations and verbal responses given by those people who were spoken with. These judgements have been made using the Key Lines of Regulatory Assessment (KLORA), which are guidelines that enable the CSCI to be able to make an informed decision about outcome areas. Further information can be found on the CSCI website with regards to information on KLORA’s and AQAA’s. The requirements made at the last inspection had been complied with. Overall this was a positive inspection with generally good outcomes for service users. The inspector on leaving the home was satisfied that residents were both safe and well cared for. The personalised needs assessment means that people’s diverse needs are identified and planned before they move to the home. EVIDENCE: The home has a Statement of Purpose and Service User Guide, which provide residents or their representatives with all the information they need to make a decision about moving to the home. Evidence was seen that all residents have a comprehensive assessment of their needs prior to moving into the home ensure their needs can be met by the home Residents and their representatives are encouraged to visit the home before deciding to move in and longer trial visits can be arranged with the owners. Evidence was seen that staff have received appropriate training and collectively have the skills and experience to meet the needs of the residents. The home does not provide intermediate care and therefore standard 6 is not applicable. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 and 11 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents benefit from having clear and in-depth care plans that identify their individual needs and give clear guidance to staff. Care plans are regularly updated to ensure changes are recorded and acted upon. Health needs are met and residents benefit from having full access to all professional health care services as required. Residents are protected by the home’s policies and procedures for dealing with medicines. EVIDENCE: Each resident has a personal care plan which gives good information regarding the care needs of that person. The plans are detailed and comprehensive. The plans were clearly laid out and gave clear instructions to staff to enable them Chimnies Residential Care Home DS0000029262.V350460.R01.S.doc Version 5.2 Page 12 to offer the required level of care. Each resident has a named key worker who reviews the care plan on a monthly basis. These reviews are thorough and well written. It is recommended that these review notes be signed by the resident to confirm they agree the updated plan. All of the plans viewed had in these in depth assessments in place. Contained in the plans were full details of any medical interventions that have occurred. These are well recorded along with the outcomes for residents. All residents are registered with a local G.P. and have full access to other care professionals according to need. District Nurses, Chiropodists, Opticians, Physiotherapist and Occupational Therapists give full support, according to need. The staff have a good understanding of tissue viability and infection control. Residents daily records are being well maintained. They are detailed, comprehensive and reflected plans of care. Entries made are signed and dated upon completion. Residents weight and nutritional records are being maintained and regularly updated. Evidence was seen on each plan that residents have agreed for their individual personal information to be shared with their immediate families, as required from the last report. Evidence was also seen that all information is stored securely. Risk assessments in care plans are in place where they are necessary. None of the current residents self medicates, although each room does have a locked cupboard or drawer to safely store medication. The method used by the home to safety administer medication followed the guidelines laid down by the Royal Pharmaceutical Society of Great Britain. . A monitored dosage system for dispensing medication is used by the home. The medical room was tidy with appropriate stocks of medication available. The home has a written medication procedure and policy this document has recently been reviewed. It is recommended that this include protocols and guidance on administering PRN medications. The home’s medication administration sheets were inspected and no obvious gaps in administration were found. The controlled drugs cabinet is a locked safe within a locked cupboard which is password protected. The controlled drugs balance was checked and found to be correct. None of the staff spoken with had undertaken comprehensive training in the safe administration of medication. Some had completed a one day course. Good practice demands that at least one member of staff should have proved competency in a more advanced programme of instruction to safeguard residents. The residents were spoken with during the inspection and all confirmed staff treated them with respect and cared for them in a caring and pleasant way. The home has a stable staff group who are familiar with all the residents. Agency staff are not employed. Many of the staff have worked at the home for many years. The home tries very hard to offer a good level of service during any illness and subsequent death. It is recommended that further information be gathered at either admission, or at subsequent reviews, to ensure the wishes of the resident is recorded and used when needed. It is understood that this can be difficult to discuss but this information is best gathered when all parties are well and able to give clear instructions and can be assured they will be supported at this difficult time. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 q14 and 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Social activities and stimulation are provided in the home and as much as possible ensure daily variation and interest for residents. Residents are enabled to maintain contact with friends and family who are made welcome in the home. Wherever possible residents are given opportunities to make choices, therefore allowing for some level of control over their lives. The meals in this home offer both choice and variety and cater for residents’ particular needs. EVIDENCE: Several residents spoken with confirm they enjoy the variety of activities on offer and say staff really made an effort to keep them happy. A range of activities are offered and these include bingo, gentle exercises to music, reminiscence sessions with a professional coordinator, sing-song sessions and Chimnies Residential Care Home DS0000029262.V350460.R01.S.doc Version 5.2 Page 15 manicure sessions. Other activities include craft sessions and organised games. Boxed games are also popular. A professional entertainer comes every six weeks. Significant events are celebrated including birthdays and seasonal celebrations. One resident said she was really looking forward to Christmas as last year she had really enjoyed the party. She also said her family had been invited to the party and she had appreciated this very much. Local schools come in to the home to sing carols. Games/ papers/ magazines/books/ TV & Radio were in evidence for residents use. There did not appear to be any routines in the home that residents did not agree with. Some individuals spoke of getting up and going to bed when they liked. They could bathe when they liked, although realised that staff had to make sure everyone was assisted with bathing and that took time. Residents spoke highly of staff and they appeared to get on well with each other. Several residents confirm that their relatives can visit at any time and are always made very welcomed. Staff were seen encouraging residents to make choices about their daily routines and preferences were observed. One resident stated she felt she was still in control of her life and that was important to her. Where possible staff were seen to actively encouraged residents to remain independent. Where help was needed it was offered in a supportive way. All of the resident spoken with confirm the food is very good and that they are always given a choice. Drinks are freely available and evidence was seen that fresh fruit and vegetables are used. The home can cater for residents who have been assessed as needing a specialist diet. Residents are regularly weighed at their reviews to ensure a healthy weight is maintained. Records regarding food intake are well maintained. The kitchen is clean and tidy. