Dudwell St Marys (New build) was registered with the Commission for Social Care inspection in May 2006 as a care home with nursing. The home was purpose built andAnnual Service Review The registration of a manager.is presented across three floors, with level access to all floors. It is registered to provide nursing care to up to 30 people who have a dementia type illness and 13 people who require general nursing care. The ground floor is designated for general nursing care, first floor to residents who have complex needs associated with dementia and the top floor to residents who have less complex needs associated with their dementia. The home is owned by Barchester Healthcare Ltd. It is located next door to another nursing home owned by the same organisation and is close to the village of Burwash. Residents accommodation consists of forty-three single bedrooms with the provision to convert some to shared accommodation if requested. All bedrooms provide en-suite facilities. Shared space consists of communal kitchenettes, lounges and dining rooms on each floor. The home is set in its own grounds with panoramic views over the nearby countryside. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received or asked for since the last key inspection on the 27 June 2008, where the rating of the home was 2 Stars -good. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service these are called notifications and are a legal requirement. Telephone contact was made with the home on the day of the Annual Service Review so as to clarify any changes to the service. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. What has this told us about the service? The AQAA was received 24/04/09 and contains clear relevant information that is supported by a wide range of evidence. The AQAA lets us know about changes they have made and where they still need to make improvements. It shows clearly how they are going to do this. The data section of the AQAA is accurately and fully completed. The home have comprehensive written information which is provided to all prospective people who may use the service. The documentation outlines the ethos of Memory lane Dudwell St Mary New Building and Barchester Health Care. The homes CSCI reports, the Service User Guide and Statement of Purpose are available in the reception area. The home actively encourage enquirers to look at other homes so an informed decision can be made. Admissions are not made to the home until a full needs assessment has been undertaken and the home have confirmed in writing that they can meet the identified needs. For people who are self-funding and without a care management assessment a skilled and experienced member of staff always undertakes an assessment. The assessment is conducted professionally and sensitively and involves the individual and their family or representative where appropriate. This includes offering an invitation to view the home to prospective resident and relatives. Trial visits are available. The home ensure that all the people who use the service have a plan of care for all aspects of daily living and long term outcomes based on the care management assessment and the use of assessment tools. The home review each care plan monthly and more frequently if it is required due to changes in general condition of the people who use the service or if the staff have identified any risk. Each care plan includes Annual Service Review Page 4 of 7 comprehensive risk assessments which are also reviewed regularly. The management of risk is positive in addressing safety issues while aiming for improved outcomes for the people who use the service. Where there are limitations the decisions have been made with the agreement of the person or their representative and are accurately recorded. The service respects the human rights of people using the service with fairness equality dignity respect and autonomy underpinning the care and support being provided. Care and comfort are given to people who are dying, their death is handled with dignity and propriety and their spiritual needs rites and functions are observed. Relatives are involved in planning for and dealing with increasing infirmity, terminal illness and death. The home have organised recreational programme throughout the week, the date and time of each activities is displayed in the communal area. The home invite the family to take part on birthdays and other festive celebrations. The home have identified that they would benefit from employing three activity co-ordinators to enable more one to one time with our frail elderly residents who are unable to attend group sessions and to provide activities at weekends. The home keeps a full record of complaints and this includes details of the investigation and any actions taken. Unless there are exceptional circumstances the service always responds within the agreed timescale. There has been 19 complaints made in the past year, all fully investigated and there has been two safeguarding alerts raised, one of which has yet to be concluded. All staff receive training to protect the people who use the service and a robust recruitment process also promotes the safety of the people who use the service. Staff members are trained and competent in health care matters. The home arranges training on health care topics that relate to the health care needs of the people who live in the home. Staffing levels are reviewed regularly against the needs of the people who use the service. The home has a good record of compliance with the receipt administration, safekeeping and disposal of controlled drugs. The staff adhere to the NMC standards of Administration of Medication. The home provides a physical environment that is appropriate to the specific needs of the people who live there. The well-maintained environment provides specialist aids and equipment to meet their needs in particular the dementia needs. The corridors have themes on the individual floors and contain memorabilia. The outside areas where the residents can spend time have been risk assessed and made safe for the people who use the service. The security of the home is paramount to the people who live there and key pads are in use. There is a continual programme of redecoration in place. Induction training is in line with the Skills for Care requirements. The manager has a clear understanding of the key principles and focus of the service based on clear values and priorities. There is a strong ethos of being open and transparent in all areas of running of the home. The manager is aware of current developments both nationally and by CQC and plans the service accordingly. The previous report was positive and the requirements made at that time are said to have Annual Service Review Page 5 of 7 been implemented. The home continues to let us know about things that have happened since our last key inspection through notifications to us, and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who use it. What are we going to do as a result of this annual service review? There will be no change to the inspection plan and will do a key inspection by 26/06/2010 However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
