Mariantonia House was first registered as a care home in 1996 for five people and there was a phased development to achieve its current registration. The home now provides accommodation and personal care for 13 adults who must require care primarily due to learning disabilities. Residents may have additional needs relating to their mental health, although the home will not accept anyone with violent or aggressive behaviours. The service`s philosophy is "to provide 24-hour supervision andAnnual Service Review 22008a safe haven, which gives protection and support". The home is owned and managed by Antonio Magro and Emanuela Magro who are both personally involved with the dayto-day care provision. Mrs Magro is the registered manager. The home is located on a main road within a reasonable walking distance of Kidderminster town centre. There are also local shops and other facilities, which include a railway station. The property is Victorian (with a listed status) and is detached. It has a lawned garden area, parking spaces and two patios. The main house occupies a large corner site that has been extended to provide more bedrooms and facilities. There are eleven single bedrooms with en-suite toilet and shower facilities and one shared bedroom with a separate bathroom. Eight bedrooms are situated on the ground floor. There are also several toilets and bathrooms, two sitting rooms, two kitchens, a laundry and a dining room for everyone to share. Information about the service is provided in a statement of purpose and service users` guide documents (the guide is also in a user-friendly format), which are available from the home. The weekly fee is dependent on individual residents` assessed needs, as agreed between the home and their funding authority. This charge covers everything provided by the service except such as newspapers, hairdressing, chiropody, opticians, personal clothing and effects, dry cleaning and the cost of holiday accomodation.Annual Service Review

Annual Service Review Page 3 of 8 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. What has this told us about the service? The registered manager of Mariantonia House sent us their annual quality assurance assessment (AQAA). It was clear and gave us all the information we asked for together with real practice examples about the services offered. The manager is making sure the services offered to people at Mariantonia House are firmly based upon each persons own likes and dislikes in how they wish to live their lives. This is generally known as taking a person centred approach and within the AQAA the manager has demonstrated that they are striving to ensure this is translated into practice. Some of the examples given are:- anything which is to be used in care plans or life books, concerning person centred care is also produced in symbolised format for those requiring it - we have given all service users the opportunity to have a say in how the home makes plans for day trips, holidays, shopping, celebrations, housekeeping etc. We will be giving them the choice of colour schemes when decorating individual rooms - have opportunities to plan menus, devise shopping lists, cook meals and bake. Some are able to write letters to family and friends, they are encouraged to go out and post it themselves - each person has a key worker who has a close and understanding relationship with the person which helps with continuity of care and support. The AQAA also reflects what the home could do better and includes details of the improvements they have made in the last twelve months together with their ongoing plans for the future. For example, at the last Key Inspection it was reported that some health and social care professionals felt that opportunities to develop peoples life skills were limited. In the AQAA it confirms, We have secured some voluntary work in a charity shop for two service users. Five of our service users enjoy working for Beacon Employment, also on a voluntary basis. Two service users have worked in charity shops for many years also on a voluntary basis. We have secured placements for two service users to help with the animals at Top Barn Farm, Worcester, and one other service user is working three full days at a local Cattery. The manager has shown in their AQAA that they are motivated to meet individuals needs in relation to gaining life Annual Service Review Page 4 of 8 skills. Although the manager confirms that this can be difficult to achieve due to courses not being available, long waiting lists and voluntary work is often limited for people who may want to attend on a regular basis. Also the AQAA tells us that all people who live in the home have their choices respected and are not pressurised into pursing activities they do not wish to do. Examples are provided in the AQAA of the daily life activities individuals are supported to experience within their home:- meal preparations, laying tables and clearing away - sweeping, mopping and emptying bins - people change their own beds, polish and vacuum their room - some people are able to write letters to family and friends and are encouraged to post these - fine needlework, nail art, crosswords, shopping and computers. Therefore this shows that people who live in this home are being supported in ways that suit them and they are encouraged to lead active lives that hold meaning to each person. The AQAA tells us that