Emsworth House is a local authority care home, under the management of Hampshire County Council. The home is registered to accommodate up to seventy-nine older people, providing long stay residential or nursing care for older people as well as those with a diagnosis of dementia. There are lounge/diners on each floor as well as small sitting areas in different parts of the home and there is a courtyard garden in the centre of the home. All bedrooms are single, the rooms in the nursing wing have en-suite facilities, but not those in the residential wing, there are sufficient bathrooms and lavatories on each floor. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people using the service and from other people with an interest in the service. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The results of the previous key inspection of the service. What has this told us about the service? The homes annual quality assurance assessment (AQAA) gave us all the information we asked for. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know some of the further improvements they need to make. We sent out survey questionnaires and received comments from four of the people who use the service, some of who were supported by relatives to complete the questionnaire. We also received comments from a health professional and a social care manager. All of the residents who completed our survey indicated that they received enough information to help them decide if the home was the right place for them, before they moved in. The majority indicated that they had been given written information about the homes terms and conditions, while one person said they had not. Four told us that they always receive the care and support they need and two said they usually do. Most people who completed our questionnaire said that staff members are usually available when they need them and one person said that staff are always available. Additional comments were that the home is occasionally short of staff and there are more staff needed. One persons relative added I have always found the staff to be very caring and helpful. A care manager commented that staffing levels are the main voiced concern from clients placed at Emsworth House or their families. There are a great number of highly dependent people there and my anxiety is that those less demanding have no alternative but to wait - sometimes for quite some time before receiving attention. The AQAA told us that dependency levels are reviewed regularly and when a vacancy arises, to ensure the home can meet the assessed needs. A departmental recruitment drive is planned to try to recruit to current ancillary vacancies. The home has reviewed and redistributed staffing hours to ensure all areas of the home receive basic cover. At the last inspection visit on 24/06/08 we found that the home had met two previous Annual Service Review Page 4 of 7 requirements and there were no new requirements identified. The AQAA told us that an activities co-ordinator has now been employed and there is an established activities programme on the nursing wing, which has resulted in the development of the sensory room. Three out of four people who returned questionnaires said that there are always activities in the home that they can take part in if they want. Another person said there usually is. One person said that they always like the meals, two said they usually do and one said they sometimes do. One person told us the evening meal could be better and another said there was not enough smoked haddock. The AQAA states there are plans to increase residents and relatives input with menus and recipes and that all kitchen staff are trained in nutrition and special diets. Additional items have been purchased to make the residential dining experience more homely. A sensory garden has been developed and there are further plans to introduce a residents gardening club. New care plan documentation is in place and there are plans to provide further inhouse training to increase staff skills in this area. A care manager told us that care plans are comprehensive and consideration is given to individual needs. The AQAA states that all staff have received additional equality and diversity training. The AQAA stated that in the last 12 months the home had received 12 complaints, 11 of which were resolved within 28 days. 5 of the complaints were upheld and 1 complaint was awaiting an outcome. The AQAA told us about plans for earlier intervention to avoid formal complaints, such as inviting family members to meet with a senior member of staff within two weeks of a new admission. As a result of listening to people who use the service, the home has introduced a system to minimise the possibility of items of clothing going missing. The AQAA also tells us that the home has increased the frequency of the service user questionnaire to make sure that the home is more proactive about issues that might arise. All those who took part in our questionnaire indicated that there is someone they can speak to informally if they are not happy and that they know how to make a formal complaint. A care manager and a health professional confirmed that the service has responded appropriately if they have raised any concerns. We received information from an anonymous source on 30/07/08, expressing concerns about the management of the home. We forwarded this information to the service provider, who investigated the matter and wrote to us to to tell us that no evidence was found to substantiate the concerns. The AQAA states there has been 1 safeguarding referral made and 3 investigations. We spoke with the registered manager who told us that the referral had not subsequently been taken up by Adult Services as a safeguarding issue and all three investigations had been carried out in line with the homes complaints procedures. The home has sent us notifications of any incidents affecting the welfare of people who use the service. Additional comments from people who use the service and their relatives included I am very happy with the care that my mother has. The staff are caring and always ready to help my mother. She seems very happy and at ease. Another relative told us that the home provides good care and does well at keeping me informed of any changes to relatives health. One person who lives in the home told us they look after everybody well. Another person who uses the service wrote Emsworth care home is exactly the best place for me to spend the rest of my days. In my room I have everything that I need. A health professional said that the service provides excellent care all round in my opinion. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
