Offmore Farm House is a care home providing personal care and accommodation for twenty-one older people of either sex who may also have a physical disability and /or a dementia type illness. The home is a converted Georgian building with two extensions. Residents` accommodation is provided on the ground and first floor. Accommodation is provided in fifteen single and three double bedrooms. One single room has been converted to single occupancy since the change of ownership in line with the National Minimum standards. Six rooms have en suite facilities. Communal rooms consist of three lounge areas and a dining room on the ground floor. The home provides a range of equipment for residents` with physical disabilities and handrails are appropriately fitted through out. Toilets, and bathrooms fitted with hoists are available on both floors and are conveniently provided across the home. A shaft lift is available for movement between floors. The top floor is used as office, staff room and meeting space. The home is on the edge of a residential estate close to local amenities. It is set in an attractive secluded garden. What the care home does well The home is very well presented to prospective clients both in general appearance and in the excellent sources of information provided to enable prospective service users to decide if the home will suit them. Arrangements for pre-admission visits and trial periods are seen by service users and staff as very helpful to settling in to the home. The home has very good assessment procedures and care planning systems in place and service users are confident that their personal needs are understood and can be met. The staff are seen to have excellent relationships with the residents interacting naturally, with empathy and delivering care as the person wishes, achieving a high level of satisfaction and sense of wellbeing amongst the residents. The property is well maintained and kept in good decorative order and in a clean odour free state. The home demonstrates a good commitment to staff training and provide for appropriate levels of formal supervision. What has improved since the last inspection? This is the first inspection of this home since the change of ownership. The proprietor has taken on previously identified requirements and address each of these. What the care home could do better: The home is achieving very good standards overall and have a clear plan for further development. As part of this the home should provide written confirmation to the service user that needs assessed pre-admission can be met by the home. Care planning is of a high standard, in seeking continued development the home should introduce care plans for short term conditions such as infections, and plans that guide staff when as required medications should be given. Service users with conditions such as diabetes and epilepsy should have contingency plans prepared to guide staff on what to expect/see should the conditions become unstable and how to respond. The home has good medication storage facility and this is kept secure, the ambient temperature of the room was satisfactory during this visit, it is recommended that the temperature be regularly monitored to ensure it is remaining within required levels at all times. This key unannounced inspection visit was undertaken by an Inspector from the Commission for Social Care Inspection using the following information: reports from the organisation relating to the conduct of the home, records maintained at the home, the annual quality assurance self assessment, comment card responses from service users and relatives and reports by other agencies. The inspection involved a full tour of the property including, a number of bedrooms, the communal rooms and service areas and provided an opportunity to speak with many service users. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 1 – 5. Quality in this outcome area is good. The homes statement of purpose and service user guide are good sources of information providing details of the service enabling service users and families to make informed decisions about admission to the home. Pre-admission assessments are undertaken by the most experienced staff and confirmation is given to the service users that their needs can be met by the home and further confirmed by contract at the time of admission. Service users are invited to visit and trial the home before committing themselves to staying at the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Copies of the recently revised Statement of purpose and Service User Guide were provided, these are issued to service users and fully meet the requirements of the regulation and schedule. The standard form of contract is that prepared by the Federation of Care Home Proprietors. Offmore Residential Care Home for the Elderly DS0000069706.V350604.R01.S.doc Version 5.2 Page 9 This is comprehensive and details requirements of the standard with the exception of stating the rooms to be occupied, the manager adds this at the commencement of the agreement. A copy of the terms and conditions are maintained on the case file. Needs assessment, 4 files were selected to be case tracked, including a recent admission, an established person, a person with additional complex need and an older person The case files were seen to have extensive assessments thoroughly completed, informative and subject to monthly reviews. Risk assessments were undertaken for moving and handling, Nutrition, Falls and pressure risk. Following admission a four week review is undertaken by the Manager with the service user and family, where applicable a Social Worker has also reviewed the placement. Monitoring charts seen and completed up to date. Weight monitoring, meals taken and personal care such as bathing are recorded daily. Wound care is undertaken by the District Nurse. Bowel records are only kept if a concern arises. Night staff maintain record charts of their attendance and checks. Currently no meeting needs letter is raised confirmation given by word of mouth following pre-admission assessment. Contracts with the Local Authority and the Community Care Assessment and Referral were seen on file. A service user with complex needs had allergies recorded at assessment and shown on MAR Chart a newly written assessment in September and monthly reviews October and November. Catheter care is provided by the District Nurse and care staff are responsible to fit the disposable night bag to day bag and to monitor output. Disposed of in clinical waste is undertaken by contract with PHS. Very good diabetes and catheter care assessments and plans were seen. A service user with diagnosed infection and treatment had no supporting assessment or care plan, otherwise assessments and risks very good. Trial visits were confirmed with one prospective service user currently in process of visits. No intermediate care service offered. