If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Up to 41 older people can make their home in Littlebourne House (the Service). The premises are an older property that has been modernised and extended. The accommodation is arranged on the ground floor and the first floor. A stair-lift connects these two levels. In the main building, there are 33 bedrooms. Three of these can be shared by two people each. All of them have a private toilet and wash hand basin. Most of them also have a private bath or shower. Also in the main property, there is a Annual Service Review Page 2 of 7 self contained flat which can be used by visitors if they have had to travel a long distance. It can also be used as another shared bedroom. In a building that is separate from but near to the main house, there are some newly built flats. These have a bedroom, lounge, bathroom and kitchenette. They are designed to be used by people who want the security of being able to call upon assistance, while at the same time wanting to keep most of their independence. At the moment, another building on the site is being converted. When finished, it will have 14 bedrooms and a day centre. Each of the bedrooms will have a private wash hand basin, toilet and walk in shower. All of the flats and the new bedrooms are connected by the call bell system to the main house. The Service is set back from the main high street of Littlebourne. This is a small village that is about five miles from Canterbury. It has a small selection of shops. The Service has a large, attractive garden at the back. There are various garden seats so that people can relax and enjoy their surroundings. A large all-weather conservatory looks out onto the garden. To the side of the main house, there is room for relatives to park their cars. The range of fees charged currently for each person to live in Littlebourne House runs from £525.00 to £650.00 per week. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last Key Inspection. This included: The Annual Quality Assurance Assessment (the Assessment) that was sent to us by the Registered Providers. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the Service. It also gives us some numerical information about the Service. Information that the Registered Providers have recently told us about how things are going in the Service. What the Registered Providers have told us about things that have happened in the Service. These are called notifications and are a legal requirement. The previous Key Inspection that was completed on 19 August 2008. What people who use the Service and members of staff have said in the questionnaires that they have kindly completed for us. What has this told us about the service? The Assessment gives us lots of useful information. A number of improvements have been made since our last Key Inspection. These include things that make a direct difference to peoples everyday lives. For example, the Service now has its own minibus. It is equipped with a special tail lift so that it is easier for people to get in. Another thing has been the introduction of various clubs that people can choose to take part in. These include subjects such as gardening and knitting. A further improvement is the introduction of a newsletter that keeps people up to date with what social activities will be coming up in the near future. Other improvements are planned for the forthcoming year. These include finishing off a new all weather walk-way that will make it easier for people who want to enjoy the garden. Another new facility will be the day centre. It is hoped that some of the people who live in the Service will want to use it to have a change of setting and to meet people who come in from the local community. A further planned development is the installation of a new call bell system. This should make it easier for members of staff to make doubly sure that people get a quick response when they ask for help. When we did our last Key Inspection we thought that things in general were going well for the people who use the Service. Therefore, there was no need for us to require that any particular improvements be made. However, we did agree with the Registered Providers that they would work on a limited number of improvements. Each person who lives in the Service has a written individual plan of care. This lists what help they need and the best way that this can be done. We thought that people needed to be given the chance to be more involved in the review and updating of these plans. This Annual Service Review Page 4 of 7 has since been done. Another thing was the need for the Registered Providers to get two more references for a care worker as part of the general security checks that are completed for new members of staff. The necessary references are now in place. In their answers to our questionniares, people who live in the Service tell us that they are happy there. They are pleased with the help they receive. They think that they have good quality meals. One person summarises the general mood when they say, the atmophere is friendly and efforts are made to make residents happy and comfortable. The kitchen is very efficient and service at meal times is very good. The Assessment says that people are regulalry asked what they think about how things are going. This includes people filling out questionnaires if they want to. They can also choose to take part in residents meetings. The Registered Providers say that they are confident from what people are saying that they remain happy with their home. People are supported to stay healthy and they are helped to seek medical attention as and when its needed. In our questionnaires for people who live in the Service, we ask about the medical attention they receive. Everyone says that they always get the medical assistance they need. Sensible arrangements are in place to assist people to take medicines in the manner intended by their doctors. These include the safe storage of medicines. Also, there are systems to ensure that medicines are given to the right person at the right time. The Registered Providers have kept us informed about significant events that involve the people who live in the Service. There has not been that much. Since our last Key Inspection, the Registered Providers have not received any formal complaints. There are arrangements to ensure that all complaints received will be looked into promptly. There are at least four care workers on duty at all times during the day and the evening. They are supported by other people who work in the kitchen or who do the housework. There is a waking staff presence at night. Also, there are special security arrangements at night. In our questionnaire for members of staff we ask, are there enough staff to meet the individual needs of all the people who use the Service? Everyone says that there usually are enough staff. The Registered Providers say that suitable security checks have been done on all members of staff who have been appointed since our last Key Inspection. This is to help make sure that they are trustworthy people. Sensible things are done to help keep people safe. The Assessment says that good standards of hygiene are achieved in the kitchen. The premises are fitted with an automatic fire detection system. The Registered Providers say that the fire safety equipment and other things such as gas appliances are regularly checked. They say that they remain in good working order. What are we going to do as a result of this annual service review? Our overall judgement is that the Service continues to provide a good quality of care for the people who have made Littlebourne House their home. Annual Service Review Page 5 of 7 We are not going to change our inspection plan and we are going to complete a Key Inspection by 18 August 2010. However, we can inspect the Service at any time, if we have concerns about the quality of the Service or if we have concerns about the safety of the people using the Service.
