Name of Service: Conifers The quality rating for this care home is: The rating was made on: two star good service A quality rating is our assessment of how well a care home, agency or scheme is meeting the needs of the people who use it. We give a quality rating following a full review of the service. We call this review a ‘key’ inspection We do an annual service review when there has been no key inspection of the service in the last 12 months. The home is a large semi-detached Victorian house close to the centre of Trowbridge. Mr and Mrs Leonard live on the premises. Peoples accommodation is all single bedrooms and all are to the first floor. There are 2 large sitting rooms, a dining room, kitchen and bathroom with toilet to the ground floor. On the first floor there are the staff sleeping in room, a large bathroom and separate toilet. There is a large enclosed garden to the rear of the property. Currently Mr and Mrs Leonard provide the care and support for one person and no staff are employed. The last Key inspection was 18th December 2007. What did we do for this annual service review? We looked at all the information that we have received and asked for, since the last key inspection of 18th December 2007 and the Annual Service Review of 29th December 2009. We looked at their annual quality assurance assessment (AQAA); this is their own assessment of how they are doing, that focuses on how well outcomes are being met for people using the service. We looked at surveys returned to us by people using the service and from other people with an interest in the service. We looked at how the service has managed any complaints. We looked at what the service has told us about things that have happened in the service; notifications that are a legal requirement. We also looked at our last report on the service, relevant information from other organisations and what other people have told us about the service. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. Mrs Leonard told us about unexpected events in the family that had had an impact on plans; the persons holiday; re-decoration of the building and the amount of training that they had undertaken. She told us this year for example we have only had one holiday not the usual 2 and that circumstances meant some planned outings etc. did not happen or were changed. Mrs Leonard told us that these things would now be addressed as a priority. In the AQAA Mrs Leonard told us any planning, activities, changes in routine are based around the needs/wishes of that one client. There are annual reviews which all interested parties can attend and we have questionnaires for those who cannot attend. The current client is fortunate to have a weekend at home once a month and this also gives us a regular informal meeting with [their family] in which concerns, thoughts can be expressed and acted on if necessary. Mrs Leonard told us how the person expressed their likes and dislikes. Mrs Leonard told us: just as we would with a verbally able client we talk over any plans, activities with the client. We have a detailed knowledge of the client, [their] very personal language/communication and of [their] needs and as such can avoid situations which might upset. We have always believed that in order to make a choice a person has first to experience and then they can make an informed choice, so we always try to offer a variety of experiences, activities and food. The aim of Conifers has always been to build a service that suits the clients needs and choices, not the other way around and that deprivation of choice, no matter how small, is an infringement of personal rights just as is doing something for someone that they could do for themselves without a very good reason. We continue to look for ways encourage communication and independence. We have made minor changes to routine, menus and certain activities to meet persons change in daily routine and choice of diet. Mrs Leonard told us the client is supported in their own particular interests but are also encouraged to participate in all the day to day aspects of living be it shopping, laundry, outings etc. We continue to access the community at every opportunity and provide support so that client is involved be it going out for a haircut, collecting items Annual Service Review Page 3 of 5 from health centre or attending local events. We shop for and eat the same food and Peter [Mr Leonard] frequently tries new dishes, we do not have planned regular menus but ensure quality, freshness and diversity. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know what further improvements they need to make. The person who lives at the home always appears to be well supported and happy there. We received comments from a visiting healthcare professional. they told us Ann who is [the persons] full time carer is always patient with [the person] and is very attentive to [their] needs and tries to anticipate and interpret what [the person] needs by [their] behaviour as [the person] is unable to speak for [themselves]. [The person] seems to be happy and settled with Ann. The home continues to let us know about things that have happened since our last key inspection and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who use it. What are we going to do as a result of this annual service review? Subject to any changes in registration and inspection practice following implementation of the Health and Social Care Act 2008, our current plan is to do a key inspection by 17th December 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service. Annual Service Review Page 4 of 5 Reader Information