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7 – 10. Quality in this outcome area is good. Care plans are derived from a comprehensive range of assessments and provide the basis for the delivery of care and detail the actions required of staff to meet the identified needs. Health care needs of service users are fully met. Medications are well managed all facilitating the promotion of service users health. Service users are treated with respect and their privacy upheld. This judgement has been made using available evidence including a visit to this service. EVIDENCE: A sample of five service users were selected for case tracking including one recently admitted service user and one with more complex needs. Care planning documentation is well organised, current, clearly written and comprehensively encompassed the range of ‘care areas’ necessary to ensure the delivery of care appropriate to the general needs of each service user. A service user with well-controlled diabetes managed by the District Nursing service had no contingency plan to enable staff to identify signs and symptoms requiring medical or nursing intervention should they occur. Offmore Residential Care Home for the Elderly DS0000069706.V350604.R01.S.doc Version 5.2 Page 11 Another with epilepsy well controlled on medication had no contingency plan describing status and giving clear direction to staff. A service user who had received treatment for an infection had no short term care plan although the daily records were comprehensive. Overall care planning is good and subject to monthly reviews. The files evidence that Residents are closely involved in decisions regarding their care plan development and lifestyle some spoken with were able to confirm this, the lady recently admitted said that she was able and assisted by staff to make her own choices and had been involved in identifying what she required assistance with. All service users are registered with a GP and allied medical services such as dentist, ophthalmic and chiropody are provided on a regular and as required basis. Service users are assessed by senior staff in such areas as pressure relief and continence equipment and referred as required for professional advice and supply. Nutrition and falls are risk assessed and necessary interventions and monitoring arrangements are in place including meals intake and weight monitoring, plans to minimise the risk of falls. Medication using the Venalink monitored dosage system was seen to be well managed. Storage and records were acceptable and medicines were only dispensed by staff who have undertaken accredited training. The previous practice of District Nurses training and authorising staff to give insulin has been discontinued. Currently no controlled drugs are prescribed but facility is available for storage and recording. Inspection of the medication administration charts show them to be thoroughly completed. Keys are held by the senior on duty and medicines are kept securely. Records of booking in and returns were seen, the medication fridge is secure and the temperature recorded daily, no recordings are currently made of the ambient temperature of the medicines room. A number of service users receive ‘as required medication’, care plans need to be developed identifying the circumstances when they may be required. Staff were observed to have an easy courteous relationship with service users and visitors that creates a relaxed and friendly atmosphere, a relative said the staff are ‘always welcoming and relate well to them’. A telephone is available for service users to make and receive calls in the hall and the dining room as they choose. Screens were available for use in double bedrooms and all bedrooms were fitted with approved door locks that provided privacy and safety. However some en suite facilities were fitted with curtains rather than doors with locks, progress to fitting doors is underway but is delayed by design difficulties and each door requiring to be individually made. Personal mail was retained for residents to open with the assistance of their relatives. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12 – 15. Quality in this outcome area is good. The home provides a varied social and recreational activity programme that provides interest and pleasure for service users. The involvement of family and friends is encouraged in agreement with the service users wishes. Staff support service users to access opportunities for their personal development and health promotion. Meals at the home are wholesome and meet the nutritional needs of service users while providing for choice and personal taste. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Weekly activities programme displayed on SU notice board in hall. This morning activities included making a Christmas cake and a game of bingo. Service users were seen to occupy all of the communal areas. One lady not participating in the bingo said she preferred to sit and watch the squirrels and birds, which are abundant. She said that she was comfortable making a free choice on how to pass her time. Another service user said they liked to listen to classical music either in own room or the small lounge. Observation and discussion with Management and other service users show that routines are flexible to suit the needs of individuals. Offmore Residential Care Home for the Elderly DS0000069706.V350604.R01.S.doc Version 5.2 Page 13 A Priest visits regularly on Tuesday mornings and the CE any day, communion is offered. A Christmas service is planned for 15/12. Visitors welcome at any time and were seen and spoken with during the afternoon. The home has no involvement with service users financial affairs and no money held, a number retain a small amount of cash for themselves. Hairdresser bills are sent direct to family, planned private chiropody is paid by home and family billed. Case files include statement of access to files and signatures seen of participation in development and reviews of care plans. Lunch was served that had choices for both the main course and the desert, a number commented that the shepherds pie was very tasty and the bread and butter pudding delicious. Appropriate dining aids were being used and staff were assisting where necessary. Residents appeared to be enjoying the meal as a social event. The 4 week menu includes choices at all meals and includes supper. No cooked breakfasts currently, said to be service user choice, menu contents are included at service user meetings, fresh fruit is served each afternoon. Special diets of soft, low fat and low sugar are regularly provided. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 16 – 18 Quality in this outcome area is good. The home complaints and protection policies are robust providing a safe environment in which service users feel they can voice concerns and that these will be listened and responded to. Legal rights are protected by efficient administration. Staff demonstrate excellent knowledge and understanding of adult protection issues which contributes to an environment that is safe from abuse. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The Statement of Purpose and Service Users’ Guide held copies of the complaints procedure and a copy of the procedure was also displayed in the entrance of the home. These documents were available in every bedroom visited. Complaints policy viewed, fully compliant. No complaints since change of ownership. Discussion with the most recently admitted service user identified that she new of the procedure and was confident that she could speak to any of the staff if she had concerns. The electoral roll has been completed for this year and all have requested postal votes. Policies relating to the protection of Residents from abuse were observed to be in place and readily accessible, these included, ‘Whistle Blowing’, ‘Abuse Awareness’ and ‘Adult Protection’. Speaking with individual staff it was apparent that they were conversant with adult protection. No Secrets guidance document and the Worcestershire joint policy document were available. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 & 26 Quality in this outcome area is good. The Home provides a comfortable, attractive, safe and ‘homely’ place to live. Specialist equipment, consistent with the needs of the Residents and the demands of tasks carried out by care staff, is available to facilitate the provision of care. The home is clean, hygienic and free from odours. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The external areas were well maintained appropriate to season and accessible to residents. Tree management has taken place bringing improved light to rooms and better outlook on views. A tour of the building including an inspection of most bedrooms and were found to be mostly nicely personalised and included some items of own furniture. Room 15 has been adapted as a single en suite room overall reducing the number of beds to 21. Furniture in lounge and dining areas are of good order and present a ‘domestic’ ambience. The handyman shared his monitoring records and demonstrated that maintenance and repairs were swiftly attended to. Servicing and safety checks were programmed into his work routine. The Home has a full range of maintenance contracts in place, and an on-going refurbishment/redecoration programme. Decoration was all in good order a programme of carpet replacement has recently been completed. Infection control measures were in place. It was observed that liquid soap and disposable towels were appropriately placed and staff confirmed that they always had personal protective equipment available to use. Clinical waste facilities were provided. The laundry was maintained in a clean, tidy manner. Equipment was appropriate and met the needs of the home. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27 – 30 Quality in this outcome area is good. The home has a stable, well-motivated and trained staff group offering consistency of care and enthusiasm to maximise the quality of life for the service users. Service users are further protected by good recruitment and selection practices. This judgement has been made using available evidence including a visit to this service. EVIDENCE: An inspection of current and recent rotas confirm that staff numbers across the 24hour period are appropriate to the needs of service users. An on-call system of senior staff is provided to support the lower staffing numbers overnight. The current level of NVQ qualified is 72 with many also holding level 3 qualification. Thirteen staff are engaged in either level 2 training or advanced courses. A sample of three staff files including a recently employed staff, show these to be completed to a good standard with appropriate pre-employment checks being undertaken. Job descriptions appeared satisfactory for roles and support the underlying values of the home. Offmore Residential Care Home for the Elderly DS0000069706.V350604.R01.S.doc Version 5.2 Page 19 Staff are subject to a thorough, and relevant orientation/skills for care induction programme, which is followed by comprehensive ‘foundation’ training, e.g. ‘manual handling and lifting’, ‘fire safety’, ‘infection control’. Staff are issued with the General Social Care Councils code of conduct. Staff each have a training file and have a training needs analysis each year, the files show that all mandatory training provided is up to date and certificates are kept on the individual file. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): -Standards 31 – 38 Quality in this outcome area is good. Leadership of this home is good and staff demonstrate an awareness of their roles and responsibilities and service users benefit from this consistency. The ambience of the Home is warm, friendly, and inclusive. Sound financial management of the home and arrangements for safekeeping of their money safeguards service users interests personal and financial. Staff receive supervision and direction to ensure that the service users receive consistent quality care. The best interests of service users are safeguarded by the homes record keeping, policies and procedures. Environment management and staff training in respect of health and safety ensures service users safety and welfare are protected. She holds the registered managers award and is keeping up to date by undertaking the NVQ management qualification. Staff at the home have mostly been in post for many years, are well motivated and experienced. There has been no drift of staff since the change of ownership and manager. Morale was observed to be good and staff spoke with the inspector said that it was a very good place to work and relationships among the staff helpful and considerate to one another. The home uses the Croners’ Care Home quality assurance process and this is presently underway including surveys of service users and relatives views and other stakeholders. Accounting and financial procedures of the company were checked at the time of registration during the summer. The home does not take responsibility for service users money but do provide for safekeeping of money and valuables with appropriate records. Supervision is well established and numbers met, topics are appropriate covering aspects of care delivery, the philosophy that the new provider brings to the home and individual development. Staff meetings are established and minuted the last was held during November. Records viewed during this inspection were found to be up to date and accurate. The case files evidence the involvement of service users in care planning and their access to the information held in the file. Arrangements for keeping records secure are satisfactory and comply with requirements. Documentation was seen of a full range of servicing, maintenance and regular monitoring of services and equipment is undertaken, staff receive training in health and safety and first aid. Monitoring of hot water is good and records show that the standard of 43°c is achieved consistently, anti scald testing and legionella prevention are also undertaken. Fire safety including tests, staff training and equipment maintenance were seen to be up to date. Records of all accidents are recorded, using data protection compliant documentation and reported as necessary. Induction and foundation training is provided to Skills for Care standards. Overall the premises were observed to be well managed to meet safety requirements.
