As requirements had not been met we wrote a warning letter to the company. This was to tell them about the shortfalls we had found, what action was required to put these right, and that we would take enforcement action should they continue to fail to address these. We looked at those requirements that have now become due to be completed, and examined progress in addressing the requirements that will become due soon. Previously made recommendations were not examined as part of this inspection. The responsible individual was there on the day of the inspection and manages the home. He said he is going to apply to the commission to become the registered manager. Requirement 1 CARE PLANNING: Since the previous inspection new documentation had been put in place and the responsible individual was in the process of re writing care plans. We looked at the care plans for two people. They were very informative and detailed. Information was cross referenced from the assessments to care plans and risk assessments. For example one person, their nutritional was high risk with a score of 15. This person suffers from diabetes which is controlled by a special diet. Their weight is checked monthly and if there is a loss of more than a certain amount a referral to the GP and Dietitian is made. The Dietitian has visited the home and given advice to the staff. In another persons mental health assessment they have recently needed to see their GP. A referral has then been made to a Psychiatrist for their medication to be reviewed. The care plan specifies particular aspects of care intervention as the role of the Registered Mental Nurse. NOTIFICATIONS OF DEATH.ILLNESS AND OTHER EVENTS: There were 3 completed reg 37 forms in a folder, which had not been sent to CQC. They were relating to incidents between service users or service users to staff. In one instance an incident form was in place, however this had not been fully completed. There was a description of the incident but no further detail regarding who had been informed, what had been done, for example a risk assessment or updating the care plan. A care assistant witnessed the event but there was no statement from the staff member, which was required on the form. There was a further completed reg 37 and incident form found in the diary in the home which had not been sent to CQC. This requirement has not been met. Care Homes for Adults (18-65 years) Page 4 of 9 REQUIREMENTS AS TO HEALTH AND WELFARE: There were 3 completed forms, where the incidents should have been referred to safeguarding. One was about an incident regarding one service user trying to assault another. Staff had intervened. Another incident was in regard to a service user who was being assisted in the shower by a senior care assistant. When the staff member was washing the service users back he lashed out at her, hitting her face and knocking her to the ground causing grazing to her cheek. A further incident was related to a service user who was shouting during breakfast and demanded a cup of tea. When a senior care assistant walked over to help, the service user picked up another persons cup of tea and threw it at the staff member, just missing her. The RMN on duty said she had attempted to report this incident to safeguarding but an answer phone was on. Another incident had happened, however the information regarding this was found in the diary in the home. This was about an incident where a service user, twice punched another service user in the back. This requirement has not been met. STAFFING: We looked at the staffing rota and had some discussion with the Responsible Individual, Mr Hukerri. Since the previous inspection Mr Hukkeri has recruited RMNs a Cook and Care Assistants. There were sufficient regular staff to cover the hours needed without the use of agency staff. On some of the days there was an increase in the number of carers and Mr Hukerri said he wanted this to continue for escort duty to appointments and trips out for the service users. This requirement has been met. STAFF TRAINING: There was evidence to show that a training needs analysis had been carried out. Training was ongoing in COSHH, People Movement, and First Aid. All staff had undertaken training in Safeguarding, working through the training book provided by Redcar and Cleveland Council. This requirement has been met. QUALITY ASSURANCE: Surveys had been sent out to service users, relatives and other health professionals asking for their views on the home and the service provided. 10 out of the 21 surveys were completed and returned. Mr Hukerri then identified areas, which need addressing. This requirement has been met. Care Homes for Adults (18-65 years) Page 5 of 9 Improvements have been made to the environment.
