We call this review a ‘key’ inspection We do an annual service review when there has been no key inspection of the service in the last 12 months. The premises are a detached two storey property that has been purpose built. There are two passenger lifts that give step-free access around the building.
Annual Service Review Page 2 of 7 We have registered the Manager in her post. Everyone has their own bedroom. All of the bedrooms have a private wash hand basin and toilet. Most of them also have their own shower. There are various shared use spaces, such as the lounges and the bathrooms. Theres a call bell system. This has points in the bedrooms so that people can alert members of staff if they need help. The Service has got its own quite large grounds. This has got a patio area where there are tables and chairs. The Service is not within easy walking distance of the shops. Broadstairs town centre is about one mile away. The Service has got its own transport. The Registered Provider is a charitable organisation. It operates a number of similar services. The weekly fee for living in the Service for people who just need personal care runs from £540.00 to £595.00. For nursing care the fee runs from £730.00 to £840.00. What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last Key Inspection. This included: The Annual Quality Assurance Assessment (the Assessment) that was sent to us by the Registered Provider. Its a self assessment that focuses on how well outcomes are being met for people using the Service. It also gives us some numerical information about the Service. Information that the Manager has recently told us about how things are going in the Service. What the Registered Provider has told us about things that have happened in the Service. These are called notifications and are a legal requirement. The previous Key Inspection that we completed on 30 May 2008. What people who use the Service and members of staff have said in the questionnaires that they have kindly completed for us. What has this told us about the service? The Registered Provider sent us its Assessment. Its very detailed and it gives us the information we asked for. The Assessment and what the Manager has told us, show that various improvements have been made since the last Key Inspection. These include things that make a direct difference to peoples everyday lives. For example, people who live in the Service now have their own copy of the individual plans of nursing and care that well tell you about in more detail below. Another thing is that more permanent members of staff have been appointed. This has been done to reduce the need to use members of staff who are supplied by an outside agency. More has been done to actively involve the people who live in the Service in decisions that are made about their home. For example, theyve been asked their views about some new carpets that have been laid in some public spaces. Another example, is that people who are interested now sit on a committee that administers the amenity fund. Other improvements are planned for the forthcoming year. These include joining a recognised scheme that aims to develop the quality of nursing care that people can be given when they are nearing the end of their lives. Another development will see the Service joining a scheme run by the local health authority that focuses upon reducing the occurrence of infections. A third thing to be done will entail more attention to being given to support people to follow a healthy diet. At the previous Key Inspection, the Registered Provider wasnt required to undertake any particular improvements. We thought that things were going well. We did recommend that more time be made available for senior members of staff. We wanted them to have enough time to do things like supervising and generally supporting other members of the staff team. The Annual Service Review Page 4 of 7 Manager says that this has now been done and that all of the nurses and the care workers are having the support they need. The Manager says that she regularly asks the people who live in the Service how they think that things are going. She says that shes confident that people who live in the Service are happy with their home. She says that theyre pleased with the nursing care and with the general help they receive. Also, she says that people tell her that they have good quality meals. In the questionnaires that people who use the Service sent back to us, theyre saying the same sorts of things. The Assessment says that people are regularly asked what they think about how things are going. There are house meetings where people who use the Service can raise any point they want to. There are also annual questionnaires. The results of these are made available in the Service. Were pleased to see evidence that when an improvement is suggested by someone, the Manager is making sure that its being acted upon. People are supported to stay healthy. This includes medical attention promptly being sought as and when its necessary. Sensible arrangements are in place to assist people to take medicines in the manner intended by their doctors. These include the safe storage of medicines. Also, there are systems to ensure that medicines are given to the right person at the right time. Some people handle their own medicines. The Manager and the nurses have carefully considered the best way to help this to be done safely and reliably. When we were last in the Service, we thought that the accommodation was presented to an excellent standard. We understand that this is still the case. There are various things to help people who have difficulty getting about. For example, there are special baths that you can step into. There are also wet rooms. These are shower areas that dont have a tray and so theres no step. There are also hoists that can be used in the bedrooms. The Registered Provider has kept us informed about significant events that involve the people who live in the Service. There hasnt been anything out of the ordinary. Since our last inspection, the Registered Provider has received four formal complaints. These have been about various things. One was from someone who thought that they should not have had to use hospital transport to attend a hospital appointment. They thought that the Service should have offered to take them using its own transport. Another one was from a relative of someone who had to move to another service because they needed special assistance. The Manager says that each of the complaints was looked into carefully. She also says that all of them were sorted out so that everyone felt okay about the outcomes. Theres always a nurse on duty. Shes assisted by a team of care workers. Theres a waking staff presence at night. Also, there are special security arrangements at night. Members of staff say that there are enough nurses and care workers on duty. The Manager says that suitable security checks continue to be done on new members of staff. This is to help make sure that they are trustworthy people. She also says that nurses and care workers continue to have the training they need and that they know what theyre doing.
Annual Service Review Page 5 of 7 Sensible things are done to help keep people safe. The Assessment says that good standards of hygiene are achieved in the kitchen. The premises are fitted with an automated fire detection system. This provides a high level of protection. The Manager says that the fire safety equipment and other things such as gas appliances are regularly checked. She says that they remain in good working order. Our overall judgement is that the Service continues to provide a very good quality of care for the people who have made Maurice House their home. What are we going to do as a result of this annual service review? We are not going to change our inspection plan and we are going to complete a Key Inspection by 29 May 2010. However, we can inspect the Service at any time, if we have concerns about the quality of the Service or about the safety of the people using the Service.