each person has an, Health Action Plan (HAP) which is used regularly and updated by all those involved in health care. The manager confirms, All residents have been given the opportunity to see practitioners of their choice and are free to access all NHS healthcare facilities in the local community. There are clear examples given which show that the manager and staff are being creative in their thinking when faced with issues such as peoples weight. In doing this examples are given which reflect a proactive approach to a sensitive area of peoples lives, such as, using dance, Thai Chi and promoting the benefits of physical activity. This shows that the manager is determined to promote peoples health and wellbeing. We are told in the AQAA that staff have received recent training in medication which should ensure that staff practices are protecting people in this area. We have also noted that the manager and staff have been able to recognise when people have mental health needs that mean they are unable to take their own medications without assistance from staff to ensure people remain safe and well. The AQAA told us, The service users benefit from the Home being beautifully decorated throughout as well as having a relaxed, open, friendly and happy atmosphere. There is a stable and consistent maintenance programme in place for redecorating and general repairs. The AQAA gave us further information about how the home is going to be improved in the next twelve months which include, a conservatory which will assist in facilitating activities, such as, pottery sessions. Also has already stated people are able to use their kitchen as they choose and take part in some of the staff training which is linked to living in their home, such as, fire safety where they recieved certificates, which is a nice touch. This shows that people are fully involved in aspects of their home and the manager ensures that it meets each persons needs. Annual Service Review Page 5 of 8 We are told that, Every member of staff is employed following a thorough recruitment procedure and in the last twelve months improvements have meant that all staff are checked to ensure that they are safe to work with vulnerable people who live in the home. The manager has shown that they are striving to improve the knowledge of staff who work at the home in the areas of mental health and behaviours that may require some support. This is positive as at the last inspection it was noted that staff need more opportunity as well to access training on specialist needs such as mental health and managing behaviours. Also the manager has told us that staff supervision sessions are being completed regularly which will assist in identifying staff training and areas for development. Seven staff members have achieved Level Two in the National Vocational Qualification, (NVQ). Other training that has been completed in the last twelve months, all full time staff have completed Level 2 Dementia training. One staff member midway through NVQ level 3 in care, and one other has completed NVQ Level 3 in Care. The Care Manager has finally received NVQ Level 4 (Registered Managers Award) and has now also completed her NVQ Level 4 in Health and Social Care. Certificates have been received and are displayed accordingly. All staff have also completed some mandatory training; Medication Administration, POVA, Food Hygiene. We will shortly begin further twelve week courses with NEW College, Worcester, these are Health and Safety, and Infection Control. Fire Safety training is December (09 and will also include all service users) and we continue with our Mental Health training. This means that peole living in the home can be reassured that they are supported and assisted by knowledgeable and skilled staff who have completed core training to meet their needs. There is a robust complaints process, and the manager tells us that they take all concerns and complaints seriously. There have been no complaints made to the service or to us in the last 12 months. The AQAA informs us that information in relation to the organisations complaints procedure is in formats that provide understanding to people who live in this home. There are safeguarding policies in place which ensure that management and staff are fully aware of their responsibilities in relation to the protection of vulnerable people. The AQAA informs us that staff have received training in the Protection of Vulnerable Adults (POVA) as stated earlier. This shows that this training is taken very seriously by the manager so that people are supported and assisted by staff that can recognise abuse, know how to keep people safe and who to report any form of abuse. There is information in the AQAA confirms that all full time staff have completed their training in Deprivation of Liberty Safeguards (DOLS). This is positive as staff should now have the knowledge to ensure peoples rights are not compromised and any unnecessary restrictions are not put in place. Also importantly some people living in this home would need support to communicate their choices and take part in making decisions about their lives. We looked at all the information we had concerning Mariantonia House, and our judgement is that they are still providiing a good service and that they have a clear vision of what further improvements they wish to make. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