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17 and 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are protected by a robust complaints system and service users and relatives feel their views are listened to and acted upon. The home has robust adult protection policies and procedures to ensure that residents are protected from abuse. EVIDENCE: The home has a clear complaints procedure and residents are issued with a copy for their reference. Residents spoken with said they know how to make a complaint if they needed to and felt comfortable in doing so. The home has received two minor complaints this year and had dealt with both appropriately and within the designated time scales. Both issues were recorded appropriately. Any minor concerns or requests were seen to have been addressed appropriately through residents meetings. The owner said they have very few complaints. They strive to maintain good relationships with residents’ relatives and listen carefully and act quickly on residents concerns. Residents said they felt safe and secure in the home. A number said they wouldn’t want to live anywhere else and trusted the owners and the staff to care for them. The owner confirmed that all residents who wished to are registered to vote and at election times local M.P.s and Councillors will often visit the home. A policy for the protection of vulnerable adults is in place and staff are made aware of the content through induction and supervision. When spoken with staff displayed a good understanding of the subject of Adult Abuse. Staff have received appropriate training on Adult Protection and POVA to ensure residents are fully protected from potential harm. There have been no referrals under the POVA scheme. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 24, 25 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents benefit from living in a clean, safe, well-maintained environment and have safe access to comfortable indoor and outdoor communal areas. Residents are encouraged to maximise their independence by having access to the range of specialist equipment supplied by the home. EVIDENCE: Residents have the use of two large lounges and a pleasant dining room. The home has extensive gardens with lovely views, which are well used in the summer. Several residents commented they enjoyed watching the animals. All areas of the home are well decorated and furnished to a good standard. All radiators in communal areas and residents bedrooms are guarded. All upper windows have restrictors fitted. All of the bedrooms are single rooms and have been personalised to reflect the individual tastes and interests. All residents have access to sufficient bathroom and toilet facilities, including assisted baths. There are three mobile hoists and a fixed ceiling hoist for use by residents. All of the home’s equipment is regularly serviced and maintained. The home’s laundry was clean, well organised and maintained. The industrial laundry machines are of good quality. The home uses a red bag system for foul laundry to protect staff and residents. The home is very clean and hygienic. Residents say the home is always pleasant, clean and fresh. The issue raised at the last inspection regarding the use of cotton towels in the communal toilets has been resolved and residents now have access to liquid soap and paper towels. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staffing arrangements provide for a suitable mixture of care staff, administrative staff, domestic, catering, and maintenance staff. Residents benefit from staff that are trained and competent to do their jobs and who enjoy good morale. EVIDENCE: There were sufficient staff on duty to meet the needs of the residents on the day of the inspection and it was stated that there is always a senior carer on shift. Resident spoke highly of the care staff and said they were caring and respectful. They also confirm that staff responded quickly to call bells and are always understanding and accommodating of their needs. Staff are confident in their roles and have a good understanding of the needs of the residents. The home has a low turnover of staff normally, which means that many have worked in the home for several years and this helps create the necessary stability and experience required to give good care. Mandatory training has been completed and Mr Ong confirmed there is always one member of staff on duty who holds a current First Aid certificate. Most staff had completed training in areas of health and safety and senior staff had undertaken one day training in medication administration. It is again recommended that at least one member of the senior staff should complete a more comprehensive course of instruction in the safe administration of medication. All staff have received training in Adult Protection since the last inspection. The home has an induction programme for new staff. Staff records were inspected. CRB and POVA checks were seen in all files viewed and recruitment processes were seen to be sound at this inspection. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33 36 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The residents benefit from living in a home where the owners are competent, enthusiastic and very experienced with the care of older people and have a clear vision for the home. Resident’s benefit from staff who are appropriately supervised by senior members of staff. The health, safety and welfare of residents and staff are promoted and respected. Staff and survey respondents said both were experienced, supportive and resident focussed. Mrs Ong and her daughter are currently undertaking the Registered Manager Award and are hoping to complete in the near future. There are clear lines of management within the home and staff confirmed they feel well supported by the owners. Mr Ong stated that he undertakes quality assurance exercises. He does not publish the results although he confirmed that feedback from stakeholders in the service continues to be good. Residents and survey respondents felt the home was run with the residents best interests at heart. Regular residents meetings are held with outcome acted upon and recorded. Staff supervision is undertaken on a regular basis and again staff say is ongoing. Records were seen to confirm this. Residents’ welfare is promoted through regular maintenance and equipment safety checks within the home. At the last inspection there had been an issue regarding releasing of personal information to relatives. This has now been resolved, with the resident signing to confirm they are happy to share this information.
