It was an inspection of the arrangements for medicine handling in the home. We were assisted by the manager and deputy manager. The manager explained that they had recently agreed to a change of pharmacy. The new system would start the following week and would mean that the medicines would be supplied in a different packaging. This system would also be used for the people who manage their own medicines. The manager believed that the new packaging would make the medicines easier to audit including assisting staff when they check on people who look after their own medicines. The people who currently manage their own medicines had agreed to the change. We looked at the care plans for those people who manage their own medicines and saw that they had been assessed as able to manage and reviewed at two monthly intervals. The audits of medicines included where staff had checked the medicines of people who looked after their own. Medicine administration records were clear, where written additions had been made they were mostly signed by two members of staff, however we found some records where this had not been done. Some people were prescribed an anticoagulant medicine and , although the records were accurate and clear, the home was not following the guidance issued by the National Patient Safety Agency which we looked at during the inspection. We saw evidence of people exercising their choices about medicines in addition to managing their own medicines. Refusals were marked on one persons records and staff could clearly explain why that was the case and showed that the GP had been informed. Another person had requested that the home contact the GP to change some of their medicines. Medicines were all stored securely, however there were two concerns. The controlled drug cupboard must be secured to the wall with rag or rawl bolts to ensure that it complies with the legislation and the fridge had been turned off at the time of the inspection when there were medicines in it. The management of the home are pro actively improving the safety of medicines handling in the home whilst supporting people to maintain their Independence. What the care home does well: What they could do better: All staff must be made aware of the importance of maintaining the fridge at an appropriate temperature and the actions needed if the temperature increases.
