What the care home does well Kingswood provides a caring, supportive and loving family home. This provides the person using the service security, love and support thus maintaining his own personal autonomy. The person living in the home said he could not imagine living anywhere else and considered the manager to be his mother. The GP commented that the home provides the care and support that the person using the service needs. What has improved since the last inspection? No requirements were made at the last inspection. What the care home could do better: Mrs Ludlow needs to maintain the loving and caring environment that the family has developed over the years. We carried out this inspection as an unannounced visit in the presence of Mrs. Ludlow and the person using the service. Kingswood is a family home, which only registered as a care home as North Somerset does not have an adult placement scheme. The person using the service states that he would prefer to be under an adult placement scheme as he feels the registration and inspection process is an ‘infringement of my privacy and rights to live in the home of my choice with my own family.’ To support our judgment as well as visiting the home, we asked the person using the service to complete a questionnaire, the manager completed an Annual Assessment Of Quality and we received comments from a GP and community services. The individual person living in Kingswood has all the information he requires to support his decision to remain in the home. EVIDENCE: Standards 2, 3 and 4 do not apply, as the Homeowner does not intend to take other people into Kingswood. The person using the service has his own Statement Of Purpose And Service User Guide however he feels that they are inappropriate, as adults living at home with parents do not have these documents. As at previous inspections we discussed his awareness of his rights and responsibilities. These are included in a contract between him and the homeowner. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 8, 9 and 10 Quality in this outcome area is excellent This judgement has been made using available evidence including a visit to this service. Appropriate support is given to enable the person using the service to make personal choices in his style of living, through an agreed and risk assessed plan of care. EVIDENCE: As in previous inspections we did discuss individual care plans and risk assessments. The person using the service still feels quite strongly that care plans are not appropriate to his placement but he understands the need for agreed risk assessments. It has been agreed that a daily record is not required however his foster parent will record any significant events in his life such as hospital, doctor and dentist visits. The person using the service makes his own decisions about how he wishes to lead his life and this is supported and enabled by the family. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The person using the service receives appropriate support to develop his own personal skills and to take part in the community. The family meals provide a healthy and nutritional diet. EVIDENCE: As at previous inspections we discussed the future and the person using the service stated that he hopes to go to college to study IT. Both the person using the service and Mrs. Ludlow said they would be going to sign up for a computer course in the New Year. The person using the service has forged links with the community in Worle and joins in family holidays and activities. On his birthday he went for a trip on a micro light and he thoroughly enjoyed his day out. They have planned to go to Australia for next Christmas. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is excellent This judgement has been made using available evidence including a visit to this service. Both physical and emotional support is given as needed. The person using the service handles his own medicine on a risk assessed basis and the subject of ageing of both himself and his foster parent is openly discussed and agreed with him. EVIDENCE: Kingswood does not provide nursing care. The person using the service has his own room and front door key; he stated that he has purposely and often closed his own room in the evenings. Records kept showed that the homeowner enables the person using the service to attend doctor and dental appointments as required. He has also been supported in dealing with bereavement issues. The person using the service handles his own medication; this process has been risk assessed and lockable storage is available in his room. The person using the service and the manager had discussed his future as and when both become older or if illness should become an event. The views and opinion of the person using the service are listened to and he is protected from abuse, self-neglect and harm. EVIDENCE: The person using the service stated that he was aware of the process he could follow if he wanted to complain about the care provided by his foster parent. He has a copy of the CSCI telephone number and contact details for his social worker who he felt he could talk to if he ever needed to complain. He stated that he always felt he was listened to and took part in making important decisions in the family home. The manager of the home has access to policies and procedures regarding safeguarding adults and they are also aware of the North Somerset policies and procedures. Kingswood is a private home and reflects this. Aids and adaptations can be provided if needed. EVIDENCE: Kingswood is homely and comfortably furnished. The person using the service has access to all areas of the family home and has his own room with his own possessions. As at previous inspections we discussed his personal space and possessions, which is he is very proud of. The home is always very clean and tidy and reflects regular family life. The person using the service proudly states that he helps with household chores, cooking and maintaining the garden. All areas in the home are accessible by wheelchair including the garden. Kingswood is run with the best interests of the person using the service in mind. Policies and procedures are in place and reviewed. Health and Safety is satisfactory. EVIDENCE: Kingswood is not managed as a care home; it is a family home and is run as such. Everybody has a say in the day-to-day running of the home. The person using the service confirmed that he contributes to decisions made in the home as much as other family members. He also confirmed that he has access to his own money and is supported in maintaining his own budget. Kingswood does have policies and procedures that have been adapted from a care home previously owned by the manager. However the person using the service feels they are inappropriate to a family home.
