The pharmacist inspector from CQC, Suni Chotai visited the home to check that there are safe systems in place to ensure that people receive their medicines as prescribed by their doctor There had been a four medication errors in recent months Two of these medication errors have been reported to the local safegaurding authority and are being investigated by them At this Inspection we only looked at medication management systems We looked at medicine storage facilities and we watched three people being given their medicines at lunch time We looked at 28 residents medicine administration record [MAR] charts out of the 55 residents in the home We looked at three care plans specifically for information relating to treatment and medicines Regarding storage we noted that there were adeqaute facilities to manage medicine safely The service has three medicines trolleys to manage safe administration of medicine ensuring that the medicine round is split so that it is not too long a round allowing people to get their medicine at the prescribed times Storage facility is provided for controlled drugs which are medicine that require more secure storage by law Two medicine fridges are available to store medicine that needs to be kept cool According to the fridge temperature records kept by the home one of the fridges was below the required recomended range The staff recording this temperature did not take any corrective action despite there being clear guidance on the fridge door informing them of the correct temperature range required for cool storage of medicines Two nurses had check the controlled drugs stocks against the records the previous day and we spot checked two items and found records to be accurate We observed a nurse giving medicines at lunch time follow good practice She observed good hygeine procedure and had a good manner with the residents We looked MAR charts for the previous cycle as the current cycle was on day five of the cycle and we would not have enough information to look at The records were completed and information such as next blood test, next due date for an injection or reason for omitting any medicine were clearly recorded However there was a discrepancy between the number of available doses against the number of signatures seen on the MAR for three medicines This means that from the records we cannot be certain that people received their medicines as prescribed There were no care plans on medicine prescribed to be taken only when needed This is required for a person centred approach and for consistency We noted that for two residents managing their own medicines as stated in the care plan there was no risk assessment or a person specific procedure on the management of that medicine As one of the errors was around leaving medicines for a resident to take later we discussed with the staff how this could be managed as a partial self administration procedure under a risk assessment and clear detailed care plan We were informed that since the errors in medicine the home had put on two refresher
